Dr. Schubert is proud to introduce MDVIP to the Tri-Cities. A couple years ago, he converted his traditional primary care practice to an MDVIP practice. MDVIP is a national company that helps manage small practices with an emphasis on personalized care. It has around 850 doctors across the United States. While this system has been growing across the US since 2000, Dr. Schubert is the first to introduce it to this area.

Three years later, he has been very pleased with the results. “It’s new here, so people aren’t famil-iar with it. But it’s really pretty simple – it’s just old time medicine, with some new technology thrown in. Instead of several thousand patients, I have a few hundred. I’m available to them per-sonally whenever they need me. We do a very thorough physical every year, which includes some of the new technology I was talking about – advanced diagnostics like a detailed cardio-vascular risk panel (the bloodwork is sent to The Cleveland Clinic), plus EKG, a urine sample, lung testing, and so on. There is an annual fee to pay for the physical. It takes a few hours to do all this testing and to go over all of it with me in my office, but we get some great information. After reviewing your information, we discuss a plan to improve on whatever problems we find, and work together over time toward achieving those goals. Plus, we have a network of special-ists in places like Mayo Clinic, Johns Hopkins, Sloan Kettering and so on available if we need them. Current patients as well as new patients are allowed to bring their children to the office as long as the children still live in the home and are under age 26. Again, it’s old time medicine. I know my patients, my patients know me, and if something is wrong, they just call me. Simple.”

Dr. Schubert says that this type of practice is his dream job. “It’s so much better. My patients like the fact that they don’t have to wait – we’re wait-ing for them. They like that I have time to focus on their problem and if other things come up, that’s ok – we finish our appointment when we’re through, not when I have to leave. I love my new practice, and my patients love it even more!”

Dr. Schubert has been practicing medicine in Elizabethton for almost 20 years and has seen many changes during that time. “Some of the advances we’ve made in medicine have been remarkable. The diagnostics have improved, some of the new drugs are safer and more effective, genetics has come a long way…but despite all of our scientific successes, we’ve lost track of what’s most important in medicine – the relationship between the patient and the physi-cian. Various changes in health care laws and insurance policies have changed medical offices into factories – we run people through like cattle. Overall I think a better model is the one we had before – you have a doctor in charge of you. When you get sick, you call your doctor (not an answering service) or go see your doctor (not a “provider” who works in the office) and your doctor takes care of your problem. You know your doctor, your doctor knows you, the visits aren’t rushed. Like it used to be in the old days. Maybe they were on to some-thing there.”

He also thinks he understands why our health care system has evolved this way to a certain extent. The pursuit of reducing costs has led to an emphasis on speed and efficiency, which has led him to personally see downsides to this mindset. “Let’s say the doctor doesn’t know the patient very well. He’s in a hurry or he’s distracted by computers or other added complexities, then it is really hard to be thorough. You tend to focus on the problem at hand, lose track of some of the details that may not be obvious at the time, and over time the quality of the medicine inevitably goes down. Bad medicine is expensive medicine. The less effective we are at finding and treating disease means an increase in the amount of sick people, which makes healthcare more expensive in the long run.”

That is why he loves his MDVIP practice so much. “I don’t see 40 patients a day – I see 8-10 or so. I have a small group of patients, and I am responsible for them. No more waiting for days to get an appointment. All my patients have my personal cell phone number, so if somebody gets sick, I know about it right away – everything is easier to treat from the beginning. Plus, with 30-90 minute appointments, I have time to focus more on prevention, so I can try to keep people healthy, rather than just waiting for them to get sick. My old prac-tice was chaos – I couldn’t keep track of what was going on. This is just a better system. My patients love it!”

The Johnson City Eye Clinic was established in 1942, evolving into what was to become the renowned Johnson City Eye Hospital. Think you’re allergic to penicillin? Odds are good that you aren’t. Allergists urge testing to be sure.

Does your medical chart read ‘Penicillin allergy?’ Penicillin is one of the most important antibiotics doctors can prescribe for ear, sinus, chest, throat and skin infections. Penicillin has many advantages – it’s often the best drug to treat infections, is safe to use during pregnancy and breastfeeding, it’s well tolerated in children and is very economical.

Of all the drug allergies, penicillin allergy is the most common. “Many people, after having experienced a minor reaction to penicillin, are told by their physicians that they are allergic when it has never been clinically established, or the patient assumes he or she is allergic,” said allergist Alexei Gonzalez, Quillen ETSU physicians “That said, a small portion of the population is allergic to the drug.” Allergic symptoms vary from a mild skin rash to a severe chain reaction within the body called anaphylaxis, which can be fatal.

Doctors tend to err on the side of caution and note

it on your medical record. Even when the initial allergic response is minor, subsequent exposure to the antibiotic can trigger a severe life-threatening anaphylactic reaction. If your doctor is unable to prescribe penicillin for you, it restricts your medication choices. Less effective or more expensive antibiotics may be needed to deal with common infections.

