disabilities, their families, and other populations, as appropriate, to achieve their

maximum potential and independence.

To say 2008 was challenging would be an understatement.

Amendment 51, also commonly known as the “End the Wait List” initiative, was placed on Colorado’s ballot this past fall for

consideration of the voters. This sales tax measure would have provided funds dedicated to eliminating the growing

waiting list in Colorado. Foothills Gateway’s waiting list is currently over 300 individuals. Ultimately this measure was

defeated but saw a great deal of collaboration on behalf of Colorado citizens with special needs. ARC’s from around

Colorado as well as the Community Centered Board system joined together to educate the voters about the needs of this

population and the dire situation for the nearly 12,000 people waiting for services. At this time it is uncertain if another

measure will be forthcoming, but Foothills Gateway will continue its commitment to actively advocate for resources for

individuals with developmental disabilities.

Foothills Gateway hosted an “Abilities for Hire” breakfast to educate and engage new and existing employers in the benefits

of hiring individuals with disabilities. This event was a collaboration between Supported Employment Service Agencies and

sponsored by The Fort Collins Chamber of Commerce, Loveland Chamber of Commerce and the Northern Colorado

Business Report. Strengthening relationships with the business community is essential and proves beneficial for the clients

we serve.

The spirit of collaboration continued with the sixth annual Legislative Forum, co-sponsored with the Larimer Center for

Mental Health. This event allows constituents the opportunity to meet and discuss legislative issues with our state and local

representatives and candidates for office.

The year began with the much needed renovation of a 35+ year old building. Staff stalwartly performed their duties in

“make shift” offices or program areas, ensuring that clients received the top-notch services they were used to receiving on a

daily basis. It was exciting to see the progress each week as I walked into the building, knowing the changes would provide

a better work environment for both clients and staff alike. Staff continued to provide services and supports to over 1,300

individuals and families throughout Larimer County during this approximately year long “spruce up”.

Foothills Gateway continued to maintain program accreditation in all aspects of our organization, including service

provision, case management and administration. Despite regulatory changes to the system, the FGI staff has been able to

deliver services professionally and effectively. Our annual audit indicates that the Board and staff are good stewards of the

funds received from federal, state and local resources. Foothills Gateway remains a strong organization despite a tough

economic climate both nationally and locally.

The Board of Directors continues to pursue its strategic planning goals of looking into possibilities of developing new

revenue sources, and has launched two new projects. The first is looking into compliance with conflict of interest rules and

guidelines, and the second is reviewing ways for making accessibility to services as easy and convenient as possible.

Each year brings its own unique challenges and obstacles. For 2008, a hope for new funding for the waiting list being

defeated, some dust from the remodel and a tough economy for America. While those challenges were daunting at times,

we see them as opportunities to grow, educate and learn. 2009 will be a year of turning those challenges into

opportunities – taking a lesson from our clients who have turned their disabilities into abilities. On behalf of the clients,

Board of Directors and staff of Foothills Gateway, we thank you for your continued support and we look forward to a year

of empowerment and opportunity.

BOARD COMMITTEES Program Committee The Program Committee reviews policies and makes recommendations to the provision and monitoring of services

and supports to individuals with developmental disabilities. The Committee periodically reviews aggregate data

regarding services and supports and waiting list information.

Fiscal and Property Committee The Fiscal and Property Committee reviews and makes recommendations to the board of directors for approval of

annual operating budget , quarterly financial reports, long range financial planning, and the annual financial audits.

In addition, the Fiscal and Property Committee monitors the condition of physical assets and recommends

budgetary funding of repairs, improvements, and replacements as required.

Community Relations Committee The Community Relations Committee reviews and/or recommends projects and proposals to enhance Foothills

Gateway’s public relations. They also monitor, assist, and coordinate fund raising activities in the community and

interview/nominate new board members.

Executive Committee The Executive Committee monitors the organization’s programs and services to ensure that they are consistent

with the mission statement and the organization’s purpose.

Intake and Waiting List Case Management

Major Accomplishments

Intake and Waiting List Case Management promoted Carolyn Dekkers to Supervisor of the Intake and

Waiting List Case Managers and welcomed an additional part time Case Manager, Amy Baca. We also

continued our collaboration with local school districts, the Department of Human Services, and other

Community Centered Boards to assist individuals and families make smooth transitions from school, foster

care programs, or a move into Larimer County.

