TheRegistryofBirths,DeathsandMarriageswillsupplyacopyofabirthcertificatetoappropriatepeopleonrequest.(RefertoCertificateAccessPolicy)

Ifyouareapplyingforyourown certificateoracertificatefor your child(undertheageof18years),threeformsofidentificationforyourselffromTable1mustbesupplied.Photocopiesshouldbecertifiedbyaqualifiedwitness*asbeing“acorrectcopyofthedocument”.Youmustprovide:

• oneformofidentificationfromeachlist

• ifyouareunabletoprovideidentificationfromList1youmustprovidetwoformsofidentificationfromList2andoneformofidentificationfromList3.
