General Information

Daily Schedule Please review the following schedule with your child. School start time has changed to 8:05. Students who arrive after 8:05 will need to sign-in at the office before being admitted to class.

Arrival Procedures

1. PARENT DROP OFF AND PICK UP ENTRANCE. Parents may enter on Cap America Drive to FIS DRIVE to drop off or pick up students. This is a one-way entrance. Parents will drop off and pick up in the same location in front of the building by the flagpole. The front entrance of the school will not be available for parent/visitor traffic before 8:15 a.m. and after 2:30 p.m. The speed limit is 10 m.p.h. Please drive slowly to ensure the safety of all students. Please exit to the right when leaving school grounds. Students are not allowed to be dropped off or picked up at the trailer classrooms. Parents must exit the school by making a right hand turn on Highway 72.

2. Early Dismissal - In case of early dismissal, parents must notify the school if there will be a change in transportation. Otherwise, the early dismissal instructions on the enrollment card will be followed.

3. Students will be allowed to enter the building at 7:30 a.m. each day. Supervision of students will begin at 7:30. The district will not be responsible for supervising students before this time unless attending a special class or event. Please do not drop off students prior to 7:30.

Dismissal Procedures Parent Pick-up – NEW PROCEDURE: Students will be release for pick-ups at 2:50. Parents will be issued two hangtags upon registration that will need to be placed on the rear view mirror with student’s name on them. The extra hangtag can be given to another person you have designated to pick up your child. If a hangtag is not visible, the parent or designated person will have to come inside to sign out the student. Identification will be required for those who sign out students. Parents should notify school if someone other than parent or guardian will be picking up a student by 2:00. Parents may purchase additional hangtags. Bus Transportation

Bus Regulations & Expectations:

A. Students who are transported will be assigned to a specific bus and bus route. B. Students who ride a bus to school or an activity are expected to return the same way unless prior arrangements have been made by the parent. Students will not be allowed to ride a different bus or go with a friend unless the parent/guardian has provided a signed note instructing the school to allow an alternative means of transportation. C. Bus Transportation Changes:

Changes in bus transportation must be made no later than 12:00 p.m. to ensure the safety of our students.

D. Bus passes are written in the office when the parent sends a note or calls to verify a change in bus routine. Your child must have a bus pass to ride a different bus home.

Critical STUDENT ABSENCES AND EXCUSES

As directed by the Board, the following procedures will be used to implement the district’s attendance policy. Definitions Attendance – A student is considered to be in attendance if the student is physically present in a class; participating in a district-sponsored or district-approved activity; participating in a class through alternative methods or media as allowed by Board policy; receiving homebound services; or receiving services at another location pursuant to law or by arrangement of the district. Parent – A parent, guardian or person acting as a parent in the absence of the parent or guardian if the student is under 18. If the student is 18 or otherwise emancipated, the student will serve as the parent for purposes of this procedure. Tardy – A student is tardy if the student arrives after the expected time class or school begins, as determined by the district. Tardiness will be counted as an absence in situations where the student arrives too late to have meaningful participation in the class, lesson or activity. A student is considered tardy if arriving later than 8:10. Any student arriving after 8:10 must sign-in at the office. Truancy – A student is truant if the student is absent from class or school without the knowledge and consent of the parents and the administration. A student is also considered truant if the student leaves school without the consent of the principal or accumulates excessive unjustifiable absences, even with parental consent. Truancy is a type of unexcused absence.

Attendance Standards The following absences will be excused. Documentation must be provided as indicated.

1. Medical appointments, with written appointment confirmation by medical provider.

2. Funeral, with written excuse from parent. The principal may require a program or other evidence

of attendance as additional verification.

3. Religious observances, with written excuse from parent.

4. Other appointments that cannot be scheduled outside attendance hours, such as court

appearances, with written excuse from parent.

5. Out- of -School Suspension

6. Visits with a parent or legal guardian who is an active duty member of the military who has been

called to duty for, is on leave from, or is immediately returned from deployment to a combat zone

or combat support posting, with permission of the superintendent or designee.

All other absence and any absence for which required documentation is not provided are unexcused. Consequences for Violations Grade K-5 Attendance in early grades is crucial to later academic development. In addition, attendance habits are formed in early grades, and many later attendance problems can be averted with intensive family and student interventions in early grades. For these reasons, principals, with the assistance of building staff, will closely monitor student attendance and implement intervention strategies and other actions as follows:

1. Any time a student is absent and the parents have not contacted the school, the social worker or

designee will make contact with the parent/guardian.

2. When a student has accumulated (5) unexcused absences in a semester, a (5) day notification

letter will be sent to the family to make them aware of the student’s attendance. The purpose of

the letter is to clarify the school’s expectations regarding the attendance policy.

3. When a student has accumulated (7) unexcused absences in a semester, the district will

determine whether there is a reason to suspect educational neglect or whether the parent is

violating the compulsory attendance laws. If so, the district will contact the Children’s Division

(CD) of the Department of Social Services or the local prosecutor.

4. When a student has accumulated (10) excused or unexcused absences in a semester, the

principal, social worker, and or SRO will set up a conference with the parent/guardian to discuss

the student’s attendance and current level of academic performance. The purpose of the

conference is to clarify the school’s expectations regarding attendance and elicit suggestions

from the parent on how to improve the student’s attendance, including identifying reasons why

the student is not attending school regularly.

5. Accumulative absences of 24 days or more in a year will be a factor in determining whether the

student may be retained or required to attend summer school as a condition of promotion.

Students are expected to make up assignments from missed classes within the time period established by their teachers. Any conference may be waived by the principal if the absences were caused by a specific event or long-term illness. In cases where the district is aware that a student must be absent for an extended period of time, the district will arrange for the student to receive instruction by other appropriate means. Students who fall below 90% attendance rate will meet with social worker weekly until attendance rises above 90%.