Foothills Gateway, Inc. participated in the Annual Transition Fair Series to provide information

on programs and services for adults with developmental disabilities. We also provided informational

presentations to the Thompson R2-J Autism Support group and the Supported Living Services (SLS) Parents

Group. Additionally, Intake and Waiting List Case Managers represented Foothills Gateway, Inc. at

Developmental Disabilities-Medically Fragile Placement Options Residential Treatment Committee, Larimer

County Children and Family Services, Foster Care Reviews for children with developmental disabilities,

Individual Education Plan meetings with Poudre and Thompson School Districts for adolescents on the

Foothills Gateway Waiting List for adult services, and Larimer County Regional Advisory Council for Aging

and Disability Resource Center (ARCH).

Intake and Waiting List Case Managers were also responsible for facilitating Supports Intensity Scale (SIS)

Assessments for individuals moving from the Waiting List into comprehensive services.

Flex-Ability in Early Intervention

2008 brought many changes to Early Intervention services at Foothills Gateway, Inc. The key to being

successful in this program is “flex-ability”. Early Intervention even went through a name shift at both

the local and state levels. Previously, Early Intervention was also known as Part C or Early Childhood

Connections. In order to make our program name more recognizable to families and physicians, the

state team for Early Intervention at the Division for Developmental Disabilities (DDD) announced that the

new program name is Early Intervention Colorado, or EI Colorado for short.

Another change experienced by the EI program was increased participation in the Assuring Better Child

Health and Development or ABCD initiative. The goal of the ABCD project is to encourage physicians to

use standardized developmental screening tools in well-child checks to increase identification of

developmental delays. There are now two large pediatric practices in Larimer County that use the “Ages

and Stages Questionnaire” to help screen children. This has resulted in increased referrals and numbers

served by Early Intervention. In December 2008, we served a record 234 children in the program, which

represents a steady increase of enrollments since 2007.

Staff working at FGI in Early Intervention also learned about private insurance and accessing private

insurance funds in response to legislation entitled “Coordinated System of Payment” which mandates

that certain insurance companies cover Early Intervention services. This has been a big shift for all

involved, but is bringing funding into the Early Intervention system outside of our state and federal

allocation. In times of change, with growing enrollments and shrinking dollars, being flex-able and

learning a new way to bill helps to keep the program going.

Housing-Ability in HUD-Section 8 Program

Foothills Gateway, Inc. continues to use it’s “housing-ability” through the Section 8 Voucher program. In

2008, Foothills Gateway had the ability to house 21 of the 27 families on the FGI waiting list, with the

issuance of state- funded housing vouchers.

This program currently provides housing support for 135 families through both rental and

homeownership programs.

HUD-Section 8 is not a program all Community Centered Boards

fund and support, however, Foothills Gateway values the program

as do the people receiving this personalized assistance. Foothills

Gateway, Inc. will continue to submit individuals to the statewide

waiting list and facilitate the transition for clients into

homeownership at every opportunity.

Support-Ability in Supported Living Services

The 2008 year brought new challenges to the Supported Living Services (SLS) program. The system-

wide changes affecting comprehensive services became a prime topic as changes were formulated for

the SLS program.

In July, Foothills took an anticipatory step to help ease the transition stress. Every sign from the state

indicated that Supported Living Consultation (SLC) would not be included as an available service for the

new fiscal year. Foothills responded to this information by working with our Supported Living

Consultants to develop a plan to identify and minimize impact to clients. The program also worked with

the Consultants to find alternative work options, including, within the company. The hard work of the

Consultants will be missed, but with ample notice and time to find alternative means of meeting the

needs, the transition went smoothly.

Another significant achievement during the past year was the completion of the SIS assessments for

Supported Living Services. These assessments were used to determine funding levels. SIS interviewers

(all employees of the Department of Resource Coordination and Development (DRCD)) met Division for

Developmental Disabilities (DDD) timelines for completing this task. Due to Medicaid limitations, the

SLS program also began working to transfer locally matched Medicaid SLS resources to state-matched

Medicaid resources. The 13 clients funded through locally matched funds did not experience any gap in

services. SLS administration worked hard to keep families and clients updated on changes as well as

offered a series of lectures on transition planning through the quarterly parent’s forums.

In 2008, SLS administration also continued to offer monthly health and safety themed classes. These

classes included topics such as nutrition, diet, community safety and fire safety. Over 200 clients

attended these classes throughout the year.

