The course(s) shall count for credit toward the Student’s high school graduation requirements. 3. Only college courses that apply toward a college degree, certificate, or 090 level basic skills courses (for 12 th graders only), are covered under the Concurrent Enrollment Program. 4. The Student must demonstrate college readiness as defined by the institution of higher learning and D20. 5. In the college application process, the Student must agree to apply for the College Opportunity Fund (COF). This establishes eligibility for the state tuition stipend that reduces a Student’s tuition costs before enrolling in any Concurrent Enrollment course. (N/A for UCCS Extended Studies and International Salon and Spa Academy) 6. The Student must authorize use of his/her COF stipend for all eligible credits for the semester stated above as defined by the institution of higher learning. College level credits used will be deducted from the Student’s COF lifetime account (145 credit hours) with the exception of basic skills courses. 7. The actual grade received in each Concurrent Enrollment course will appear on the Student’s official high school and college transcript College course credits may transfer in congruence with Colorado Guaranteed Pathways. The Student and his/her Parent/Guardian are responsible for researching the transferability of college credit to subsequent institutions of higher learning. 8. If the Student seeks to add, drop, or withdraw from a college course, he/she must meet with his/her high school counselor and notify the college Concurrent Enrollment staff. 9. If the Student receives a grade of F, Incomplete, or withdraws from a course after the college drop deadline, the Student and the Student’s Parent/Guardian will be required to pay D20 the tuition initially paid on the Student’s behalf. Any outstanding tuition will be added to the Student’s fees. The F/WF grade will appear on the student’s high school and college transcripts. 10. In compliance with the Family Educational Rights and Privacy Act (FERPA) of 1974, the Student agrees to allow information to be shared between the college and D20 as it relates to the successful completion of the courses covered under the Concurrent Enrollment Program. The Student/Parent give D20 permission to release disability specific information or education plans to the college. 11. For an approved Student, D20 shall only pay, directly to the institution of higher learning, the prevailing community college on-campus tuition rate with COF applied and other fees applicable to the program. This rate applies for online and on-campus courses whether taken at a 2 year, 4 year, or vocational college (including Extended Studies) and does not include fees, books, supplies, or any tuition costs in excess of the rate mentioned above. 12. The Student shall pay to the institution of higher learning any additional costs not covered by D20 to include campus fees, course fees, books, supplies, and tuition beyond what is covered by D20 as outlined in item 11. 13. The Student is responsible for applying for admission with the college, registering for college courses, and monitoring progress. 14. The Student and parent/guardian acknowledge concurrent enrollment classes are taught by college/university faculty and, therefore, are not taught or supervised by D20 Colorado licensed teachers or administrators. 15. Courses are reviewed for high school credit type and weighted status, and they are posted to the Student’s transcript accordingly.

Concurrent Enrollment (CE) Programs

Career Start/ISSA Student Agreement

The purpose of this agreement is to clarify the student responsibilities for participating in district Concurrent Enrollment (CE)

Programs.

The student agrees to:

1. Remain in the program for a full school year. The student’s high school counselor must approve any schedule changes. Please note! Continuation in the program for the 2nd semester is contingent upon successfully passing the 1st semester. Likewise, students wishing to apply for the 2nd year of a 2-year program must have successfully completed year 1.

2. Complete a transportation agreement. This form identifies how the student plans to get to and from the CTE program. Advise the high school counselor during the year if transportation needs change.

3. Observe the student attendance policy for CE programs. Five absences in any one quarter are considered excessive and

may result in the withdrawal of the student from the program. Note: When calculating absences, 4 tardies equals one

absence. “School related absences,” such as those for testing, athletic participation or school closures are not included in the

limit but must be validated by a counselor or district official. Program withdrawal may result in a loss of credits and thereby

jeopardize high school graduation and may require repayment of tuition to the district.

5. Attend CE programs on all days for which District 20 provides transportation. CE programs may be in session on days

when other District 20 students do not have school. Please refer to the calendar that the student will receive prior to the start

of the school year.

6. Schedule doctor’s appointments, etc. after school whenever possible. Please make every effort to avoid missing CE

programs. Career and Technical courses are approximately 160 minutes in length, the equivalent of almost 2 block classes

in the student’s home high school.

7. Agree to pay for and obtain immunizations or background checks. Please check with the counselor for the requirements

for specific programs. (i.e., Hepatitis B series for medical prep).

8. Abide by the rules of conduct under the student’s home high school. See the High School Student Handbook and those of

the Career and Technical Education course (provided at orientation). Conduct oneself in a manner that reflects personal and

district pride.

9. Complete payment of the $75.00 application fee. The Career Start/ISSA application fee can be paid via check made out

to ASD20 and attached to this application, or it can be paid via PayforIt after the application has been received. The fee is

non-refundable if the student is accepted into any concurrent enrollment program.

Career Start/ISSA Student Transportation Agreement

If you wish to allow your student to drive his/her own vehicle at any time during the school year to PPCC or ISSA please read and sign below.

I give permission for my student, to drive his/her own vehicle to the morning and/or afternoon courses he/she attends. I understand that if this privilege is abused, the district will require the student to ride the school bus. Senate Bill 05-36, prohibits a minor driver who has held a license for less than six months from transporting a passenger who is under 21 years of age and who is not a member of the driver’s immediate family. Furthermore, a minor driver who has held a license for less than one year may not transport more than one passenger who is under 21 years of age. I understand that this prohibition applies to minor drivers attending Career and Technical courses. I further understand that the district does not monitor the occupants of privately driven vehicles including those containing students who may elect to carpool. I am giving my permission for my student to participate as a driver or passenger in a carpool in accordance with Colorado Law as it applies to newly licensed drivers.
