Dialysis Rounding on Desktop App Hemo, PD and HHD

INTRODUCTIONLog in

Go to https://falconehr.davita.com/ehr/ and enter the login information you received in an email message from your Falcon Physician team.

Hemo Encounter

1. In the Falcon Dashboard > Patient Locator, click to select theHemo filter, select the center, select the date, and so forthfrom the drop-down menus.

2. Select a red (Not started) or yellow (In process) button in theLimited Visits: or Comp. Visit: column to open an encounter.

3. The Create New Note window opens. Provider: defaults tothe provider logged in. If you are not a provider select providerfrom the Provider: drop-down.

4. The Encounter Date: automatically fills with today’s date.

5. Click the Create button.

6. Select the Template.

7. You may complete the encounter note, and then clickSign and Finalize.

PD or HHD Encounter

1. In the Falcon Dashboard > Patient Locator select thePD/HHD filter, select the center, select the date, and soforth from the dropdown menus.

2. Click the red circle icon in the patient row to go to eChartingto begin the PD/HHD encounter.

3. You may complete the encounter note, and then clickSign and Finalize.

On the Dashboard, the Limited Visit or Comprehensive Visit button that was selected earlier turns green, indicating the visit is completed.

Create a Patient Portal PIN

Charge Summary Report

Care Team Notes

The notes created by members of the Care Team in Falcon Dialysis appear in the Patient Chart tab in Falcon Physician. The notes are read-only.

1. Select Main Menu > Patient Manager > Demographics.

2. Click the Generate PIN button.

1. From the main menu, click Charge Summary. Enter the month and year. The report may be run selecting one Provider, or for allPractice’s Providers by selecting the All checkbox. Leave the Transmit Method set to the default value: View. Click Run Report to display the report.

2. To download the report to an Excel file, select Download fromthe Transmit Method dropdown menu, select the patients toinclude: Select all patients by clicking the check box on theHeading Row, or Select individual patients by clicking thecheck boxes on the same rows as the patients’ names

3. Click Run Report. You may then save the report as an Excel File

The PIN will populate in Patient Demographics, which may be given to that patient, and the Patient Portal welcome letter will appear, which may then be printed and given to that patient. This letter instructs the patient on how to access the Patient Portal, and can also be viewed and reprinted from Patient Manager > Documents.

Disclaimer:

Set to Normal and Snippets should only be used by physicians to apply to their dialysis notes during a patient visit that accurately reflect and represent the care and observations from that specific patient visit. Set to Normal and Snippets should not be applied to any note without careful review. All Set to Normal settings and Snippets must be carefully reviewed before storing, and thoroughly evaluated for appropriateness of later use.

DOWNLOAD THE APP

GETTING STARTED

1. Open the iTunes App Store located on your iPad home screen

2. Search for Falcon Rounding by typing in the search bar.

1. Open the application and tap the Falcon icon to launch the App

2. Log In Not a current Falcon user?

3. Accept or Decline our Terms of Service:

Scroll to the end and tap Accept or Decline. If you decline the terms of service you will not be able to use the App. You will be able to select options once the words Accept and Decline are highlighted blue.

OVERVIEW AND NAVIGATION

Select a Center

A user will be required to select a center if not using geo- locations. The Please select a location to continue… pop-up window will display if a Location has not yet been selected. You may choose the location in that window.

Or if you tap the screen and that window closes, tap the Select Location link, then tap the location from the drop-down list.

After selected that center will remain the default center until the user selects another center or if geo-locate is then used to select another.

Tap Close Filters in the upper left corner.

Not a current Falcon user?

Enroll at by visiting www.DaVitaRounding.com to receive a username and password.

Existing Falcon user?

Your username and password were emailed to the address provided during enrollment.

START AN ENCOUNTER

NAVIGATION IN AN ENCOUNTER

Navigation icons display at the bottom of the screens.

In the Schedule, tap a red or yellow circle to start a limited or comprehensive visit.

Stages of an Encounter:• Red = Not Started• Yellow = In Progress• Green = Signed and Finalized

Tap to navigate between CHART, LABS, and NOTE.• You may tap to Set to Normal as applicable,

• Tap to select an option.

• Tap to edit.

• Switches: To turn a switch on or off you may tap the toggle.Blue indicates that the switch is on.

Filter By Options:• Time Frame• Modality Type• Patient List• Location• Day of Week• Shift• Sort By (Tx Time Left or Alphabetically)

You can filter your schedule by clicking in the top left corner.

CREATING SNIPPETS

ORDER SIGNING IN FALCON PHYSICIAN ON IPAD!

Tap Add to chart for additional options. You may then tap the option and complete.

• Add New Rx• Add Home Medication• Add Problems• Add Allergies

Within an encounter, tap in a text box to create, remove or edit a Snippet.

Example:

An All Orders navigation icon is available on the bottom navigation bar.