Communication with School Education is a partnership between parents, students, and school. Communication is essential to providing the best learning environment for students. There are many ways parents can communicate and receive communication from school.

1. Classroom Dojo – A safe way for teachers to share experiences of the classroom with parents. It is a communication tool that uses text messages to notify parents of events, important information, and to encourage specific classroom student behaviors to keep students engaged. Parents may sign up with the classroom teacher.

2. Remind 101 - Parents can sign up to receive information pertaining to Intermediate School such

as important reminders of events and learning zone information. Parents may sign up for REMIND 101 at any time.

3. District Notification System – The Fredericktown School District will automatically opt you in for school/district notifications using the numbers(s) you have on file with the district. Messages may include but not limited to, lunch fees, announcements, cancellations, and other important information. These messages will be sent via automated phone calls and/or SMS text messages. You are encouraged to update your number with your child’s building office regularly. By signing the student handbook, you are consenting to these messages.

4. Website - The district’s website is www.fpsk12.org . Please refer to the website for important information about our school district such as menus, calendars, and announcements.

5. Monthly Newsletter - A monthly newsletter will be sent home containing important dates, goals, events, parent tips, and recognition of students and staff.

6. Parent Portal - on the district’s website is available for all parents/guardians. This portal gives parents/guardians the opportunity to view information about his/her child at any time. This includes grades, attendance, lunch charges, and discipline referrals. To access the parent portal a parent/guardian will need to do the following:

Log on to the school website at http://fpsk12.org and click on the “for parents” tab. A. Read the Parent Portal User Expectations and Computer Requirements. B. Fill out the Parent Portal Web Access Agreement Form. You may want to also check out the

information under the Parent Portal EZGuide and/or the Parent Portal Instructional Video. C. Bring completed form in person to one of the school offices. You will need a form of

identification.

Contacting School Parents are encouraged to contact school for transportation changes, student absences, questions, or concerns. Teachers will be available during their prep period, before, or after school to discuss student issues.

1. Transportation Changes - Please contact school at (573) 783-6455 by 12:00 noon to make transportation changes.

2. Absences - Each time a student is absent parents are expected to:

A. Call the school at 783-6455 by 9:00 a.m. to notify us of your child’s absence.

B. Send a note with the student when he/she returns if no phone is available or parent was unable to call. If the absence is not cleared with the office, our school nurse or elementary social worker may call to follow up the student’s absence. C. Send or have doctor’s excuse faxed to school.

Leaving School -

A. Students are only allowed to leave school property with a parent, legal guardian, or their designee. The designee must have a note with the parent’s signature or be listed on the enrollment card as a parent designee to pick up the student.

B. Students will only be excused for good cause, such as sickness, doctors/dental appointments, etc. Family trips or other reasons to leave school should be approved by the principal.

C. Sign Out: When a parent or other adult picks up a child from school or after school activities, the child must be signed out in the office. The child will be called to the office. Please do not go to your child’s classroom. A parent sign - out form is available in the office to register these early dismissals. Only those individuals listed on the student’s enrollment card will be allowed to transport your child. Please make prior arrangements with the school if someone else is picking up your child.

School-based Social Worker Families who need community resources or assistance may contact the school-based social worker at (573) 783-6455. The social worker is an employee of the district and does not work for the Division of Family Services. Parents requesting help may contact school counselor, social worker, resource officer, or principal.

Parties: Holiday Parties - All grade levels will have three parties a year; Halloween, Christmas, and Valentine’s Day. Parents are welcome to attend Halloween and Valentine’s Day Parties. Classroom teachers will host the Christmas parties. Parents are required to sign-in and given a visitor tag before entering a classroom or other part of the building. Allergies - Due to severe allergies of some of the students, all snacks must be prepackaged (store bought) snacks with ingredients listed. Drinks should be individually packaged. Some foods may not be allowed in the classroom due to student allergies. Please contact your child’s teacher before bringing or sending food for the class. Invitations – Invitation to parties will not be passed out at school unless an invitation is provided for each child in the class. Parents may opt their child out of a district party by contacting the principal.

Bad Weather

When inclement weather occurs, please do not call the radio station or the school. Parents or guardians will be notified of school closings by phone using the notification system. Other ways to get information about school closings are KTJJ and KREI radio, or television channels 2, 4, 5, and 12. Parents must notify the office when phone numbers change to receive notifications.

Breakfast/Lunch Programs Each student has been assigned an account in the computerized meal accounting system which only meals (breakfast/lunch) and extra milk will be deducted. Credit will remain in the account for days when breakfast or lunch is not purchased. Pre-payment into your child’s account may be made by cash or check using the lunch envelopes provided by the school. If you have more than one child at the Intermediate School, you need to use a separate envelope for each child. Please make sure that the envelope is filled out completely and used only for that purpose. These envelopes will be collected each morning by the teacher and then sent to the cashier in the office to be deposited into student accounts. You may prepay weekly or monthly. If you prepay weekly, we strongly encourage pre-payment on Monday of each week. Checks should be made payable to Fredericktown R-1 School. A note will be sent home if your child’s account is low to remind you that a deposit is needed. Please do not expect your child to charge lunches when the balance reaches -0-. Send a lunch with your child or contact the school if hardship prevents proper payment. If your child had a credit from last year, it will be carried over into their account for the new school year. If you have any questions about your child’s account, please call the office at 783-6455. Students SHOULD NOT bring soda to school. Juice, water, or milk is acceptable. Parents are asked NOT TO BRING FAST FOOD LUNCHES to school for their child. Money Owed the School Any money owed for lost or damaged books, pictures, lunches or other costs should be paid before the end of the year. Parents will be notified monthly of outstanding lunch charges. A record of all debts will be kept in the office.

