General Risk General Risk Management Q. Do I need to complete a Risk Assessment? A. The University of Western Australia requires all staff and students to manage risk on a daily basis. A formal, documented Risk Assessment is required to be completed and submitted to line management when: the activity has a high or extreme risk rating; applying for grants or other funding; a project is being planned and also during its ongoing management; and travel is to a region and/or the activity is high risk (see Travel Risk below). The following UWA approved documents are required to do this task: 1. UWA Assessment Template 2. UWA Risk Matrix Template – included Likelihood and Consequences descriptions -- Q. General Risk Management Q. Do I need to complete a Risk Assessment?

A. The University of Western Australia requires all staff and students to manage risk on a daily basis.

A formal, documented Risk Assessment is required to be completed and submitted to line management when:

A. All Risk Assessments are submitted to the line manager, project lead or senior research officer.

Risk Q. Do I need to complete a Travel Risk Assessment - Destination and Activity?

A. You should always assess the risks of all travel as part of your preparations. A formal Travel Risk Assessment is required to be completed and submitted when the destination and or activity are deemed to have a high or extreme risk rating.

Q. How do I know if my travel plans fall into either high or extreme destination or activity risk categories?

A. The travel booking system (Concur) will automatically trigger if you select a country that is considered to have a high risk rating or above. In addition to this you will be required to identify your activity when you fill in the ‘Activity’ field in Concur – some of the options are considered to have high or extreme risk ratings and the Concur system will inform you if a formal Travel Risk Assessment is required.

A. It is easier to complete the Assessment if you have received training. However there are several resources available on the UWA Legal and Risk division webpage that have been developed to help novices.

Q. Can I book travel if I haven’t filled in a Risk Assessment and one is required?

A. You can request quotes and also fill in the booking forms in Concur. However you will not be able to pay for the tickets until a UWA Level 4 Approver has accepted your Travel Risk Assessment and approves travel. The Approver is informed of the requirement for an Assessment at the same time as the person booking the travel. The system can not issue tickets until the Approver has completed their processes and approved travel in the system.

Q. Is travel approval automatic?

A. No, travel approval is not automatic on submission of the Travel Risk Assessment.

Although the traveller/s may have submitted a Travel Risk Assessment this assessment has to be approved by the UWA Level 4 Approver in the Concur system. If the Approver is not satisfied with the Assessment they can

1) not approve outright

2) approve with conditions (the Assessment has to be updated and resubmitted)

3) request the Assessment is reviewed and a more detailed document submitted.

Q. What do I do if travel is not approved for any reason?

A. There is an appeal process. Details can be found on the UWA Legal and Risk webpages.

Q. Who needs to fill in the Travel Risk Assessment?

A. The Assessment must be completed by the traveller. Where more than one person is traveling together and/or participating in the same activity a single Assessment can be submitted by one traveller and the names of others in the group recorded in Concur. It is highly recommended that everyone in the group assist in the development of the Assessment. Everyone travelling must sign-off on the Assessment and the mitigations. See UWA Legal and Risk webpages for instructions on the requirements for UWA Travel Risk Assessments.

Q. What do I do if I am going to a high risk area and undertaking a high risk activity?

A. You will need to complete 2 Travel Risk Assessments, one for destination and one for activity.
