Siemens Pakistan has received the“Environment Excellence Award” presentedby the National Forum for Environmentand Health. This award was instituted threeyears ago and Siemens has been a recipientever since. Mr. Mansoor Iqbal, GM, QualityManagement Systems, received the awardfrom the Minister for the Environment Mr.Malik Amin Aslam at a special ceremony.

Diesel Generating Sets AreConsumer’s Choice In Pakistan

Siemens Pakistan Diesel Generating Setswon the “Consumer’s Choice Award,” whichis based on a customer survey thatmeasured not only the product but alsothe service quality. This award waspresented by the leading industrialist ofthe country, Mr. S.M. Muneer, and receivedon behalf of Siemens by Mr. MurtazaAbbas, General Manager A & D.

As we complete thelast quarter it istime to consolidateour success andpush forward to not

only attain our goals but to give a pleasantsurprise by surpassing them in a big way.It was Vince Lombardi who said “The priceof success is hard work, dedication to thejob at hand, and the determination thatwhether we win or lose, we have appliedthe best of ourselves to the task at hand.”This is the only way to go in the fiercelycompetitive world of today where onlythe best can survive. We have the tools of

and the fit4more principles to guideus and the resources of a world classcompany at our disposal. I see no reasonwhy we cannot achieve and surpass thetargets we had set for ourselves last year.

As I see it there are a few things on whichwe need to concentrate in order tosucceed. The first thing is discipline. If wedo not have discipline in our lives we cannever succeed. If you look at the successfulcountries of the world you will realize thatthe cornerstone of their success isdiscipline. Without discipline all efforts arewasted and while you might be very busyand work till late at night if you are notdisciplined you cannot produce the desiredresults. It was Thoman Jefferson who said“That government is best which governsthe least, because its people disciplinethemselves.” This is also true forcompanies. If you are disciplined in yourwork you will not need constant guidance

from the top.

Another important factor is productivity. This is the benchmark for investment in acountry. You might find cheap labor in acountry but if it is not productive accordingto world standards then investors will bereluctant to invest. Now there are worldfigures on the productivity index ofdifferent countries and they form the basisfor international investments. We need toincrease our productivity by every meansat our disposal and the tool providedby the company should be used as itensures maximum productivity andminimum waste of time and resources.

I have always said that smart work is betterthan hard work and now as you aresteaming ahead to surpass your targets togive us a pleasant surprise you will do wellto think about smart work. This is the sumof all the qualities described above.Increasing productivity through disciplineand smart thinking. Not wasting time inbeating around the bush but facing theproblem squarely and finding lastingsolutions. Inspire others by your exampleand lead from the front.

I wish you every success and look forwardto the pleasant surprise you will give atthe end of the last quarter.

If you don't finish what you start, you will be distracted by amountain of unfinished business.

It is a sign of character and professionalism to live up to yourcommitments. It has been said that the most valuableemployees are the ones who consistently do what they saythey will. The same can be said of managers. Closing businessgives people confidence in their leaders. It also inspires (andsubtly demands) that they do the same.

Another angle on this is your ownexample of productivity and efficiency.

The more "loose ends" you have in yourwork style, the greater the likelihood of mistakes

or poor work. A pile of unfinished work creates a situation inwhich you lack focus and drain your mental energy.

Confucius once said:The superior man is distressed by the limitations of his ability;he is not distressed by the fact that men do not recognize theability that he has.

Ionizing radiation is used as a standardmethod of cancer treatment.Radiotherapy can be used alone or incombination with chemotherapy andsurgery. One type of treatment ormodality of treatment may be chosenover another for its advantage in cureand for its advantage in minimizing sideeffects. For treatment purposes, mostradiation oncology departmentsgenerally use a linear accelerator thatproduces high energy electrons and X-rays (about 100 times more energeticthan diagnostic X-rays). The radiationbeam is shaped using a complexcollimator device (typically a multileafcollimator – MLC), while the patient lieson a high-precision table. Additionally,a linear accelerator can be equippedwith an imaging system (electronicportal imaging device – EPID) used toverify correct patient position prior totreatment delivery.

