Shetty International School” - in white paying Slip Book. Fees should be paid on or before the last date indicated above between 9.30 a.m. to 12.30 p.m. on bank working days.

DOCUMENTS TO BE SUBMITTED FOR CONFIRMED ADMISSION:

At School Office :

1. Original Birth Certificate of the Student. (Only the Birth Certificate obtained from the Municipal Corporation or competent authorities is valid and the Birth Certificate issued by Nursing Home/Hospitals will not be valid).

2. If the student belongs to SC /ST /OBC caste, attested copy of the caste certificate in the name of student.

INSTRUCTIONS REGARDING PAYMENT OF FEES :

1. School fees will be accepted only through local cheques/pay order/DD in the School premises at the Extension

Counter of Cosmos Bank at 9.30 a.m. to 2.00 p.m. and 3.00 p.m. to 6.00 p.m

2. In case fees paid through cheque is returned/bounced, parent has to pay penalty of Rs.200/- per transaction in cash to the school.

3. Please fill in all the details of students Name, Std. & Div. in the space provided in all three counterfoils of pay in slip to

enable the School to maintain proper records in the computerized system as this system is updated from the pay in slip received from the bank.

Please also write the students Name, Std &Div. on the reverse side of the cheque/pay order/DD