Over the years, Dr. Alexei Gonzalez has seen patients reporting penicillin allergies in a number of ways. “I’ve had patients come in and report an allergy because they had a reaction when they were very young and their parent told them they were allergic. Other patients worry that they may have inherited a penicillin allergy from a family member, or are confused about the difference between penicillin’s side effects and a true allergic reaction.”

If you think you have a penicillin allergy, consider seeing an allergist for testing 90 percent of patients tested are found not to be allergic. The standard test for penicillin allergy involves skin-prick testing (introducing the allergen by scratching the skin) and intradermal testing (injecting the allergen just beneath the skin).

A positive result would confi rm that you should not take penicillin. A negative result may be followed by a controlled test in your allergist’s offi ce with oral dose(s) of penicillin to truly ensure that you are not allergic to the drug.

“Anyone who thinks they may have had an allergic reaction to penicillin should schedule a consultation with a board-certifi ed allergist,” said Dr. Alexei Gonzalez. “In most cases, the ‘penicillin allergy’ label can be removed from your chart.”

No one wants to hear from their doctors that they have joined the millions of people across the globe to be diagnosed with heart disease. The Heart Foundation reports that heart disease, which includes diseases of the heart and cardiovascular system and stroke, is the No. 1 cause of death in the United States, affecting both men and women and most racial/ethnic groups. Heart disease also is one of the leading causes of death in Canada, claiming more than 33,000 lives per year.

Many factors contribute to the development of heart disease, including smoking, lack of exercise and stress. Diet and whether a person is overweight or obese also can have a direct link to heart health. Diet, particularly for those with diabetes and poorly controlled blood sugar levels, is a major concern.

A variety of foods are considered helpful for maintaining a strong and healthy heart and cardiovascular system, while others can contribute to conditions that may eventually lead to cardiovascular disease or cardiac arrest. Moderation enables a person to sample a little of everything, but not to make any one food a habit. The fol-lowing are some foods to promote heart health and some foods you might want to avoid.

GOOD• Tree nuts: Tree nuts contain unsaturated fats that can help lower LDL cholesterol (the bad stuff) and improve HDL (the good stuff). Nuts also are a filling source of protein and other healthy nutrients.

• Whole grains: Whole grains contain complex carbohydrates for energy, as well as protein and fiber. Fiber can help scrub cholesterol from the blood, lowering bad cholesterol levels.

• Fatty fish: Many cold-water, fatty fish, such as halibut, herring and salmon, contain omega-3 fatty acids, which are heart-healthy. Ome-ga-3s also can be found in walnuts, flaxseed and some soy products.

• Beans: Beans and other legumes are an excellent source of protein and can be a stand-in for meats that are high in saturated fat. Beans also contain cholesterol-lowering soluble fiber and folate, which can reduce blood homocystein levels. The Bean Institute reports that consuming beans may reduce cholesterol levels by roughly six to 10 percent.

• Yogurt: Researchers in Japan found yogurt may protect against gum disease. Left untreated, gum disease may elevate a person’s risk for heart disease. Yogurt contains good bacteria that can coun-teract bad bacteria and boost immunity.

• Raisins: Raisins contain antioxidants that may help reduce inflam-mation. Inflammation is often linked to heart disease and other

debilitating conditions. Fresh produce also is a good source of anti-oxidants.

POOR• Fried foods: Many fried foods have little nutritional value, as they tend to be high in saturated and trans fats. French fries are particu-larly bad because they are carbohydrates fried and then doused in salt.

• Sausage: Processed meats have frequently earned a bad reputa-tion among cardiologists, but sausage can be a big offender, due in large part to its high saturated fat content.

• Red meats: Enjoying a steak is probably not as bad as eating a deep-fried brownie, but it’s best to limit red meat consumption to about 10 percent or less of your diet. Red meats can have a consid-erable amount of cholesterol, saturated fat and calories.

• Added sugars: Sugar can increase blood pressure and triglyceride levels. Sugar often hides out in foods that you would not associate with the sweetener. Plus, many people unwittingly consume too much sugar simply through sugar-sweetened beverages and ready-to-eat cereals.

• Salty foods: Leave the salt shaker in the spice cabinet and opt for herbs for flavoring, advises the American Heart Association. High-sodium diets often are to blame for hypertension, a major risk factor for heart disease.

• Dairy: Artery-clogging saturated fat also can be found in dairy products, particularly the full-fat versions. Butter, sour cream and milk can be problematic when people overindulge. Opt for low-fat dairy when possible.

Stress has an immediate and potentially long-term effect on the human body. Though it’s a natural response to both good and bad experiences, stress, when chronic, can produce a host of negative consequences that greatly diminish one’s quality of life.

Combatting stress can sometimes be difficult, as the causes of stress are never too far away for many adults. In its 2015 “Stress in AmericaTM: Paying With Our Health” survey, the American Psycho-logical Association found that money is the top cause of stress for Americans. The survey was conducted on behalf of the APA by Harris Poll, which asked more than 3,000 participants about their issues with stress. Sixty-four percent said money was a somewhat or very significant source of stress, and that number was even higher for parents (77 percent). Survey respondents also noted that work is a significant source of stress.