Ever y day around the globe, approximately 77,000 patients receiveradiation therapy delivered by Siemenslinear accelerators. And next year alone,2.6 million newly diagnosed cases willbe reported worldwide, pressuring clinics

to narrow the “15-minute slot.” Ourvision is to deliver evidence-basedsolutions that can fast-forward thecapabilities of even modest oncologypractices. Many customers are nowusing advanced techniques, such asIntensity-Modulated Radiation Therapy(IMRT) and Image- Guided RadiationTherapy (IGRT) in their clinics. IMRTrequires the clear visualization of thetarget area with the patient in treatmentp o s i t i o n . I G R T u s e s l i n e a raccelerator–based 2D and 3D pre-treatment imaging to identify dailytumor displacement, locate organs atrisk, and provide other pertinentinformation to help determine if patientpositionadjustmentss h o u l d b emade prior tot r e a t m e n t .IMRT and IGRTboth use lineara c c e l e r a to rbased imagingt o m o r esuccessfullytreat a widerrange of tumorsites. Siemensoffershardware andsoftwaresolutions thatenableclinicians tocost-effectively deliver these types ofhigh-quality treatment applications.

The Siemens linear accelerator offeringis comprised of two product lines:PRIMUS and ONCOR. As with all Siemens

medical equipment lines, our linearaccelerators are highly integrated witha common SYNGO software platform. Both PRIMUS and ONCOR offer a widechoice of photon (X-ray) ranging from4MV to 18MV and electron energiesranging from 5MeV to 21MeV toaccommodate all clinical needs andspecific treatment techniques. ONCORis the latest addition to Siemens linearaccelerator portfolio. We recently setupS.I.U.T Hanifa Suleman DawoodOncology Center at Karachi with ourstate of the art ONCOR Lineara c c e l e r a t o r , V s i m a n dKONRAD/PROWESS Treatment PlanningSystems. BANGKOK

Siemens played a major role in theconstruction of Bangkok's local publictransport system. In fact, all the light railsystems in Bangkok are based on Siemenstechnology.

A new subway and the Skytrain, whichtogether move half a million passengersper day, are relieving traffic congestionand helping to improve air quality. TheSkytrain travels 12 meters above thestreets of Bangkok at a speed of up to 80km/h, while the average speed of the carsbelow frequently stagnates at 10 km/h.

The turnkey package provided bySiemens for the subway and theSkytrain included signaling systems, the entire telecommunications system, power supply, monitoring technology, some of the track work and the trainsthemselves. After only 39 months of

construction time, the customer wasable to put the system into operation, with everything coming from a singlesource and at a fixed price.

A long-term maintenance contract wasalso signed to ensure the safety, reliability and punctuality of the trains. In the first five years of its operation, the Skytrain exceeded all agreed-uponavailability targets: More than 98.5percent of the trips taken arrived attheir destination with no more thanone-minute delays. As a result, theoperators of the subway system alsoopted for a turnkey solution fromSiemens.

The new 28-kilometer Airport Raillinkshuttle from Siemens, which willconnect the New Bangkok InternationalAirport with downtown Bangkok, isscheduled to go into operation beforethe year is out.

The proof of the pudding according to an old proverb is in the eating. Here are some examples of Siemens providing successful solutions to major problems faced by mega cities around theglobe. In this issue we will focus on transportation related solutions.

MUMBAI

Millions of people ride the commuter trainsfrom Mumbai's suburbs to the city centerevery day. The Indian Railway Ministryhas awarded Siemens a major contractthat will stretch over three years for theproduction and delivery of the completeelectrical equipment and drive systemsfor 170 three-section trains. The scope ofdel iver y inc ludes pantographs, transformers, traction and auxiliary

inverters, traction motors, a train controlsystem and a passenger informationsystem. The new motor train sets to bedelivered will be used on regional lines inthe Mumbai region. The systems, whichhave been adapted to local climatic andenvironmental conditions, will reduceenergy consumption by up to 30 percentand also lower maintenance and servicingcosts. Passenger comfort will be enhancedby IGBT electric inverters that make forlurch-free acceleration and braking.

SPECIAL REPORT HONGKONG

Hong Kong's public transport systemmoves more than 7.7 million peopleevery day, and the traffic density of itsroad network is one of the world'shighest. Every day almost four millionpassengers use the over 200 kilometerlong railway system of the two largestrailroad companies (MTRC and KCRC).This is one of the most intensively usedsystems in the world. Some of the linestransport 70,000 passengers per hourduring commuting hours. Siemens isone of the prime partners of the railoperators: as supplier of the rollingstock and the traction power, thesignaling, control and communicationssystems, and the electrification of theroutes. Siemens is also a key providerof the communications systems for trainstations.

