She also has a small mass on the back of the right shoulder which has a remarkable increase in contrast uptake on nuclear scan. She has a previous history of an excision of a thyroid mass done at a private institution and the biopsy result was that of a Chondrosarcoma. . . . The gross, histological features and immunohistochemical profile of this tumour are those of a extraskeletal mesenchymal chondrosarcoma. An incisional biopsy of the left thigh mass was performed and the result was a mesenchymal chondrosarcoma. Then an excisional biopsy was done. Immunohistochemistry S100 - Positive Vimentin - Positive; Cytokeratin AE/13 - Negative. . Microscopic findings The sections show a diffuse proliferation of pleomorphic small, round to oval cells. In some areas, a haemangiopericytoma-like growth pattern is present. Cartilagenious areas with mild pleomorphism of chondrocytes in the lacunae and calcifications are present. Approximately two thirds of cases of mesenchymal chondrosarcoma occur in bone while the rest occur in places outside of the bone—i.e.in extraskeletal locations. They are aggressive lesions with a high propensity for metastases. Mesenchymal chondrosarcoma is a fast growing tumor that spreads more often. They can remain dormant for long periods of time. This case illustrates multi- focality, recurrence and metachronous nature of a extraskeletal mesenchymal chondrosarcoma.
