Choose “Mortality-underlying cause of death”
Go to the website: http://wonder.cdc.gov

Choose Mortality for 1979-1998 with ICD 9 Codes

Choose the Following Options:
1. Organize Table Layout:• Group Results By = County
2. Select location • States = New Jersey
3. Select years and demographics• All Ages, • Years (1989-1998), • All Genders, • All Races

4. Select cause of death: ICD 9 Codes•460-519 (Diseases of the respiratory system) and open detail category
●Asthma: 493
5. Select rate options: •Calculate Rates per “100,000”•Age-Adjusted Rate Options
●Use standard age-adjusted rates●2000 US Std. Population
6. Other Options, select: •Export Results•Show Zero Values•Precision = 2 decimal places
Click “Send”

You can save text file and open with Excel

When you open the file with Excel, you will see the following window. • Choose “delimited” • Click “next” • Click “finish”

Be careful for the following items:-Make sure you change variable names (less than 9 characters)-Make sure you put COCODE as text data (not numeric data)-Make sure you define “2 decimal” numeric format for ASTRATE variable.
Once you save “asthma.dbf” file you can join the data with NJ County boundaries and create a map.
Edit the information in the first excel spreadsheet to look like the second spreadsheet