Just as the railway system, the roadnetwork also has one of the highesttraffic densities in the world. Siemenshas been awarded a contract todevelop, deliver and install an extensivetraffic control system for the fast-growing administrative districts of TuenMun and Yuen Long in Hong Kong. At

the moment, the traffic light systemremains uncoordinated - i.e. eachintersection is individually controlled. That will soon change, however, thanksto the new Urban Traffic Control (UTC)system, which will regulate vehicle andpedestrian traffic and coordinate trafficlights. It will reduce vehicle waitingtimes at major intersections, which inturn will cut down on driving time andincrease the efficiency of the roadnetwork. The UTC system will also belinked up to the existing urban railnetwork in the administrative districts, which moves a round 400,000passengers each day. Following theplanned completion of the UTC systemin August 2008, Siemens engineers willtake on responsibility for systemmaintenance for a period of five years.- Siemens has also set up a trafficmonitoring system for the AberdeenTunnel in the southern part of HongKong that is equipped with sensors andintelligent cameras that automaticallymeasure traffic density and speed, andalso identify and report problems suchas accidents and traffic jams. Inaddition, LED displays are used as aninformation system for car drivers.

CHINA

Siemens and China Railway Signal &Communication Corp. are currentlyequipping the approximately 25-kilometer long underground Metro Line10 with state-of-the-art control, signaling, and safety technology. Thesystem will make it possible to achieveshort intervals between successivetra ins and fast adaptabi l i ty topassenger volumes. Line 10 will gointo operat ion for the SummerOlympics in 2008 and will link the siteof the games with the center of Beijingand other subway lines. The scope ofequipment being supplied by Siemensincludes a train control system, theoperational center, signal boxes, radiosystems, and onboard components forthe 34 trains that run on Line 10. Thisis the second subway project in Chinathat involves the innovative traincontrol system from Siemens (the firstwas the Guangzhou Metro). Siemenshas also been awarded a contract forhigh-speed trains: Together with itsChinese partner Tangshan Locomotive& Rolling Stock Works, Siemens willbuild 60 trains (top speed: 300 km/h)

that will initially be used on the Beijing-Tianjin route beginning in 2008.Tianjin will be one of the sites for theOlympic soccer matches in 2008. Thetrains will be based on the Velaroplatform from Siemens, which housesthe motors, brakes and transformerson the undercarriage. The advantageof this platform is that it enables thetrains to offer 25 percent more spacefor passengers without having toincrease the length of the trains.Infrastructure maintenance costs arealso kept low through the use of theplatform.

In 2004 the Shanghai a irpor t ’ sTransrapid maglev rail link also wentinto commercial operation. This iscapable of a top speed of 430 km/hand covers the 33 kilometers toLongyang Station in less than eightminutes. Siemens likewise supplied thesignaling and telecommunicationstechnology, the overhead power supplyequipment and 96 subway trains fortwo Shanghai Metro lines as well asb e i n g re s p o n s i b l e fo r p ro j e c tmanagement

MOSCOW

One of the new highways in Moscow isthe "Third Circle" interurban beltway, whichwas completed in December 2003.Siemens is currently installing anautomatic traffic control system for the34-kilometer highway that consists of acentral traffic control computer, a videomonitoring center and a meteorologicalwatch center. The system also includesroad traffic and weather condition sensors, variable traffic signs, video cameras andappropriate infrastructure, including a datatransfer system. The entire control systemshould be ready for operation by the endof 2006.Siemens has also been awarded a contractto deliver eight Velaro-type RUS high-speedtrains to Russian Railways (RZD) and alsoassume responsibi l i ty for theirmaintenance for a period of 30 years.Contract volume for the trains and servicewill total approximately €600 million.Designed to travel at 250 km/h, these newtrains will be used initially on the Moscow-St. Petersburg route, and later along theroute between Moscow and NishniNovgorod. The Velaro platform offers the

advantage of having all drive equipmentmounted on the undercarriage, whichenables the trains to offer 20 to 25 percentmore space for seating, without havingto increase their length. Maintenance costsare also reduced due to lower axle loads.Domodedovo International Airport is theleader among Moscow's threeinternational airports in terms of passengerand cargo traffic - and Siemens has beenawarded the contract to install a state-of-the-art parking management system for1,400 parking spaces at Domodedovo.Plans are already in place to expand thesystem in the future to enable drivers topay for parking with their credit cards, forexample. Sheremetyevo Airport has beenbenefiting from a Siemens video-dockingsystem since 2003. The systemautomatically provides pilots withguidance information during the dockingprocess, thereby helping to increase airportcapacity, plane safety and the safety offlight and ground personnel. The systemuses video sensors and an imageprocessing unit that locates and monitorsaircraft approaching the gate. Pilots areguided via an LED display, which is locatednear the gate and is easy to see.

