Request For Estimate Fill out Service Request Form, which can be accessed at: http://facilities-mgmt.ndsu.nodak.edu/maint_const/servreq.pdf Requires departmental funding. Requestor will be contacted by the assigned Project Manager in the

order in which the requests are received. Project Manager calculates estimate, and requestor will be mailed a

“budget estimate.” After departmental approval, return signed copy of estimate form to

Facilities Management and your project will be placed in project queue.

Facilities Asset Management Information Software – Facilities Management Software which is an ancillary system to PeopleSoft.

Billing is through an interface to PeopleSoft. Departments can request access to Discoverer Reports, which will then provide the detail of the charges in a particular billing cycle:http://facilities-mgmt.ndsu.nodak.edu/famis/index.html

• These forms exist to collect the necessary information needed to maintain the University's facility space inventory. The information is updated on the FAMIS Facilities database for Space Use Analysis through the use of these records. This database provides information for campus planning; grant applications, building renovations, and ensuring building code compliance and utility usage. Please refer to the Space Move Authorization Form Instructions regarding whether to use Form A or Form B.

• Form A. When moving personnel from one room to another within the same department fill out the INTRA-DEPARTMENTAL MOVE FORM (ex: English dept. move personnel room 109 to room 229).

Phone in State Fleet vehicle requests to 231-9619, hours of operation 7:30 am to 4:00 pm

Faculty, Staff, and Students must submit Reservation Form for billing purposes prior to vehicle release. Complete key request form with the following required information: NAME EMPL ID BUILDING ROOM NUMBER KEY CODE

Building Key Control Official must sign off on all key requests It is the policy of the Facilities Management Department to issue no more

than one key per person for each lock. If a lock needs re-keying, also complete a service request form.
