A person must have a disability and unable to utilize fixed-route transportation. A person requires a ramp or wheelchair lift to board or exit public transit vehicles

In-Home Care Adult Services – Services that help older adults and adults with disabilities remain in their homes. In-home services includes housekeeping, laundry and bathing.

Eligibility based on assessment, and income eligibility (70%) of the median area income.

Nursing Homes and Assisted Living

Birmingham Green – Nursing home facility jointly owned with other jurisdictions. Fairfax has 92 beds.

Willow Oaks & Willow Arms – Assisted living facilities jointly owned with other jurisdictions. Fairfax has 32 beds.

Lincolnia Assisted Living – 52 bed facility for individuals 55 and over who can’t live alone.

Fairfax County residents. Must meet Medicaid functional and financial eligibility criteria.

Volunteer Solutions Transports Meals on Wheels, assists with grocery shopping, household maintenance, medical appointments. Conducts social visits, and recruits volunteers for Senior Centers and Adult Day Health.

Medicaid Waiver and Nursing Home Preadmission screenings are conducted by a Social Worker and a Public Health Nurse.

Assisted Living Facility screenings are conducted by a Social Worker.All screenings are conducted by the Local Dept. of Social Services.

Medicaid Waiver screening: Conducted to assess an individual’s functional eligibility for community-based services.

Nursing Home screening: Conducted to assess an individual’s functional eligibility for nursing home placement.

Assisted Living Facility Preadmission screeningConducted to assess an individual’s functional eligibility for residential or regular assisted living.

Must meet Medicaid nursing home functional criteria and Medicaid financial eligibility criteria.