LONDON

The London bus network comprises 700routes, which makes it one of the largestand most complex in the world. Each dayit carries some six million passengers.Siemens is currently implementing asatellite-tracking system for the network, which will be able to determine theposition of 8,000 buses in real time. By2009, electronic panels mounted at 2,000bus stops will display important passengerinformation based on this data, includingthe time to arrival of approaching buses. In addition the operations control systemwill calculate the punctuality of bus

services, thus providing an important basisto determine the quality of serviceprovided by transport operators. Siemensis supplying systems to monitor roads andautomatically identify license plates foruse in conjunction with the road toll nowlevied in the heart of London. Manysuburban, regional and intercity rail linesrun trains supplied, serviced andmaintained by Siemens. Likewise Siemenssupplied, services and maintains the trainsused by the two airport links HeathrowExpress and Heathrow Connect. Last butnot least, many of the traffic lights andother traffic systems in London aresupplied by Siemens.

NEW YORK

Siemens is currently modernizing the 100-year-old New York City subway throughthe installation of a train control andmanagement system that is the mostadvanced of its kind in the U.S. The RailControl Center can monitor in real timethe location and speed of each of the 540trains that run on the subway's 1,100kilometers of track. The information isdisplayed on large-format screens, andtechnicians can also control trains via

radio, thereby improving the reliabilityand punctuality of the system and makingit possible to utilize a greater number oftrains. Passengers will also benefit from apassenger information system thatautomatically generates the correct noticesand announcements for every subwaystation. Siemens is also in the developmentstage of a telematics project that willimprove the efficiency of New York Citybuses by providing commuters with kiosksthat offer near real-time information onlocations and arrival times.

BERLIN

Europe ' s mos t modern t r a f f i cmanagement center and traffic controlcenter are located at Tempelhof Airportin Berlin, where data from 22 trafficcomputers, 2,000 traffic lights, andhundreds of Siemens Traffic Eye infraredsensors and video cameras fromthroughout the Berlin metropolitan areacomes together. Specialists at the trafficcontrol center use the data to adjust andregulate electronic traffic signs andtraffic lights. The neighboring trafficmanagement center, which is operatedby Siemens and is also being further

developed by the company, processesdata on public transport systems androad traffic and combines all theinformation into an inter-modal trafficinformation system. This systemcontinually provides users with up-to-date information, as well as statisticaldata and traffic forecasts, via the Internet,or else by means of dynamic electronicinformation boards posted throughoutthe city.In 2005, Berlin also became the first cityin Germany to introduce a mobileparking fee system from Siemens thatallows motorists to pay parking fees viatheir cell phones

SINGAPORE

Mobility / transport / traffic

Singapore has an excellent public

transport system - and Siemens has

supplied all the trains for the Woodlands

MRT line. Singapore also installed a fully

automatic road toll system back in the

early 1990s in order to reduce smog and

traffic congestion.

KARACHI

TRAFFIC LIGHTS

Karachi now a bustling Mega City with apopulation of 15 million and growing atthe rate of 2.5 % requires a large andefficient traffic control system. Followingare the highlights of Siemens involvementin this system the backbone of trafficmanagement in the city

Siemens Pakistan has been in the trafficsignal business in Karachi since 1960.The road network consists of 5000 km inmetropolitan Karachi with 1.5 millionregistered vehicles (up to 2004). As anaverage every year 10% traffic volume isincreasing in the city. Siemens providesthe services with technical support fromSiemens Germany and Siemens TrafficControls, UK. Siemens provides onewindow operation from designing toinstal lation, commissioning andMaintenance. 85% of traffic signals inKarachi are part of this system.Currently Siemens is maintaining all thetraffic signals (120 sites) in Karachi withinthe jurisdiction of City District Government,Karachi.

The UTC system project was approved bythe Government of Pakistan in 1991.Tender floated in the same year andcontract was awarded to Siemens Pakistanin Jan, 1992. The contract was planned

to be executed in three phases that were:- Phase I included Supply, Installation,Commissioning of 57 nos. controllers andTraffic signals Equipments for 62 sites atKarachi with Allied Traffic EngineeringWorks and operation and maintenance for04 years.

- Phase II included setting up of CentralControl Room and computerization /linking of above mentioned sites and theiroperation and maintenance.

- Phase III included the expansion of thePhase I and Phase II of the project forfurther 138 sites. Uptil now around 45new intersections have been added aftercompletion of Phase I.CDGK are nowplanning to go for Phase II of the projectwhich includes setting up of CentralControl Room and computerization /linking of around 120 sites.

Due to lack of funds only Phase I wasimplemented.