Textbooks The district provides textbooks for all students. These books are to be kept clean and undamaged. If a book is lost or damaged (beyond normal wear) it must be paid for before another can be issued.

Emergency Drill and Procedures

All students will participate in fire, tornado, intruder, and earthquake drills. All parents should impress upon their children the importance of maintaining a serious attitude when practicing for emergency situations. During these drills, it may be necessary for staff and students to evacuate the campuses. Staff and students will practice evacuating to the Kelly A. Burlison Middle School as part of tornado preparedness drill. In real emergency students will go to the Middle School prior to any tornado warning for Madison County. The following is a list of procedures for each of our drills and/or emergencies.

1. Fire - Students will exit the building to designated locations. 2. Earthquake – Students will get under desks and protect their heads. Students will

evacuate the building to designated location as soon as it safe to do so. 3. Tornado –

A. Level 1 – Tornado Watch - All students from trailer classrooms will be relocated to designated areas in the building until watch is over.

B. Level 2 – Tornado Warning in surrounding counties – All students will be transported to the Kelly Alan Burlison Middle School FEMA shelter.

C. Level 3 – Tornado Warning – NO PRIOR WARNING IN SURROUNDING COUNTIES – Students will be directed to the hallways in the building and follow safety procedures.

4. Intruder – Students will participate in drills related to intruder/assailant situations. Students will be directed to the safest procedure based upon location and situation of the incident.

Emergency Information - In case of emergency, each student is required to have on file at the school office the following information:

1. Parent(s) or guardian (s) name, address & phone number(s). a. Please include cell phone and work numbers. b. Update school records when phone numbers change.

2. Emergency numbers of relative, friend, or neighbor. 3. Medical alert information 4. Authorized person(s) allowed to pick up your child. 5. In case of evacuation, parents will be notified where to pick up their child.

Field Trips Field trips within our area and to nearby points of interest are scheduled by various classroom teachers throughout the school year. Parents will receive certification form at registration and signature will be considered as permission for students to attend field trips throughout the year. Notification of field trips will be sent home as they occur. Parents may request students not to attend a field trip at any time. Making arrangements for the field trips sometimes includes an exact count for event tickets within a deadline. Occasionally, a fee may be requested from each student to help defray transportation or admission cost. Field trips are for the benefit of students attending Fredericktown Intermediate School. All parents who wish to accompany the classroom on field trips must complete a background/records check in order to attend. Parents planning to accompany the classroom on field trips should plan to make child care provisions for younger siblings. No siblings are invited to attend field trips. Due to severe allergies to peanut butter and peanut products are not allowed on field trips.

Pictures School pictures are taken twice a year (fall & spring). The date will be scheduled and you will

be notified well in advance. There will be a scheduled make-up date for absent students or retakes for fall pictures only.

Homework Homework is assigned to give students extra practice in their studies. Our staff plans well-designed homework assignments that relate directly to class work and extend student learning beyond the classroom. This should be a maximum of 30 minutes per evening. You are encouraged to help your child set a time and place for completion of homework assignments. It is advised to make a visual check to be sure your child has assignments completed and ready to turn in when they are due. Support from home can help our students avoid developing a habit of procrastination. If homework becomes a problem with your child, please consult your child’s teacher early. A student who has been absent will have one day for every day absent to make-up work that has been missed. Students who need assistance with homework may enroll in Learning Zone our afterschool program.

Report Cards/Progress Reports Student progress reports are issued mid-quarter and are intended to alert parents if a student’s grades are not acceptable while there is still time and opportunity to bring up a poor grade. Formal grade reports are issued on a report card print-out following each nine-week quarter. If there is concern about grades or any issues, call the school to make an appointment with your child’s teacher. Report Cards and Progress Reports require a parent/guardian signature and should be returned within a few days.

Parent- Teacher Conferences Parents are always welcome to visit the school. Conferences and visits must be planned with a one-day notice. If you would like to talk with your child’s teacher, please contact the office to arrange an appointment at 783-6455. This allows you the time to talk with the teacher without interruption. Parent/Teacher conferences will be scheduled in the fall. You will receive information at a later date with information on how and when to set up an appointment with your child’s teacher.

Response to Intervention (RTI)

Response to Intervention (RTI provides opportunities for students to get assistance on various skills to be a successful student. Students who need assistance throughout the year will be given additional time weekly to work in small groups on skill deficits. The progress of each student is monitored and shared with parents throughout the year.

A Parent Guide To

Response to Intervention (RTI) Recent changes in federal and state laws have directed schools to focus more on helping all children learn by addressing problems earlier within the general education setting. These laws emphasize the importance of providing high quality, scientifically-based instruction and interventions. These laws also hold schools accountable for adequate progress of all students, as well as, providing interventions for students who are at-risk for academic and/or behavior problems. This process is called: Response to Intervention (RtI). What is Response to Intervention (RTI)?

RtI is a problem-solving process designed to provide high quality interventions that are matched to student needs, as well as, monitors student progress on a frequent basis. Screening assessments are given three times per year to the student population. The data generated from the screening measures, along with student performance within the general educational setting, is utilized to make decisions regarding the student’s educational program and needs. What is the Problem-Solving Process?

The problem-solving process is a systematic method for addressing students’ academic and/or behavioral problems within the school setting. At the school level, the RtI problem-solving process follows the following steps:

What Does the RTI Model Look Like?

