STATEMENT OF NEED

As the nation’s older population grows, the US will require a well- trained workforce of health care providers with expert knowledge and skills in geriatric medicine. This CME activity, featuring experts in Geriatrics, emphasizes core competencies related to optimal geriatric care. A multidisciplinary approach will be demonstrated in case-based discussions with audience interaction. A wide range of topics address common geriatric syndromes and chronic diseases as well as strategies to coordinate care in a variety of settings – home, physician’s office, hospital, and skilled nursing facilities.

TARGET AUDIENCE

This activity is intended for physicians and allied healthcare professionals who provide care for geriatric patients.

LEARNING OBJECTIVES

n Assess and apply to practice core competencies related to optimal geriatric care:

- Medication management - Cognitive, affective and behavioral health - Complex or chronic illnesses - Ambulatory care - Transitions of care - Palliative and end of life care

n Recognize and appropriately manage geriatric syndromes in older patients.

n Develop strategies to work in a multidisciplinary environment to coordinate care for older patients in a variety of settings.

n Identify resources available in the community for better care of the elderly.

ACCREDITATION AND DESIGNATION OF CREDITS

Physicians: Stanford University School of Medicine is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

Stanford University School of Medicine designates this live activity for a maximum of 10.5 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Nurses: The California Board of Registered Nursing recognizes that Continuing Medical Education (CME) is acceptable for meeting RN continuing education requirements, as long as the course is certified for AMA PRA Category 1 Credit(s)™ (rn.ca.gov).
