Executive Summary

The 2017-2018 physician renewal cycle represents the 32nd year the Georgia Board of

Health Care Workforce has examined physician supply and demand in the State of

Georgia. The purpose of this document is to measure and track trends in physician

demographic characteristics, supply, and distribution using data from the licensure and

license renewal process from January 2017 to December 2018. The physician data is

compiled, validated, and analyzed by the Georgia Board of Health Care Workforce staff

and presented on the state, county, and Primary Care Service Area (PCSA) levels.

This document highlights trends in physician workforce patterns such as:

Physician demographics by specialty, age, gender, plans to retire, and race.

The total number of physicians and rate per 100,000 people in the population.

There were a total of 36,175 physicians who renewed their licenses (32,094) or applied

for a new license (4,081) during the 2017-2018 cycle. Of those, 13,704 were removed

because they indicated that they either: were retired (481) or did not work in the state of

Georgia (13,223). This left a valid physician workforce of 22,471 physicians who are

licensed and actively practicing in a Georgia location.

For the entire state of Georgia, the overall rate of actively licensed and practicing

physicians per 100,000 population is 213.6, compared to the national average of 301.1

Note, however, that this national average did not use the same criteria to exclude retired

or out of state/duplicate licensees.

Physician Workforce Trends Since Last GBHCW Report in 2015

Georgia’s population has grown from 10,214,860 in 2015 to 10,519,475 in 2018,

a growth of 3%.

The rate of physicians per 100,000 population has increased from 209.7 in 2015

to 213.6 in 2018.

The total number of physicians in primary care/core specialties has increased

from 9,269 in 2015 to 12,670 in 2018. Note that since the last report in 2015,

Emergency Medicine and Psychiatry have been added to primary care/core. If

these two specialties are removed from the comparison, Georgia has still seen a

growth in primary care/core to 10,322 in 2018, which represents 11% growth

over the time period.

49% of all physicians practicing in Georgia work in the top 10 ranked PCSAs,

which accounts for 27% of Georgia’s population. This is a decrease from 51% in

2015.

Demographics

51.8% of the physician workforce is aged 50 or above, which is a decrease from

2015 when 53.4% of the workforce fell into this age grouping. Still, this is quite a

large increase from the year 2000, when 33.8% of the workforce was age 50 or

older.

12% of physicians plan to retire in the next 5 years, with 54% of those planning to

do so being between the ages of 61-70 at the time of licensure or renewal.

Women make up 34% of the physician workforce, which is an increase from the

2015 figure of 31.3%. Together, this indicates that the female percentage of

Georgia’s physician workforce is nearing the Federation of State Medical Board’s

national average of 35.1%1.

77.7% of physicians accept Medicaid, while 72.4% accept new Medicaid

patients. 80.8% of physicians accept Medicare, while 77.2% accept new

Medicare patients. These figures are down slightly from the 2015 report, but this

may be in part due to data missing from the analysis of newly licensed

physicians.

Internal medicine is the largest primary care/core specialty, accounting for 15.7%

of the workforce. Next largest is family medicine at 12.4%, then pediatrics at

8.3%.

Women make up a majority of physicians in two primary care/core specialties:

OB/GYN (54.9%) and pediatrics (64.0%). This is a change from 2015, when

women were the majority of physicians only in pediatrics.

The primary care/core specialty with the least gender parity is general surgery,

where men account for 85.3% of the workforce. The primary care/core specialty

with the most gender parity is OB/GYN, where there is a difference of less than

10 percentage points between women and men.

Methodology

The licensure survey population analyzed here consists of two years’ worth of data

collected by the Georgia Composite Medical Board. The survey is not mandatory,

which means that some data is missing for analysis purposes. GBHCW staff receives

the data from the Medical Board and then compiles and validates the survey data to

create the analyses presented here. This year, GBHCW is able to analyze new

licensees in addition to license renewals.

Physician work location is derived from multiple different fields in the licensure survey,

so this warrants a brief explanation of how each person was counted only once.

Physicians input up to three work counties and three zip codes, potentially yielding 6

different work locations. To simplify the analysis process, a physician is counted only in

one location, which may yield a conservative estimate of the workforce in each county.

However, a physician could also have put the same county or zip code for all six fields,

but should only be counted as one person, not six.

If a physician did not enter a first work location county, then their second (or if none,

third) work location county was counted as their work location. If no Georgia work

location counties were entered, then their first, then second, then third work location zip

code was used. If a physician entered only one work location zip code, then they were

counted for the county that contains that zip code. This means that if a physician

entered that they work in any Georgia county or valid Georgia zip code, they are

counted as part of the workforce of 22,471.

The rate of total physicians per 100,000 population was then calculated for each county

and PCSA. In addition, rates per 100,000 were calculated for each of the seven primary

care/core specialties by PCSA and by county.
