Forest Division Select the forest from thedropdown list(Forests in thedropdown are correspondingto the village selected) ORClick on the Add button toenter a new forest. (ReferForm-2 for the details)

If not selected, a message “Please select the value Forest”appears on submission.

On submitting the number of claimants, rows equal in number to the number of claimants areadded to the form. The user needs to enter the details for the claimants into these fields.

Note: The first row that appears on the form is for the main claimant and the rest of the rowsare for the other claimants. The following fields are applicable for all the claimants

Name of Claimant Enter the name of theclaimant

If name of first claimant notentered, a message “Please fillthe detail for Name of firstclaimant” appears on thescreen on final submission.

Gender Select whether the claimant isa Male or Female

Age Enter the age of the claimant If non-numeric value isentered, a message “Pleaseenter the numeric value forAge of Claimant” appears onthe screen.

Relationship with firstclaimant For the first claimant, Self

appears on the screen alongwith an option to attach hisphotograph. (See Form –3) For rest of the claimants,select their relationship to thefirst claimant from the dropdown list

On attaching photograph offirst claimant, a link “click toview appears”. By clicking onthis link, the attachedphotograph can be viewed.

Father,s Name Enter the father's name of thecorresponding claimant

Mother‟s Name Enter the mother's name ofthe corresponding claimant

Status Select whether the claimant isSchedule Tribe (ST) or OtherTraditional Forest Dweller(OTFD). Attach certificate ifthe claimant is a ScheduleTribe. (See Form-4)

Marital Status Click yes, if the claimant ismarried. Otherwise, click No.

Spouse Name Enter the name of the spouseof the claimant, if theclaimant's Marital status is Yes(i.e. if the claimant ismarried). Otherwise, leaveblank.

Is Spouse ST Check on the checkboxprovided if the claimant'sspouse is a Scheduled Tribe.And attach the relevantcertificate (See Form – 4)

Address Enter the address of theclaimant

On submitting the number of member in the family, rows appear onto the form equal to thenumber of number entered by the user. Note: The family members entered should be the familymembers of the first claimant only. The following details are applicable for each family member.

Name Enter the name of the FamilyMember

Gender Select whether the familymember is a Male or Female

Marital Status Click yes, if the claimant ismarried. Otherwise, click No.

Relation Select from the dropdown list,the relation of the familymember to the first claimant

Age Enter the age of the familymember

If non-numeric value isentered, a message “Pleaseenter the numeric value forAge of Claimant” appears onthe screen.

After filling the details of the family members (of the first claimant), other details are filled.

Field Description Message

Source of Livelihood Select the source oflivelihood of the first claimantfrom the dropdown list. If notpresent in the list, select"Other" and enter the sourceof livelihood in the adjacenttextbox that appears onselecting Other.

If not selected, amessage “Select the valueLivelihood” appears on thescreen on submission.

Date of Claim Select the date of claim fromthe adjacent Calendar icon.After selection, the date ofclaim appears in the textbox

If not selected, a message“Please fill the detail for Dateof Claim” appears on thescreen on submission.

Claim Details – Claim can be made for either Habitation or Self-cultivation or both. Followingdetails need to be entered corresponding to the claim made.

Area (In Hectares) Enter the area of land claimedin hectares. Float values arepermissible

If non-numeric value isentered, a message “Enternumeric value for Area”appears on the screen. If the total area(habitationand/or self-cultivation) isgreater than 4 is entered, amessage “Area cannot begreater than 4) appears on thescreen.

Form to Add Evidences

Click on the checkbox for the evidence you want to enter. You can also upload files supportingthe evidences by clicking on the Browse button and selecting the file. Click on the CONTINUEbutton to upload the evidences. If no files are attached in support of evidences, a message appears on the screen – “You

have not uploaded the evidences. DO you want to continue?” The user can continue byclicking on Yes button or press No button to upload the evidences.

If less than two evidences are selected, a message appears on the screen- “Select at leasttwo evidences”