Perhaps the highest benefit of the RTI approach is that it eliminates a “wait to fail” situation because students get help promptly within the general education setting. As soon as assessment data indicates a problem area for a student or group of students, interventions are put into place to address these concerns. While the interventions are taking place, school staff monitors the progress these students are making in the area of concern. These progress monitoring techniques used within the RTI process provide information that allows staff to better evaluate the student’s needs, as well as, match instruction, resources and interventions appropriately. Support is provided on an as-needed basis. Students may move back and forth within the tiers as well. If gaining success, the interventions are reduced and a gradual release of support is utilized until the student is performing at grade level. If, however, progress is not evident, new interventions are implemented and/or additional time is provided in a smaller group setting. Tier I and Tier II interventions are provided during non-instructional time. Tier III interventions are also scheduled during non-instructional timeframes if possible. It is our hope that the RTI process will offer our students a differentiated curriculum based upon their needs, as well as, support their growth within education.

Learning Zone Learning Zone is a STEM (Science, Technology, Engineering, and Math) afterschool program. Tutoring/homework help classes are also available for students. Students may enroll in Learning Zone throughout the year. Transportation is available.

C.A.T.S. (Gifted Education Program)

The Creative and Talented Students program is offered to students who are academically gifted and are recommended for testing based on multi-criteria.

Promotion/Retention Procedures:

1. If a student is having difficulties, teachers are responsible for the following:

i. Referring the student to Student Support Team

ii. Analyzing a student’s data – Aims Web, Benchmark Assessments, progress of RTI

iii. Analyzing Assessment Data

iv. Conferencing with the parents

v. Develop Reading Improvement plan/RTI tier 3 implementation

vi. Mandating a summer school reading program to determine promotion

2. Parents will be provided a copy of the policy at the beginning of each school year.

3. If a student’s ability level indicates possible retention (below their current grade placement), parents must

be notified by the end of 2nd quarter by their classroom teacher.

4. K-5 identified students will be assessed to determine current academic level within 45 days of the end of

school year.

5. Once a student has been retained, but his academic level is still low, remedial summer school will be

available.

6. Each student who has a Reading Improvement Plan should attend summer school reading program. They

will be assessed at the end of the program. Promotion will be determined based on the student’s ability to

raise his reading ability to an acceptable level.

7. If summer school is a requirement, attendance will be mandatory.

8. A Reading Improvement Plan will be developed for students in grades K-5 who have been identified reading

below grade level.

9. Those who are still reading below the third grade level at the end of the fourth grade shall be required to

attend summer school for reading instruction. Assessment will be given at the end of summer school. The

child will be retained if the student is still reading below grade level.

Mandatory retention may only be imposed once. Missouri law states, “No student shall be denied promotion

more than once solely for inability to meet the reading standards set out in this (law).” However, school

districts MUST repeat the assessment procedures to monitor children’s reading progress during grades 4-6,

and school districts MAY continue to mandate summer school attendance.

The following students are exempt:

Students receiving special education services under an IEP

Students receiving services pursuant to Section 504 whose service plan includes and element

addressing reading

Students who have limited English proficiency

Students who have been determined prior to the beginning of the school year to have a cognitive

ability insufficient to meet the pre-determined required reading levels. The district must still provide

reading improvement plans to these students.

Students determined to be reading at or above grade level.

Special Education Services: Special Education services are available to all students who meet the eligibility criteria established by state

and federal guidelines.

Guidance Program Elementary guidance is an integral part of the total education experience. Academic growth cannot be separated from the social-emotional growth of a child. The school counselor works with individual and groups of students and implements Missouri Model Guidance in the classroom. All students may visit the guidance office by appointment or upon request of a teacher. Parents should also feel free to consult with the counselor. The program is a collaborative effort among students, parents, teachers, administrators, and the community.

Health and Wellness Illnesses If a student becomes too ill to remain in class, the procedure outlined by the parent on the enrollment card will be followed. (Usually this is a call to the first parent listed.) It is very important that we have up to date phone numbers. We have temporary facilities to help comfort your ill child while he/she is waiting. Transportation is not provided for sick children. A student with a contagious illness or condition such as pink eye, chickenpox, ringworm, impetigo, or pediculosis (head lice) will be sent home and must remain home until the condition or disease is corrected.

Physical Education Classes Health and wellness is an important part of a student’s education. Students will participate in physical education classes unless a student has an excuse from a doctor.

Medical Services/Policies Prescription Medications- Please read all guidelines. The school nurse will follow these with NO exceptions being made.

Prescription Medication must be ordered by a physician (physician and parent authorization forms are available in nurse office).

Orders must be renewed each school year.

Medication must be brought to the school nurse by the parent and must be in original container with student’s name and not be expired. Pharmacy will give you 2 bottles for the medication-if you tell them you need second bottle for school nurse.

Medication brought in a plastic bag or in a pocket WILL NOT be administered and parent must take home with them or pick it up. The medicine will NOT be sent home with student.

Inhalers must also come in original box or have a prescription label attached to the inhaler.

Students are not to share medications.

Only medication that is medically necessary to be given during school hours will be administered at school. Example-if medication is three times a day-it may be given at home: before school, after school and at bedtime.

Only antibiotics that are 4 times daily need to be given during school hours, please ask the pharmacy to give you 2 bottles (1 bottle to be left at home and 1 to be left at school).

PLEASE PICK UP ALL MEDICATIONS AT THE END OF THE SCHOOL YEAR. ANY MEDICATION LEFT AT SCHOOL WILL BE DISPOSED OF 1 WEEK AFTER SCHOOL IS OUT FOR THE SUMMER.

OVER THE COUNTER MEDICATIONS

The nurse keeps the following over the counter medications in the nurses’ office: The following medications will only be given if we have a parent permission slip signed in the nurse office. Caladryl/Benadryl Cream or Gel Tylenol (Generic) Liquid or Pill Ibuprofen (Generic) Liquid or Pill Vaseline Oragel (Generic) Antibiotic Ointment Generic or Brand Name) Mentholated Cough Drops, Also are peppermints available TUMS or Generic Substitute Sterile Water Eye Drops Note: Dr. McGraw (local physician) has signed orders for the above medications to be given if parent signs permission slip. Other over the counter medications may (on occasion) be given by these guidelines: Parent must bring the medication in to the school nurse in ORIGINAL CONTAINER with guidelines administering the medications still on the bottle or box. Medication sent in plastic bags or in pockets will NOT be administered. Parent must write a note and sign it requesting that the medication be given at school. If medication is not absolutely necessary to be given at school, it must be given at home.