Few adults can imagine a life that does not include financial- or work-related stress. But there are ways to combat stress that can benefit people’s long-term health and improve their present-day quality of life.

• Develop a support network. Speaking about problems with trusted friends and family members can be an effective way to combat stress. The APA study found that participants who reported having an emotional support network reported lower stress levels than those who had no such networks to rely on. Try to overcome any reticence you might have about speaking about your stress to a close friend or trusted relative on those days when stress seems overwhelming.

• Get more exercise. Routine exercise is another healthy way to combat stress. According to the Anxiety and Depression Association of America, studies have shown that exercise can reduce fatigue, improve alertness and concentration and enhance overall cognitive function. Those are considerable benefits to people dealing with elevated levels of stress, which can contribute to both physical and mental fatigue and negatively impact one’s ability to concentrate. Studies also have shown that regular exercise can decrease tension, which tends to increase as stress levels rise, and elevate and help to stabilize mood, which often decreases as stress levels increase.

• Don’t lean on alcohol after stressful days. Many people respond to stressful days by consuming alcohol. While alcohol helps some people forget a stressful day, it also produces psychological and physiological side effects that can compound the effects of the very stress drinkers are looking to relieve. People who drink to alleviate stress may only be doing more harm with each drink, so find a way to cope with stress that has a more positive impact on both your body and mind than that produced by alcohol.

• Breathe deeply. The American Institute of Stress notes that focused breathing is a relaxation response that stimulates the nervous sys-tem and promotes a sense of calmness. Deep breathing can combat stress, lower blood pressure and draw your attention away from those things that are causing your stress. Visit www.stress.org to learn about deep breathing exercises.

Physical Therapy Services, P.A. has been serving the East Tennes-see, Western NC region since 1980. Currently, there are three offices available to the public, with or without physician referral. They are located in Elizabethton at 1975 W. Elk Ave, 1500 W. Elk Ave, and 401 East Main St. in Johnson City. Occupational therapy is offered in the 1500 W. Elk location, as well as the Johnson City location.

Best know over the years for their care and rehabilitation of regional athletes, Physical Therapy Services continues to offer sideline and courtside coverage for athletic events, sports injury clinics, edu-cational programs for area coaches at all levels and sport specific rehabilitation services.

Dr. Danny D. Smith, PT, DHSc, OCS, SCS, ATC is the founder, Presi-dent and CEO. He has provided sideline and courtside coverage of athletic events in Elizabethton and Carter County since 1977, and continues to do so today. Dr. Smith has conducted over 200 con-tinuing education courses in the U.S. and abroad. He has a series of educational programs through Medbridge, which is an online educational program for physical therapists, athletic trainers and other professions involved in the care of athletes. These educational programs are available worldwide. Additionally, Dr. Smith has been the recipient of the two most prestigious awards presented by the Sports Physical Therapy Section of the American Physical Therapy Association, The Turner A. Blackburn Lifetime Achievement Award and the Ron Peyton Award. The Sports Physical Therapy Section rep-resents over 9,000 sports physical therapists on a worldwide basis.

Dr. Justin C. Smith PT, DHSc, SCS, RN serves as clinical director of

the practice. He is a 2006 graduate of Belmont University School of Physical Therapy. He holds the distinction of being one of the youngest physical therapists in the nation to attain the title of Sports Clinical Specialist. His primary responsibilities are to oversee reha-bilitation services in all clinics, provide rehabilitation services at the 1975 W. Elk Clinic, and to provide sideline and courtside services in Elizabethton and Carter County high schools. Additionally, Dr. Smith serves as an instructor in Emergency Medical Response for Physical Therapists on a nationwide basis, having taught the course at major universities and hospitals such as Duke University, Cleveland Clinic, and the Mayo Clinic.

Physical Therapy Services, P.A. has the distinction of being the loca-tion for the only Sports Physical Therapy Residency program in the State of Tennessee. Physical therapists from around the country come to the practice and are involved in a 12-13 month program with structured learning, sideline coverage, formal educational presentations to coaches, high school students, undergraduate and graduate college students. Former residents have worked as Sports Physical Therapists, for the US Olympic team, college and high school sports, professional sports and serve as faculty members in residency programs.

A Medically Integrated Gym is the latest endeavor of PTS. The facil-ity is located at the 1975 W. Elk office in Elizabethton. The exercise facility has a full line of exercise equipment as well as treadmills and elliptical trainers. The gym is designed for those individuals who do not respond well to the typical fitness center environment or whose insurance has expired and they need to continue an exercise program that has been established by a physical therapist. All par-ticipants will be evaluated by a physical therapist prior to beginning a program.

Rosellen Ryals, OT, serves as director of Occupational Therapy and is responsible for 11 occupational therapy professionals concentrating primarily on the pediatric population in East Tennessee and South-west Virginia. Rosellyn has extensive experience in OT services for the pediatric population specializing in the treatment of children with ADD and ADHD as well as a number of other pediatric dis-abilities. She is assisted by other OTs and COTAs experienced in the rehabilitation of children.
