5. List of training attended by Cooperative officers( 2011-2013)

6. Amended articles of Corporation/by laws under RA9520

7. Certificate of Good standing for regular purpose

8. Certificate of tax exemption/ Application duly received by BIR

9. Cooperative Development plan

10.List of training conducted/initiated for FY 2013

11.Valid Certificate of Accreditation as Cooperative Training Providers, if applicable