Medication will be given according to the bottle or box. No exceptions will be made.

FREDERICKTOWN R-1 SCHOOL DISTRICT HEAD LICE POLICY

The school nurse will do screenings for head lice. If a student is found to have head lice or nits, a parent/guardian will be notified. The child will need to be treated with a lice treatment product. The child will be checked by the school nurse upon returning to school. If the child is not lice free, the parent/guardian will again be notified. The student will have his or her head checked again for lice by the school nurse in seven to ten days after the first treatment. Once a student is found to have head lice, a classroom head check will be done. All buildings where siblings are located will be notified so that they may be checked for head lice by the school nurse. When the child is sent home with head lice/nits, an instructional sheet will be provided and also reviewed with the person picking the child up from school. In addition, a copy of the district policy on head lice will be sent home at that time. If a student is found to have three consecutive cases of head lice during a school year, a referral will be made to the school-based Social Worker to see if assistance is needed in the home to help eliminate the head lice problem. A child should not need to miss more than one day of school to be properly treated for lice. If a parent feels additional time is needed for treatment, each case will be reviewed individually by the principal, school nurse and/or school-based Social Worker. Head lice/nit removal is the parent/guardian’s responsibility. Neither lice nor nits will be removed by school nurse/personnel.

Epi-Pen Policy

All personnel within the R-1 School District will be trained to administer epi-pens to students who have PERSONAL prescriptions in the nurse’s office and also those students who carry their prescribed epi-pens with them. These students know they will have a reaction and when they do, they need the medication. Students who do NOT have knowledge of a possible anaphylactic reaction and do NOT have a personal prescription must, by law, be evaluated by the school nurse or a medical professional before any medication can be administered. School staff can be trained to use a student’s PERSONAL epi-pen, but they cannot be trained to recognize an anaphylactic reaction and give a student an epi-pen that has not been prescribed specifically for that student. This decision can only be made by the Licensed Nurse or authorized trained medical personnel. If at any time you feel a student is having an anaphylactic reaction and the school nurse is not available, check the medicine cabinet in the nurse’s office for an epi-pen labeled with a prescription for that student. If none is found for that specific student, do not administer anything, and proceed with the following.

Fredericktown R-1 School Nurses will administer epi-pens per above policy in the event of an anaphylactic reaction. If you DO NOT want your child to receive the epi-pen under any circumstance, please see the school nurse and ask for a refusal form. This form must be completed and on file in the nurses office or an epi-pen will be used in case of severe anaphylaxis.

Student Conduct

Positive Behavior Support System

Using the Positive Behavior Support System, behavior expectations will be taught and modeled to students in all areas of the school. Some of the expectations are listed. The classroom teacher will be responsible for classroom discipline and will use measures necessary to ensure the best educational environment for all concerned. Teachers are expected to manage student behavior in classrooms, hallways, grounds, resource rooms for all Level I and Level II Misconduct. Students are given opportunities for improvement of his/her behavior by the teacher re-teaching the correct behavior, reviewing of the FIS expectations, and/or use of alternative placement in classroom (safe seat). If a student continues the misbehavior, then the student may be required to go to another classroom and complete a “Paws and Reflect” sheet to write about their actions and how he/she will correct the misbehavior. If a student’s behavior becomes unmanageable or reaches Level III, the student will be issued a discipline referral and sent to the office. Parents will receive a copy of the office referral form. In serious cases, the parent will be notified immediately.

Discipline Policy/Procedures

A. Students will be treated with respect and are expected to treat others respectfully. Intermediate School students are old enough to understand the school rules and to respect their teachers and classmates. B. Students should not bring toys, games, radios, CD players, trading cards, skate boards, laser lights (it is against Fredericktown City Code for any person under the age of eighteen to possess or use a laser pointer), or other nuisance items to school. C. Bullying: Bullying Definition: Bullying is unwanted, aggressive behavior among school children that involves a real or perceived power imbalance. The behavior is repeated, or has the potential to be repeated, over time. Both kids who are bullied and who bully others may have serious, lasting problems. Types of Bullying

Verbal bulling is saying or writing mean things. Verbal bulling includes: o Teasing o Name Calling o Inappropriate sexual comments o Taunting o Threatening to cause harm

Social bullying, sometimes referred to as relational bullying, involves hurting someone’s reputation or relationships. Social bullying includes:

Fredericktown Intermediate School promotes a bully free school climate. Positive Behavior Support and the guidance curriculum focus on the importance of having a bully free school environment where all students feel safe. Each year students take a Bully Survey; the results are reviewed and, if necessary, changes are made to address students’ bullying concerns. Students are encouraged to report any bully incident to a teacher, the counselor, or the principal. D. CELL PHONE: The Intermediate School requires a permission letter on file from a parent or guardian for students to carry a cell phone to school. Students are encouraged NOT to bring their cell phones to school; however, there will be times that parents want to be able to reach students after school. Students who have a letter on file granting permission to bring their cell phone to school must turn off the cell phone and will not be allowed to use it once they arrive at school and until they leave school grounds. We will follow the District’s guidelines on violation regarding the cell phone policy as listed below: 1st Offense: Warning given. Student asked to put away. Parents notified. 2nd Offense: Confiscation of the phone for the rest of the school day. Parents notified. 3rd Offense: Confiscation of the phone until the parent picks it up. 4th Offense: Confiscation of the phone until the parent picks it up. Students will not be allowed to

have the cell phone at school.

