Financial support for death or terminal illness

Death and Terminal Illness (TI) Bene�t*

If you die, the full sum insured will be paid to your bene�ciaries. We can make a S$5,000 advance payment of your sum insured to help manage funeral expenses.

If you are diagnosed as terminally ill, the full sum covered will be paid to you or your bene�ciaries.

2-Year Term Life cover for your spouseWe understand that if you were to die, your family will need time to build a life without you. To protect them further, we will provide your spouse a 2-year premium-free insurance policy with a sum insured of S$250,000 for death*

* What is Terminal Illness? Terminal Illness is the conclusive diagnosis of an illness or condition that is expected

to lead to death within twelve (12) months.

This diagnosis must be supported by a specialist and con�rmed by our appointed Medical Practitioner.

*Surviving spouse must be less than 60 years old to qualify for the 2-year premium-free insurance policy.

We will assign a Claims Concierge to help retrievemedical reports if needed to process the claim. Wewill also provide an advance of S$5,000 from yoursum insured to your spouse upon your death, tomanage costs before the full claim is paid out.

We will pay for up to 10 counselling and life coaching sessions with a counsellor; but we will only pay up to S$5,000 in total for all sessions.

We will pay for up to 3 legal advice sessions with alegal adviser; but we will only pay up to S$5,000 intotal for all sessions.

Waiting PeriodThese de�nitions are prescribed by the Life Insurance Association of Singapore (LIA)

The Life Insurance Association Singapore (LIA) has standard De�nitions for 37 severe stage Critical Illnesses. These Critical Illnesses fall under Version 2019. You may refer to www.lia.org.sg for the standard De�nitions (Version 2019).

The above applies even if the signs or symptoms were not apparent to the person insured, if they would have been apparent to a reasonable person in the same place.

Before a claim can be made, the following critical illness conditions need to be diagnosed (or the signs or symptoms �rst became apparent to the person insured) more than 90 days of cover starting, being reinstated or being increased (for the increased amount).

We will pay you 100% of the rider sum insured covered if the person insured su�ers 1 of the 36 critical illnesses; and

*for angioplasty & other invasive treatment for coronary artery treatment we will pay 10% of sum insured under this rider cover subject to S$25,000 maximum sum insured.

Total & permanent disability

De�nition of Total & Permanent Disability rider

For a person insured under age 75 who is working for remuneration when they �rst su�er an illness or injury:

Means the person insured has su�ered an illness or injury which has caused them to be unable to work for at least six months in a row, and to be totally and permanently unable to engage for pro�t or reward in: ● the occupation they were engaged in when the illness or injury �rst happened, and ● any other occupation.

For a person insured under age 75 who is not working for remunerationwhen they �rst su�er an illness or injury:

Means the person insured has su�ered an illness or injury that requiresphysical assistance of another person for at least six months in a row, and they are totally and permanently unable to perform at least 2 of theactivities of daily living without someone’s assistance.

For any person insured up to age 75:

Means the person insured has su�ered the total and irrecoverable loss of: ● the sight of both eyes, or ● the sight of one eye and the loss of use of one limb at above the ankle or wrist, or ● the loss of: ● both hands at or above the wrists, ● both feet at or above the ankles, or ● one hand at or above the wrist and one foot at or above the ankle.

Activities of daily living

The following activities.

● Washing - the ability to wash in the bath or shower (including gettinginto and out of the bath or shower) or wash satisfactorily by othermeans;

● Dressing - the ability to put on, take o�, secure and unfasten allgarments and, as appropriate, any braces, arti�cial limbs or othersurgical appliances;

● Transferring - the ability to move from a bed to an upright chair orwheelchair and vice versa;

● Mobility - the ability to move indoors from room to room on levelsurfaces;

● Toileting - the ability to use the lavatory or otherwise manage boweland bladder functions so as to maintain a satisfactory level of personalhygiene;

● Feeding - the ability to feed oneself once food has been prepared andmade available.

We will not pay any bene�t under this rider if the claim arises:

● from suicide, attempted suicide or an intentional self-in�icted act; and

● within one year of the start of your rider cover, or the date we last reinstate(restart) your rider.

This applies regardless of the mental state of the person insured.If this happens, the policy will be cancelled.

We will not pay any bene�t under this rider if the claim arises because you or the person insured deliberately participated in an unlawful act, or failed to act in accordance with the law.

Note: Age refers to age at the last birthday

If you change your mindIf you aren’t completely satis�ed with your policy, and you haven’t made a claim under it, you have 14 days from the date you receive your policy to cancel it and receive your premiums back, less medical expenses (if any).

When we won’t pay any bene�tsAt FWD, we do not believe in setting a long list of hidden clauses that prevent us from giving you what is due in the event of a claim. So we’ve simpli�ed our exclusion list to just 2 criteria:

Unlawful acts

Person insured

You or your spouse or youremployee

Your child

Age when you can apply for cover When cover ends (whichever is earlier)

Cover ends at age 100 or an age where we cannot o�er another full 10 year term before the person insured reaches age 100 or up to chosen policy term.

Must be between 18 and 65 years old.

Must be 1 month old when they �rstapply for cover.
