Course Requirements:

1. Performance: Interns enrolled in this course are expected to complete the tasks assigned by their preceptor(s) or course instructor. Interns will receive a performance evaluation(s) based on their performance at the supervised practice site from the preceptor(s). All competencies must be performed at a satisfactory level or higher. Additional time needed to improve performance may be required. See Section on Preceptor Complaints or Non-Compliance/Performance in CPD Handbook. Course assignments are graded by the instructors.

2. Completion of supervised practice hours are the intern’s responsibility based on the schedule assigned by the preceptor. This may include additional supervised practice day(s) to compensate for facility observed holidays. Use “Rotation Hours Verification Log” form to document daily the number of hours worked at the site. In the case of an intern absence, all arrangements for makeup time are subject to the preceptor’s and CPD director’s approval. The intern is required to complete the “Schedule Deviation Form” for all absences. See Absence Policy and Attendance Policy in CPD Handbook. If an intern fails to meet the required supervised practice hours for this course, additional time will need to be scheduled for completion before a verification statement can be issued, which is mandatory for eligibility to take the Registered Dietitian exam.

In order to pass this course an intern must complete all competencies at a satisfactory level or higher (I=A (95 %) or II=B (85 %)). Assessment of “needs improvement” (III or IV) will require additional time for the intern to complete all competencies at a satisfactory level. If the preceptor recommends a separation from the supervised practice site, all attempts will be made by the Coordinated Program staff to find an alternative supervised practice site for the intern to complete supervised practice competencies for this rotation at a satisfactory level. If no supervised practice site can be found, separation from the program may be indicated. In that case the intern will be assigned a failing grade (F).

Contacting your Primary Preceptor 1. When you receive the contact information from Dinah Dalder, please contact your primary preceptor to introduce yourself and

discuss planning issues. 2. The dates of the rotation are Monday, August 28 through Monday, September 18 and you will be scheduled full time during three

weeks. 3. When you are talking with your primary preceptor or e-mailing your primary preceptor, remember to ask about dress code,

parking, when to arrive and other planning details. 4. Discuss in advance the site’s official holiday schedule for the time period of your rotation to plan required hours of supervised

1. Include the title of the assignment, date and your name on each assignment/ report. 2. You are expected to write in a professional style (proper spelling and grammar, no contractions, no slang or shop-talk). 3. Use word processing software and the spell-check function to complete your reports.. Points will be taken off for spelling and

grammatical errors. 4. Assignments need to be completed individually and written independently, even if there are two or more interns at the same

site working on the same project(s). Use the assistance of the Purdue University writing laboratory staff as needed to write professionally.

Attendance Policy and Rotation Hours Verification log 1. The Community rotation is planned for 120 supervised practice hours.

2. Record supervised practice hours daily (do not include meal break). This form may be handwritten. (See instructions on the

Rotation Hours Verification log)

3. Preceptor must sign the form at the end of two weeks.

4. Since this rotation is for three weeks only, please make sure the preceptor signs the rotation hours verification log for the final week, prior to leaving the rotation site.

5. If the primary preceptor is not available, please confirm an alternate preceptor or designee who can sign your hours verification log.

6. Save form to a “pdf” format and submit on Blackboard. 7. Include all the original signed rotation hours verification logs at the front of your portfolio.

Instruction for Completing Diversity Log

1. Record activities contributing to your diverse experiences with individuals of different ages, populations and cultures. 2. Put a check mark or cross is the appropriate category. An example of a population could be “lower socio-economic status

(SES)” and an example of a diverse culture may be “Burmese.” 3. Every column may not be relevant for each learning activity. Check only those that apply. 4. This form may be handwritten. 5. Include all the original diversity log at the end of your portfolio.

Instructions for Evaluations

1. If needed, provide a copy of Assessment of Learning Outcomes for Supervised Practice NUTR 42600 to the preceptor in advance of your evaluation (available to you on Blackboard—your preceptor does not have access to Blackboard).

2. Confirm the date, time and location of the evaluation with your preceptor and Dinah Dalder. 3. Complete your own copy of Assessment of Learning Outcomes for Supervised Practice NUTR 42600 in advance of your

evaluation. Include comments, strengths and areas needing improvement as your self-evaluation. Bring the completed form to the evaluation meeting with the preceptor and Purdue faculty and be prepared to present and discuss your self-evaluation.

4. Bring your up-to date portfolio to the evaluation meeting. The portfolio does not need to be finished, but it needs to be current (e.g. there may be unfinished projects at the time of your evaluation meeting).

5. At the conclusion of the evaluation visit, the preceptor, Purdue faculty and intern sign the Assessment of Learning Outcomes for Supervised Practice form to document the meeting.

6. The intern is responsible for keeping track of the completed Assessment of Learning Outcomes and for turning in the original signed form with the portfolio on the due date. A copy will be made of the Assessment form by the Purdue faculty and the original Assessment form will be returned to the intern for his/her portfolio. The copy is filed in each intern’s CPD file.

7. If needed, provide a copy of the Performance Evaluation Form to the preceptor (available on Blackboard). The Performance Evaluation Form is used to assign a grade to the supervised practice portion of the course. The preceptor will complete the

form and submit directly to Dinah Dalder (fax, email, or regular mail). A copy will be made a filed in each intern’s CPD file. The original will be given to the intern.

Rotation Objectives CRDN 2.12 (15 points) 1. Using the information learned in NUTR 42400 for writing learning objectives, create at least 4 objectives for your community

rotation experience 2. Turn in your objectives for review by 5:00 pm Tuesday, August 22. Upload the Word file through Blackboard. 3. On the first day of your rotation, be prepared to discuss your objectives and ideas with your primary preceptor. 4. Revise your objectives as necessary based on opportunities available at the supervised practice site. 5. Remember that you will have to be flexible and when you are actually doing the rotation, your objectives may change. 6. Document progress toward your objectives in your portfolio. Write a short summary on how you have met or are in progress

toward meeting each of your objectives.