E. Dress Code Policy JFCA-AP(1)

The Board of Education expects student dress and grooming to be neat, clean and in good taste, so that each student may share in promoting a positive, healthy and safe atmosphere within the school district.

Student dress and grooming will be the responsibility of the individual and parents/guardians, within the following guidelines:

1. Dress and grooming will be clean and in keeping with health, sanitary and safety requirements.

2. All students must wear shoes, boots or other types of footwear.

3. Appearance, dress and grooming will not disrupt the educational environment.

4. Class activities that present a concern for student safety may require the student to adjust hair and/or clothing during the class period, in the interest of maintaining safety standards.

5. Additional dress regulations may be imposed upon students participating in certain extracurricular activities.

When, in the judgment of the principal, a student's appearance or mode of dress does not comply with the above criteria, the student may be required to make modifications. No employee or volunteer shall direct a student to remove an emblem, insignia or garment, including a religious emblem, insignia or garment, as long as it is worn in a manner that does not promote disruptive behavior. We ask our students to dress in a manner befitting an educational institution. The following items are not acceptable school attire:

1. No midriff tops, spaghetti straps, or tank tops shall be worn. Shirts and blouses need to cover from neck to shoulders (both sides), have a full back, and be long enough to cover the entire torso.

2. Shoes shall be worn at all times. House slippers are not permitted. 3. Trousers, slacks, shorts, and skirts must be the correct size and worn appropriately, plus be

securely fastened. Shorts must be below fingertip length. No pajamas. No long, floor length coats or “trench coats”.

4. No see-through clothing shall be worn. Undergarments must not show. 5. No hats, caps, bandanas, or other headgear, including sunglasses, are permitted, unless special

dress up days. 6. No clothing which illustrates, with words or symbols, statements condoning alcohol, tobacco, or

drug use, views concerning sexual issues, or depictions of violence or profanity. 7. No student on or about school property or at any school function shall wear, posses, use,

distribute, display, or sell clothing, jewelry, large chains, emblems, badges, symbols, signs, or other things which are evidence of membership or affiliation in any gang or cult.

8. Body piercing that becomes a safety issue is prohibited. 9. No spiked or studded jewelry or large wallet chains. Any jewelry that makes distracting noises,

espouse violence, suggest obscenity, or be possible hazard to the wearer or other students shall be prohibited.

10. Hair style/color that would be deemed a distraction to the learning process is prohibited

Discipline Levels

Level I: These misbehaviors are considered minor and while interrupting the educational process, are not

considered serious. (Ex. Minor rudeness/disrespect, horseplay, inappropriate language, etc.)

Level II: Misbehaviors that are more serious and/or frequent and tend to disrupt the school environment yet,

are non-violent and manageable with Positive Behavior Support. (Ex. Continuation of unmodified Level I

misbehavior, cheating, minor theft, non-compliance, etc.)

Level III: Acts that are directed against persons or property not considered violent yet are disruptive beyond

tolerance and will result in an office discipline referral. (Ex. Continuation of unmodified Level II misbehavior,

Vandalism, Truancy, Overly rough play, etc.)

Level IV: Acts which result in violence to another person or property or which pose a direct threat to the safety

of others. (Ex. Unmodified Level III, Hands on Fighting, Abuse, Assault, Extreme Defiance, etc.)

Technology Acceptable Use Policy

This policy defines the boundaries of "acceptable use" for district technology resources, including computers, tablets, networks (both wired and wireless), electronic mail services and electronic information as detailed below. It includes, by reference, a self-contained compilation of specific rules that can be modified as the electronic information environment evolves. No access will be permitted to the districts technology resources before the district has an appropriately filled out and legibly signed user agreement on file at the Technology Department.

1. All district regulations apply including, but not limited to, Board Policy, Network Acceptable Use Policy,Web Acceptable Use Policy and PC Policy Guidelines.

2. The opportunity to access on-line information is a privilege, which may be revoked by building administration or administrator(s) of the computer system at any time for abusive, unauthorized, or inappropriate conduct. Disciplinary action may be taken under standard school district rules for mis-conduct including but not limited to suspension or revocation of my access to district technology, and/or detention, suspension or expulsion from school.

3. Any behavior with respect to the electronic environment which interferes with the mission or activities of the School

District, or jeopardizes safety, security, or usefulness will be regarded as unethical/inappropriate conduct. Such conduct may include, but is not limited to:

a. The placing or retrieval of any unlawful information on or through the computer system. b. Accessing/using another person's registered account, password, files or email account. c. Keeping passwords confidential and changing them on a routine basis. (A user will not be held responsible

for theft of passwords and ID's, but may be responsible if the theft was the result of user negligence. d. Any use that has the purpose or effect of discriminating or harassing any person or persons on the basis of

race, color, religion, sex, national origin, ancestry, disability, age, pregnancy, use of leave protected by the Family medical Leave Act or the violation of any person(s) rights under applicable laws is prohibited.

e. Making district computing resources available to individuals not affiliated with the district without the approval of the Director of Technology or designated representative. This includes but is not limited to; connecting foreign equipment to wired or wireless district connections and sharing of district software.

f. Using obscene, abusive or otherwise inappropriate language or images in either public or private files or messages. Deleting, examining, copying or modifying files or data belonging to other users without prior consent is prohibited.

g. Using an account, computer/media equipment to obtain, view, download, or otherwise gain access to inappropriate material which includes but is not limited to: text, images, videos, sound files, pornography, pervasively indecent or vulgar advertising any product or service not permitted to minors.

h. Use of district technology for soliciting, advertising, fund-raising, commercial purposes or financial gain is prohibited.

i. Users are required to obey all laws including, criminal, copyright, privacy, defamation and obscenity laws. j. Circumventing district content filters is strictly prohibited including the usage of cellular data. Only district

provided networks (wired and/or wireless) will be used to access the Internet. k. All users must remain within their allotted amount of disk space. l. The copying of district software for use outside of the district is prohibited unless permitted by the district's

software license and approved by district personnel. m. At no time shall district technology be moved or relocated without approval from the Director of

Technology. n. Users may not install software on district electronic devices. o. The introduction of computer viruses, hacking tools, or other disruptive or destructive programs is

prohibited. p. Mass consumption of technology resources which inhibits use by other users is prohibited.

