___ Complete and PASS the Pharmacy Technician Training program (including Externship)

___ Turn in your High School Transcripts / GED Equivalent / Before externship begins (International Transcripts – please see Christine)

___ Verification of age 18 by beginning of your externship

___ US Citizen or Student Visa to remain for duration of program and externship. For LICENSING you must be a USA citizen or a permanent resident. (Social Security No.)

___ You will have a Live Scan fingerprinting on campus once you start your externship class

___ Clear Drug Screening (required for most Externships and ALL PAID PHARMACY JOBS)

___ Self-Query – Online Background Verification(do this in class)

___ Board of Pharmacy application

How Do I Get My California Pharmacy Technician License?

Payment Options (1) Full Certificate Program - $3,300 + books, materials, scrubs & fees

Pay in full tonight or by Tuesday, Oct. 8

(2) Three Installment Payments - $3,550 + books,materials, scrubs &fees.1st installment - $1,450 – pay tonight or by Tues, Oct. 82nd installment - $1,450 – due by Jan. 13, 20143rd installment - $650 – due by May 5, 2014
