PROPOSAL TO INTRODUCE A NEW DRUG INTO THE HOSPITAL FORMULARY

If any doctors feels strongly that a new drugs should be included inside the hospital

formulary, they could bring this up to their respective head of departments. If the head

of department agrees that the new drug(s) should be included, they can proceed by:

1. Filling up the form Dossier 4. (Kindly get the copy of Dossier 4 form from

Pharmacy Department).

2. This form should then be submitted to the Unit Maklumat Ubat, Pharmacy

Department.

3. A drug meeting will then be held to discuss the feasibility of adding in this new

drug(s). Please understand that not all new drug(s) can be approved as this is

subjected to the available budget.

4. If there is a similar existing drug, then this drug(s) quantity will have to be

reduced. Furthermore, a quota system will be in place for the new drug(s) that is

(are) approved.

5. All this is to ensure that there will be enough budget available for the entire year

6. If the application is approved, please allow 1 month for the purchasing process.

The head of department will be informed once the drug(s) is (are) available.

A/KK = Consultants/Specialists/Family Physician Specialists

B = Medical Officers (MO)

C = Paramedical Staff

C+ = Paramedical Staff doing midwifery