4. When demand for computing resources may exceed available capacity, priorities for their use will be established and

enforced. The Director of Technology or designated contact may set and alter priorities for exclusively local computing/networking resources at any time.

5. Users should be aware there is no expectation as to the right of privacy. The district may monitor technology usage, including but not limited to accessing browse/Internet logs, email logs, any other history of use including non-school owned devices connected to district technology and network directories both public and private. The district reserves the right to inspect any material stored in files to which users have access and will edit or remove any material that the Director of Technology or designated representative, in their sole discretion, believes may be unlawful or harmful at any time.

a. Routine maintenance and monitoring of the system may lead to discovery that the user has or is violating the technology acceptable use policy, board policy, or the law.

b. An individual search will be conducted if there is reasonable suspicion that a user has violated the technology acceptable use policy, board policy, or the law. The nature of the investigation will be reasonable and related to the initial context of the nature of the alleged violation and/or as related to items discovered during initial investigation.

c. Personal files may be discoverable under state public record laws.

6. The school district does not warrant the functions of the system or that it will be error-free or uninterrupted; nor shall it be liable for any direct, or indirect, incidental, or consequential damages (including lost data, information, damage or removable media) sustained or incurred in connection with the use, operation, or inability of the system.

7. Non-school owned electronic devices that are lost or stolen are the responsibility of the user and the district shall

bear no liability.

8. Rules and regulations of the acceptable use policy are subject to change without notice by the Director of Technology or designated representative reflecting district guidelines. All users are subject to these rules and regulations.

9. Use of the district licensed software after graduation and/or enrollment ceases and is prohibited.

10. All network access and privileges will be revoked immediately when a user ceases to be enrolled at the district

regardless of when, during the calendar year, enrollment is dropped.

11. Email accounts for users that graduate will remain active for a period of no more than forty-five (45) days after graduation. This will enable the former user to transition to a new email account so long as the user remains in good standing with the school district. At the end of the forty-five (45) day grace period, the email system will automatically deactivate access to the user account.

12. The district reserves the right to revoke network, email, electronic, wired, wireless, and all other technology access at

any time the Director of Technology or designated representative deems necessary.

13. Users are responsible for all data in their directories both public and private and are responsible for all email originating from the users email account, including but not limited to remote access via mobile devices.

14. Remote devices, regardless of school owned or not, are subject to remote wipe if deemed necessary by the Director

of Technology or designated representative.

15. Users shall use only district approved and district managed communication and email systems.

16. Misrepresentation (including forgery) of the identity of the sender or source of an electronic communication, alteration of the content of a message originating from another person or computer with intent to deceive, the interception or attempted interception of communications by parties not explicitly intended to receive them is strictly prohibited.

17. User accounts that have not been accessed for ninety (90) days will be deactivated.

18. Violation of any of the policies and procedures in this acceptable use policy or violation of any of the policies

referenced herein may constitute disciplinary action. The extent of the disciplinary action shall be determined by the Director of Technology, Building Principal or a designated representative.

Public Notices and Board Policies

504 Public Notice

The Fredericktown R-1 School District, as a recipient of federal financial assistance from the

United States Department of Education and operates a public elementary or secondary education

program and/or activity, is required to undertake to identify and locate every qualified person

residing in the District who is not receiving a public education; and take appropriate steps to

notify disabled person and their parents or guardians of the District’s duty.

The Fredericktown R-1 School District assures that it will provide a free appropriate public

education (FAPE) to each qualified disabled person in the District’s jurisdiction regardless of the

nature of severity of the person’s disability. For purposes of Section 504 of the Rehabilitation

Act of 1973, the provision of an appropriate education is the provision of regular or special and

related aids and services that (i) are designed to meet individual educational needs of disabled

persons as adequately as the needs of nondisabled persons are met and (ii) are based on

adherence to procedures that satisfy the requirements of the 504 federal regulations.

The Fredericktown R-1 School District has developed a 504 Procedure Manual for the

implementation of federal regulations for Section 504 of the Rehabilitation Act, Subpart D. This

Procedures Manual may be reviewed at 704 E. Hwy. 72, Fredericktown, MO 63645.

PUBLIC NOTICE

All responsible public agencies are required to locate, evaluate, and identify children with disabilities who

are under the jurisdiction of the agency, regardless of the severity of the disability, including children

attending private schools, children who live outside the district but are attending a private school within

the district, highly mobile children, such as migrant and homeless children, children who are wards of the

state, and children who are suspected of having a disability and in need of special education even though

they are advancing from grade to grade. The Fredericktown R-1 School District assures that it will

provide a free, appropriate public education (FAPE) to all eligible children with disabilities between the

ages of 3 and 21 under its jurisdiction. Disabilities include autism, deaf/blindness, emotional disorders,

hearing impairment and deafness, intellectual disability, multiple disabilities, orthopedic impairment,

other health impairments, specific learning disabilities, speech or language impairment, traumatic brain

injury, visual impairment/blindness and young child with a developmental delay.

The Fredericktown R-1 School District assures that it will provide information and referral services

necessary to assist the State in the implementation of early intervention services for infants and toddlers

eligible for the Missouri First Steps program.