Foothills Gateway is always looking for ways to support client development. In an effort to maintain this

goal, SLS administration continued to sponsor a men’s and women’s group. The groups are open to all

clients and focused on promoting social and relationship skills and gave the clients a chance to develop

ongoing friendships.

Lastly, we are proud to share the SLS staff’s continued tradition of volunteering their time to take clients

to the YMCA of the Rockies in Estes Park for a weekend away.

Adapt-Ability in Adult Case Management

In the past year, Case Managers supporting adults in services

focused on the ability to complete and update processes and

adapt to changes in the system. This was accomplished in many

ways by many different people.

Several Adult Case Managers diligently worked with Supported

Living Services (SLS) staff and Intake Case Managers to complete

the Supports Intensity Scale Assessments (SIS) for all individuals in

SLS and any individual admitting to the Comprehensive waiver.

Foothills Gateway proved to have the ability to complete this

process within the short timelines.

Another system that underwent changes through the year is the electronic Benefits Utilization System

(BUS), which has proven to provide consistency in both Case Management services and clearly outlines

expectations for all adult wavier services providers. This state wide system allows for both Division for

Developmental Disabilities (DDD) and Health Care Policy and Finance (HCPF) staff to easily view long

term care assessments, and service plans for oversight and monitoring of the CCB. The major changes

this system underwent in the past year have caused Case Managers to enhance their ability to adapt to

change.

The PARS process continues to be updated on a regular basis. Case Managers have worked closely with

Program Approved Service Agencies (PASAs) to gather needed information to complete prior

authorization requests (PARS) for all waiver services. Case Managers had to improve their ability to

adapt to these, sometimes daily, changes.

Currently, Foothills Gateway has 12 Adult Case Managers and has been able to increase retention over

the past year. With enhanced training opportunities and clear processes to follow, we hope retention

will continue to increase over the next year. Our goal of increased retention will allow us the ability to

increase our knowledge and the ability to develop long term relationships and support clients more

Employ-Ability in Supported Employment/

Abilities for Hire Event Many local businesses are already very aware of our clients’ outstanding abilities in the workplace. FGI’s

Supported Employment program currently has clients working in many area businesses, however we are

always looking for ways to promote our clients and the abilities and benefits they can bring to an

employer. On October 14, 2008, Foothills Gateway teamed up with other service agencies who provide

Supported Employment in Larimer County to present Abilities for Hire. This was an event aimed at

educating local employers about the benefits of employing individuals with cognitive disabilities.

The event took place at the Fort Collins Hilton, included breakfast and featured a panel of employers and

clients who have enjoyed successful partnerships through a Supported Employment program.

Businesses represented in this year’s panel were New Belgium Brewing, Fiona’s Delicatessen and

Catering, Nita Crisp Crackers, City of Fort Collins/Transfort, and High Country Beverage; the panel was

moderated by Executive Director, Tim O’Neill. These businesses currently employ Foothills Gateway

clients and all spoke very highly of their clients’ contributions to the workplace and praised the support

that was provided to the employer and client by Supported Employment staff.

Foothills Gateway staff put forth a great effort in planning this event; promoting it to local businesses,

preparing marketing materials and coordinating panel members and clients to participate. Staff also

worked to secure promotional partners which included the Loveland and Fort Collins Chambers of

Commerce and the Northern Colorado Business Report who provided advertising for the event. While

the event required months of planning and coordination, all involved felt the effort was successful in

accomplishing our goal of providing an educational and inspiring program for local business people. It

is our hope that this event will lead to greater awareness of Foothills Gateway’s mission, as well as

increased employment opportunities for our clients. If you have or know of a business that would be

As is true with most corporations, Foothills Gateway, Inc. is and has always been concerned with

performance measures and outcomes. This year, as in years past, individualized surveys were sent to

various individuals including adult clients in Larimer County, families of adult clients in Larimer County,

adult clients who receive direct care services from Foothills Gateway, Inc., and families of adult clients

who receive direct care services from Foothills Gateway, Inc. Target Levels are set for each objective as

appropriate. The objective is to maximize client and family satisfaction in all areas on an annual basis.

Target Levels were exceeded and/or achieved in the following areas:

Client and Family Satisfaction with Community Inclusion

*Although both client and family surveys exceeded Target Levels, FGI feels it

would be beneficial to research the disparity between the satisfaction levels and

develop approaches to increase family satisfaction.