• Show More Information: Expand a selection to read moreinformation by clicking the arrow icon.

• Displaying and Selecting Alternatives: A checkmark indicatesthe selection chosen. Tap on the desired text to select adifferent option.

• Actionable Text: Actionable text within the application,meaning you can select or edit the information, will behighlighted blue.

Falcon Mobile Quick Start Guide for iPhone

Download the app

Ready to use

Touch ID

1. Open iTunes App Store located on your iPhone home screen.

2. Search for Falcon Physician.

Tap “Get” and enter your iTunes account information to complete the download.

If you don’t know your iTunes password:• Visit: https://appleid.apple.com/• Click “Reset Your Password” and follow instructions

1. Falcon appears as an icon on your phone. Tap the icon tolaunch App, then Log In.

2. Accept or Decline our Terms of Service.

Scroll to tap Accept or Decline: • If you decline the terms of service, you will not be able to

use the App.• You will be able to select options once the words Accept and

Decline are highlighted blue.

3. Understand Falcon Mobile Capabilities.• Touch ID• View Patient Summary• ePrescribe• Orders• Charge Capture

1. If you would like to use Touch ID to sign orders, first set up thefunctionality on your iPhone under Settings.

2. Log into Falcon Mobile and enable the feature in the Settingsscreen. When you return to the login screen, if you enteredyour username, it should be retained you will be prompted tologin with Touch ID.

View Patient Summary

ePrescribe

1. Tap Patients Search and then type a specific patient’s namein the Search by Patient Name box.

3. Tap to select a patient. Tap a section of the chart to displaya view only summary.

2. You can also tap Patients from the bottom bar to access thepatient list.

• Patients are listed in theorder of most recentlyviewed

• Scroll through list or tapin search box to typepatient name.

You can view:

• Demographics, Labs, BP & Fluid Management,Problems, Medications, Allergies, CulturesVaccinations and Access

1. Tap ePrescribe to access the list of patient’s pendingprescriptions from DaVita dialysis applications.

Note: Prescriptions originate from DaVita Dialysis applications.

2. Tap Patient name to access the Prescription Detail screen.

You may select to:• Deny/Approve - New Prescriptions, Discontinue Prescriptions,

Formulary Exception Requests, Note for Physician Consideration, Renew Prescription

• Deny/Resend - Pharmacy Refill Requests, Pharmacy ChangeRequest, Generic, Therapeutic, and Prior Authorization

3. If approved, the Prescription submitted message appears.

5. If a pharmacy sends a Pharmacy Refill Request or PharmacyChange Request for a patient who does not automaticallymatch with a patient in the system, you may be promptedto select a patient from a list of possible matches.

4. If denied, the action is completed and the updated patientprescription list screen appears updating the number ofprescriptions. To use the filter function, select Filter.

You may select to:• Filter – filter prescriptions to see All or Pharmacy refills• Back/Done – return to the list

You may:• Tap Patient Match Required button• Select the patient name on the next screen under the Matching

Patient list• Click Verify as applicable and then Approve or Deny that

prescription

Note: If the patient is not listed in the Matching Patient List the user may select Deny and then select a reason to send the message to the Pharmacy.

Orders

1. Tap Orders to access the list of orders. 2. The screen displays a list of 20 orders at a time. Tap the arrowto forward to the next 20 orders. To sign multiple orders, scrollthrough the list to the bottom. The dark blue Sign All bar willbecome a lighter shade of blue allowing you tap to sign all20 orders.

3. The first time you sign orders in the session, you will beprompted to enter your password or you can use Touch ID,if you have enabled this feature.

5. Tap the check box next to the order to select it and thentap at the bottom to Dispute Selected or Sign Selected.

7. Once you have selected to Sign or Dispute, you will return to the updated All Orders list.

4. To sign or dispute one order at a time, tap Select to viewindividual orders.

6. If you tap Dispute Selected, a list of choices appears.Make your selection or tap Cancel to return to the Order.

Charge Capture Settings

1. Hospital Charge Capture is avail-able for Gold and Platinum customers for an additional monthly fee. Tap to see the demo or contact Falcon Physician.

1. The Settings screen allows to you change your password,set the Touch ID feature, and to Log Out.

View Patient Summary

ePrescribe

1. Tap Patients Search and then type a specific patient’s name in the Search by Patient Name box.

3. Tap to select a patient. Tap a section of the chart to display a view only summary.

4. Tap the sections of the chart you would like to view. Tap the tabs at the top or swipe to access another module.

2. You can also tap Patients from the menu to access the patient list.

• Patients are listed in the order of most recently viewed.

• Scroll through list or tap in search box to type patient name.

You can view:• Demographics, Hemodialysis Treatment, PD Treatment Orders, Labs, BP & Fluid Management, Problems, Medications, Allergies, Cultures Vaccinations, Access and Clinical Study
