FACULTY

Diana Kornetti, PT, MA, HCS-D, COS-C is co-owner of a

home health agency in Central Florida where she continues to see clients. Dee is nationally recognized as a speaker in the areas of homecare, standardized tests and measures in the fi eld of physical therapy, therapy training and staff development.

Cindy Krafft, PT, MS, is the current President of the Home

Health Section of the APTA and the Director of Rehabilitation Consulting Services for Fazzi Associates. With 14 years of home health experience, she works with agencies and state homecare associations to improve understanding and utilization of therapy services.

Kenneth Miller, PT, DPT is the clinical educator for Catholic Home Care, where he provides staff development, continuing education and training. He is an adjunct instructor and guest lecturer at Touro College’s, Doctor of Physical Therapy Program, in Bay Shore, NY.

The Pennsylvania Homecare Association and the American Physical Therapy Association’s

Home Health Section are pleased to once again sponsor this timely session on evidence-

based practices for home health therapists. Our three speakers are nationally recognized for their work and have real-life experiences in providing therapy to home health patients.

This session will provide attendees with an opportunity to become clinically competent in a variety of therapy measures appropriate for use with the Medicare homecare population, as well as the tools and resources for establishing a competency program for their homecare agencies.

Tools included in a hands-on lab will be based on those included in the Home Health Toolbox, developed by the Home Health Section’s Practice Committee Task Force.

COURSE OBJECTIVES

Upon completion of this course, participants will be able to:

1. Demonstrate competency in a variety of standardized instruments appropriate for use with the community-dwelling older adult and frail geriatric populations.

2. Implement a competency program for peer therapists in tests and measures.

3. Demonstrate knowledge of how and when to most appropriately utilize Section-approved standardized instruments with their geriatric patients.

4. Identify the possible impacts of PPS 2013 on the delivery of therapy services.

5. Discuss the relationship between therapy services and the outcome and process measures as defi ned by OASIS.