Although improvement has been made, target level has not been met for clients

and families. Foothills Gateway values the rights of every client, therefore our

target level for clients being informed of their rights is set at 100%. Additionally,

it is the practice of Foothills Gateway, Inc. to provide, annually, a copy of client

rights at every client’s service plan meeting. Client and/or legal guardian or

authorized representative sign and acknowledge that these rights have been

received. FGI plans to review the data to determine any disparity between

Comprehensive and SLS services as well as continue to research other possible

causes for not meeting target to include turnover and training.

In these areas, Foothills Gateway is committed to researching the reasons for underperformance and

developing new approaches for achieving target levels in the following year. Target levels in all areas

have been kept at current levels. A complete Outcomes Measurement Report can be found on our

Additional areas where FGI puts an emphasis on setting and achieving high standards are client job

retention through the Supported Employment Program, General and Administrative expenses, and

overall staff turnover.

Although the goal was not met in comprehensive client job retention, SLS client job retention goal was

exceeded in the 2007-2008 Fiscal Year. Supported Employment plans to maintain goals and strives to

continue efforts in both Comprehensive and SLS job development and retention.

General and Administrative expenses for the agency increased by .8% in the 2007-2008 Fiscal Year for a

total of 10.1% of budget. FGI attributes this to one-time costs for a special event and grants, which were

offset by additional revenues. Building renovation costs will continue into next year. With statewide

systemic changes, administrative demands have increased. FGI is dedicated to analyzing administrative

duties to reduce costs.

Foothills Gateway also experienced consistency in staff turnover, reporting at 42%. Target Level was not

met in this area due to substantial system changes which has hindered efforts to reduce turnover. We

plan to continue efforts to reduce turnover and maintain the target level at 35%.

The number of individuals served by

Foothills Gateway, Inc., grew by 9.2%

in FY2008 over those served in the

prior fiscal year. This was reflected in

the 9% increase in support and

revenues for the year. The major

increase in funding was a 17.3%

increase in Medicaid program funds,

offset in part by a decline of 14.2% in

State program funds.

Management and General expenses

rose as a percentage of total expenses

due to some unusual organizational

expenditures. Public relations costs

for the Foothills Gateway 35th

Anniversary event and grant writer

consulting costs, though contributing

to the expense increase, were offset

by the additional revenues generated.

Also, the organization undertook a

remodeling effort to improve the

condition of its 35 year old building

and equipment. The remodeling

utilized funds from donations received

in the prior fiscal year.

through Foothills Gateway placements

expanded 33% to 12 provider

agencies, with additional agencies

applying for DDD approval. While

adding to administrative requirements

and accompanying costs, new

agencies broaden the choices

available to clients in selecting a

provider to meet their service needs.

Community Outreach and Fund Raising

The 2008 Legislative Session was filled with many issues for Colorado voters to decide. Two issues would

have had significant impact to individuals with developmental disabilities, Amendment 51 and County

Referred Issue #1A. Amendment 51 was a statewide measure that would have provided a critically

needed safety net for children and adults with developmental disabilities through a modest, phased in

sales tax of 2/10 of 1%. This issue was commonly referred to as the “end the wait list” issue. County

Referred Issue #1A, a Larimer County ballot issue, proposed a .25% sale tax to build and operate a

detoxification and residential treatment facility in Larimer County

for individuals with a substance abuse and/or mental health issues.

Both issues would have provided enriched services for clients

currently receiving services and those waiting for services through

Foothills Gateway and our services providers. Unfortunately, these

two issues were not approved by the voters in the November vote.

However, an increased awareness about the issues facing individuals

with developmental disabilities, their families and those struggling

with mental illness and substance abuse was heightened during both

campaigns and highlighted during our annual co-sponsored

Legislative Forum with the Larimer Center for Mental Health and by

participating in several legislative functions sponsored by Alliance

and the Northern Colorado Social Legislative Network.

We also continued working with our support groups to raise awareness

and funds for our programs through annual fundraising events such as, the Fort Collins Service League’s

Kitchen Kaper, the Foothills Gateway Foundation’s Bonanza Auction and the Shanks for the Memories

and Hogerty Open golf tournaments, the Foothills Service League’s Comedy Night, and the Taste of

Loveland wine & food tasting. These groups also hosted several fun and social activities for our clients,

including an Ice Cream Social, a Pizza Party, a fishing trip to Red Feathers, and the annual Christmas Gift
