    Quality Improvement Survey of the

    University Eye Center

    The purpose of this survey is to monitor the everyday operations of the University Eye Center

    (UEC) at the Michigan College of Optometry (MCO). If you currently do not examine patients at

    the UEC but have examined patients there in the past, please answer all the questions as

    they pertain ONLY to the UEC at MCO.