Community Nutrition Rotation Career Portfolio CRDN 2.13 (15 points)

The portfolio is considered documentation of the intern’s activities and accomplishments.

1. Purchase/ locate a medium size binder and index dividers for the Community Nutrition rotation. 2. The front cover or first page of the binder needs to include the rotation name, name of supervised practice site, date and your

name. 3. Prepare index dividers. Label tabs on dividers with the competency numbers. Group competencies together on a tab if the

competencies are grouped together on the Learning Outcomes form. For example, CRDN 2.12 and CRDN 2.13 go on the same tab since they are grouped together on the Learning Outcomes form.

4. Prepare a title page after each divider listing the competency number, competency statement, and the learning activity / learning objective from the Learning Outcomes form.

5. Document your activities as specified on the Learning Outcomes form.. 6. Keep your portfolio up-to-date and bring with you to your evaluation visit. 7. Completed portfolios are due on September 19, 2017. Portfolios will be returned to you after reviewed by Purdue faculty.

2. Search scientific nutrition literature for articles that have used data from one of the surveys or surveillance activities. Use at least one article using NNM data that applies to your practice site. Options for finding resources: A. Articles can be found by doing a search of Medline/PubMed at http://www.nlm.nih.gov. For many of the NNM activities, you

can use the acronym or the entire activity name. Examples of journals that frequently feature NNM results include the Journal of the Academy of Nutrition and Dietetics, Journal of Nutrition, American Journal of Clinical Nutrition, American Journal of Public Health, Morbidity and Mortality Weekly Report (MMWR), and Journal of the American Medical Association.

B. Alternatively, you can go to www.cdc.gov and search for ‘MMWR’ and then put the full name of the activity in the MMWR search field.

C. Another alternative is to look for reports directly at the website of the sponsoring federal agency.

3. To meet the criteria for this assignment, the paper needs to have the subject recruitment description in the Methods section and specify which NNM program the subjects came from, e.g., NHANES. For example, the two abstracts below came up when searching for “NHANES” on PubMed. Note that Abstract A uses the NHANES sample as the study population, however Abstract B does not.

ABSTRACT A. METHODS: Data were analyzed from the 2001-2002 National Health and Nutrition Examination Survey (NHANES) conducted on a nationally representative sample of the U.S. population. This study focused on adults aged 20 years or older who were both interviewed and examined (n =5027). ABSTRACT B. PARTICIPANTS AND DESIGN: A single 24-hour recall was collected from 48 male and 44 female Yup'ik Eskimos (aged 14 to 81 years), who resided in three villages in the Yukon Kuskokwim River Delta, AK, during September 2003.

Assignment #2: Data resources on health status of Indiana and the US (15 points) (CRDN 1.2)

1. You can gauge the health of your state by accessing a snapshot of your state’s health status. Indicators on causes of deaths, and more can be found online through several websites. You can use the information to compare your state’s health status with the nation and identify strengths and areas needing improvement. Healthy People 2020 is designed to achieve two overarching goals: 1) Increase quality and years of healthy life; and 2) Eliminate health disparities.

2. In your oral presentation, include applicable descriptive health status information related to your assigned field site as background information about the target audience of your field site or health disparities addressed by your field site.

Professional Communication: Thank You Letter to Community Nutrition Preceptor (10 points) CRDN 2.2 1. At the end of your Community Nutrition rotation, write a thank you letter to your preceptor. 2. Use accepted business correspondence format for the thank you letter. In the body of your letter, personalize the thank you and

be specific in complimenting your preceptor. Refer to some specific elements of the rotation experience that were interesting or particularly useful.

3. If you know the individual well that you are corresponding with, you may use the person’s first name in the salutation. However, in business correspondence, this is used judiciously. If you have enclosures in correspondence this is noted at the bottom of the letter, two (2) spaces after your closing.

4. Using page set up, set margins as follows: top 2.0”, bottom 0.9”, left 1.0”, right 1.0”. Use 12 point font. Your letter will be printed on department letterhead.

The following is used to assist the Director of the Coordinated Program in Dietetics in determining the performance of the intern and the grade to be assigned for the practicum

course. Three (3) credits for NUTR 42600: Laboratory in Community Nutrition.

PLEASE RANK THE INTERN’S PERFORMANCE BY PLACING ONE CHECK IN: I, II, III, IV or V.*

I.__________ This candidate has met and/or exceeded all the objectives/competencies in the time allowed. Given my knowledge of this candidate’s current performance, he/she

should have little difficulty practicing as an entry-level dietitian in the area of community nutrition.

II.__________ This candidate has satisfactorily met all the objectives/competencies in the time allowed. Given my knowledge of this candidate’s current performance, he/she

should continue to develop skills in the area of community nutrition to be successful as an entry-level dietitian.

III.__________ This candidate has met most of the objectives/ competencies satisfactorily in the time allowed but needs improvement. Given my knowledge of this candidate’s

current performance, he/she should plan to devote additional time to study and practice of the community area. With additional study/practice, this candidate should be able to

function as an entry level dietitian.

IV.__________ This candidate has marginal performance suggesting lack of motivation or ability to maintain normal performance and growth. This candidate has failed to rectify

any and all complaints (remedial action) documented in the Non-performance/Complaint form. Separation from the supervised practice site is indicated. This candidate may have

difficulty practicing as an entry-level dietitian nutritionist.
