Use this form when you are applying for title on an assembled light trailer or an assembled heavy trailer that has never been titled or registered before AND was not manufactured by a commercial firm.

Complete all sections of the form and sign it.

A release of interest from whomever you purchased the trailer, if you didn’t build it; or Receipts or bill(s) of sale for both the frame and axle if you built the trailer.

If you cannot provide one of the above requirements, you must give a written explanation in the space provided on the front of this form.

It is important that you complete the form thoroughly. Your transaction may be returned if you do not provide the information needed to determine ownership or vehicle type.

A vehicle identification number (VIN) inspection is required. You must take the trailer to a DMV field office for the inspection. There is a $7 fee for the inspection and be prepared to pay the title fee. To determine if VIN inspections are available at the DMV field office in your area, see the contact information below.
