Sir,

Please provide me the information on recruitment of District Epidemiologists under Integrated DiseaseSurveillance Project (IDSP) in State of Punjab till date 5/0412011. Please include the following points in theinformation:

l. Provide information (with certified photocopies of document/reports/minutes of meetings, file notingalong with Name, Designation of officers/members participated in meetings/discussions) of allmeetings/discussions held for the recruitment process of district epidemiologists.

'2. Provide the certified photocopies of document/reports/orders/minutes of meetings/ file noting preparedfor the categorization of pay-package of district epidemiorogists.

3. Give the details of total number of created posts of district epidemiologist along with the photocopies ofnotification/advertisement for the same till date 5/0412011.

4. Provide the attested copies of the Performa submitted by the candidates to the IDSp punjab, regardingthe identification details, qualifications and choice of centres/districts on the date of interview held onl4e January ,2011for the of the posts of district epidemiologist.

5. Provide the certified copies of selected candidate's lists for the post of district epidemiologist by IDSpPunjab till date 510412011.

6. Provide the attested photocopies ofjob offer letters dispatched to the selected candidates for the posts ofdistrict epidemiologisr by IDSp punjab till date 5104/Z0ll.

7. Provide the attested copies of joining reports of seleoted candidates for the posts of districtepidemiologist in Punjab till date 5104/2011.

8. Provide the attested photocopies ofjob rejection/resignation letters/mails submitted to the IDSp punjab

by selectedlappointed district eBidemiologist till date 5/a4r20ll.

9. Provide the certified copies of waiting lists prepared for the post of district epidenniologist by IDSpPunjab tilf dare 5/04120N1.

10. Give the details (including name, age, sex, education qualification, work experience, office address, date

of duty joining, monthly pay- package along with date of duty leaving if any) of the persons

employed/selected as district epidemiologist till date 510412011.

I l. Give the details of the curent status of vacant posts of district epidemiologist along with the date of

vacancies created.

12. Give the details of plans/policies/programs, implemented and under process for the fulfillment of the

vacant posts of epidemiologist

The information should be given according to the above stated points. All points are necessary for the

informatioh. All points of information are concerned with health department Punjab so please collect the

information from concerned authorities /units and send it to me. If information regarding to some points is

not fall within the health department Punjab then give the details of the department from where information

can be obtained. Please check the Rule, regulation and concemed Judgments of Central Information

Commission before giving any excuse regarding Third party information because all the points are made

after considering the RTI Act and Judgment of Central Information Commission and according to my

knowledge all fall in the Jurisdiction of RTI Act and should be provided to me.