The Fredericktown R-1 School District assures that personally identifiable information collected, used, or

maintained by the agency for the purposes of identification, evaluation, placement or provision of FAPE

of children with disabilities may be inspected and/or reviewed by their parents/guardians.

Parents/guardians may request amendment to the educational record if the parent/guardian believes the

record is inaccurate, misleading, or violates the privacy or other rights of their child. Parents have the

right to file complaints with the U.S. Department of Education or the Missouri Department of Elementary

and Secondary Education concerning alleged failures by the district to meet the requirements of the

The Fredericktown R-I School District prohibits the use of visual or audio recording equipment on district property or at district activities by students except:

1. If required by a district-sponsored class or activity.

2. At performances or activities to which the general public is invited, such as athletic competitions, concerts and plays.

3. At open meetings of the Board of Education or committees appointed by or at the direction of the Board.

4. As otherwise permitted by the building principal.

Directory Information

Directory information is information contained in an education record of a student that generally

would not be considered harmful or an invasion of privacy if disclosed without the consent of a

parent or eligible student. The district will designate the types of information included in

directory information and release this information without first obtaining consent from a parent

or eligible student unless a parent or eligible student notifies the district in writing as directed.

Parents and eligible students will be notified annually of the information the district has

designated as directory information and the process for notifying the district if they do not want the information released.

Even if parents or eligible students notify the district in writing that they do not want directory

information disclosed, the district may still disclose the information if required or allowed to do

so by law. For example, the district may require students to disclose their names, identifiers or

district e-mail addresses in classes in which they are enrolled, or students may be required to

wear, publicly display or disclose a student identification card or badge that exhibits information that is designated as directory information.

The school district designates the following items as directory information.

General Directory Information – The following information the district maintains about a

personally identifiable student may be disclosed by the district to the school community through,

for example, district publications, or to any person without first obtaining written consent from a parent or eligible student:

Student's name; date and place of birth; parents' names; grade level; enrollment status

(e.g., full-time or part-time); student identification number; user identification or other

unique personal identifier used by the student for the purposes of accessing or

communicating in electronic systems as long as that information alone cannot be used to

access protected educational records; participation in district-sponsored or district-

recognized activities and sports; weight and height of members of athletic teams; dates

of attendance; degrees, honors and awards received; artwork or course work displayed

by the district; schools or school districts previously attended; and photographs,

videotapes, digital images and recorded sound unless such records would be considered

harmful or an invasion of privacy.

Limited Directory Information – In addition to general directory information, the following

information the district maintains about a personally identifiable student may be disclosed to:

school officials with a legitimate educational interest; parent groups or booster clubs that are

recognized by the Board and are created solely to work with the district, its staff, students and

parents and to raise funds for district activities; parents of other students enrolled in the same

school as the student whose information is released; students enrolled in the same school as the

student whose information is released; governmental entities including, but not limited to, law

enforcement, the juvenile office and the Children's Division (CD) of the Department of Social

Services:

The student's address, telephone number and e-mail address and the parents' addresses,

telephone numbers and e-mail addresses.

Law Enforcement Access The district may report or disclose education records to law

enforcement and juvenile justice authorities if the disclosure concerns law enforcement's or

juvenile justice authorities' ability to effectively serve, prior to adjudication, the student whose

records are released. The officials and authorities to whom such information is disclosed must

comply with applicable restrictions set forth in federal law.

If the district reports a crime committed by a student with a disability as defined in the

Individuals with Disabilities Education Act (IDEA), the district will transmit copies of the special

education and disciplinary records to the authorities to whom the district reported the crime as allowed by law.

Law enforcement officials also have access to directory information and may obtain access to

student education records in emergency situations as allowed by law. Otherwise, law

enforcement officials must obtain a subpoena or consent from the parent or eligible student before a student's education records will be disclosed.

Children's Division Access

The district may disclose education records to representatives of the CD when reporting child

abuse and neglect in accordance with law. Once the CD obtains custody of a student, CD

representatives may also have access to education records in accordance with law. CD

representatives may also have access to directory information and may obtain access to student education records in emergency situations, as allowed by law.

Statewide Assessments

The district will implement the components of the Missouri Assessment Program (MAP) in order

to monitor the progress of all students in meeting the standards adopted by the Missouri State Board of Education.

End-of-course (EOC) assessments will be administered in accordance with law and the rules of

the Department of Elementary and Secondary Education (DESE). In courses where EOC

assessments are given, five percent of the course grade will be decided by performance on EOC assessments.

If a student is taking a course that requires an EOC assessment and is failing the course or for

some other reason may be required to retake the course, the district may choose to delay

administration of the EOC assessment until the student has completed the course the second

time. A team consisting of the course instructor, the principal and a counselor will determine

when delayed administration of an EOC assessment is appropriate. In the case of a student with an individualized education program (IEP), the IEP team will make the determination.

The School Board authorizes the superintendent to establish a process designed to encourage

the students of this district to give their best efforts on each portion of any statewide

assessment, which may include, but is not limited to, incentives or supplementary work as a consequence of performance.

The district’s policy on student participation in statewide assessments shall be provided at the

beginning of the school year to each student and the parent/guardian or other person

responsible for every student under 18 years of age. The policy will also be kept in the district office and be available for viewing by the public during business hours of the district office.

National Assessment of Educational Progress

If chosen, the district will participate in the National Assessment of Educational Progress (NAEP) as required by law.

The Fredericktown R-1 School District is required to inform you of certain information that you,

according to the NO Child Left Behind Act of 2001 (Public Law 107-110), have the right to

know.

Upon your request, the Fredericktown R-1 School District is required to provide to you in a

timely manner, the following information:

• Whether the teacher has met state qualification and licensing criteria for the grade

levels and subject areas in which the teacher provides instruction.