Many of us routinely do things acertain way because we've alwaysdone it that way or we were taughtto do something in a certain wayand never considered change. One often hears praise for a managerwho keeps a clean desk and delegatesall tasks to his subordinates. On theother hand managers with a cluttereddesk and busy working are frownedupon for not delegating adequately. Themyth of delegations needs to beanalyzed in situational perspective forcorrect understanding of managementstyles.

When and whom to delegate is the keyquestion. In this context one mustconsider the competence level of thede legate . Competence versusincompetence, in a situational scenariocarries a different meaning from thenormal understanding of these words. One may be competent to drive a carby virtue of possessing a valid drivinglicense and many years of self drivingexperience, however such attributesdoes not make one competent to drivea 40 ton trailer on the same road androute. Competence is an attribute inrespect of skill and can be developedwith training.

In real life, we encounter fresh graduatesand experienced professionals whosecommitment to the job may be high onday one, but their competence for the

defined position, low. Delegation insuch a situation is hardly desirable. Theonly communication mode that can beeffective is “direction”. The incumbentparticularly fresh graduates shouldinitially be inducted into work streamby the manager directing him or her toaccomplish small tasks. At this stage, direction builds confidence and helpsthe incumbent to accl imat izehim/herself with work process andenvironment.

The next stage is “supervision”. Thismeans the manager (or the traineefather, as the case may be) passes onthe entire assignment to be processedby the subordinate, however keeps aclose eye on the progress. Regularguidance is necessary during thisprocess for developing adequate levelof confidence. Incumbents with positiveattitude gets over this phase fairly fast.

Next step to delegation is “coaching”,where the difference from “supervision”is the replacement of constant watchby periodic reviews. With better resultsthe milestone for delegation is reached.

It must be understood that delegationis not a permission to act completely on

ones own. Reporting progress at definedintervals and timely reporting exceptionsare absolutely essential for the successof delegation process/concept.Delegation is not abdication. The persondelegating a task remains responsiblefor it notwithstanding the degree ofdelegation he/she may have employedto complete the task.

Can all tasksbe delegated?The answer isn o ; w h e nconsider ingpractical life.Certain tasksc a n n o t b edelegated andsome shouldn o t b ed e l e g a t e d .Someexamples are,

· Policy making· Personnel decisions· Handling crisis

One must also take care that the taskyour immediate superior has asked youto do personally, should never bedelegated.

To Delegate or not?BY: Sharjeel Jawaid, BU TD

One of the most memorable casestudies on Japanese managementwas the case of the empty soapbox, which happened in one of Japan's biggest cosmetics companies. Thecompany received a complaint thata consumer had bought a soap boxthat was empty. Immediately theauthorities isolated the problem to the assembly line, which transportedall the packaged boxes of soap to thedelivery department. For somereason, one soap box went throught h e a ss e m b l y l i n e e m p t y.Management asked its engineers tosolve the problem. Post-haste, theengineers worked hard to devise anX-ray machine with high-resolution monitors manned by two people towatch all the soap boxes that passedthrough the line to make sure they

were not empty. No doubt, they worked hard andthey worked fast but they spent awhoopee amount to do so. But when a rank-and-file employee in a smallcompany was posed with the sameproblem, he did not get intocomplications of X-rays, etc.,but instead came out with anothersolution. He bought a strongindustrial electric fan and pointed itat the assembly line. He switched thefan on, and as each soap box passedthe fan, it simply blew the emptyboxes out of the line.

Moral Always look for simple solutions.Devise the simplest possiblesolution that solves the problems.

Computers Repair Themselves with an Autoimmune System

Siemens is developing a computer system that can automatically immunize itself and correcterrors, the research magazine Pictures of the Future reports. This system, which does not interferewith computer users, will make it possible for computers to recognize problems and externalattacks by themselves, and then take corrective measures within seconds. The completeautoimmune system, which will be used for the first time at the end of this year, is beingdeveloped by Siemens Business Services (SBS) and Siemens IT Services consists of four components, three of which are already on the market. The Corporate Error Analysis (CEA) system provides a complete overview of theavailability and security of an IT system. This Microsoft-based software collects information from individual subscribers, andsuggests solutions for eliminating problems. The system works on the principle of the probability of repetition, as chancesare that a disturbance caused by a program or virus has occurred somewhere else, and has already been successfully dealtwith. All such incidents are therefore archived, so if a problem occurs, the Service Engine searches all the databases for asuitable solution and then applies it in the form of updates for example.

Integrated Motor for Ironing Clothes with Power Steam

Siemens has introduced an iron with an integrated steam generator. The appliance offers all the advantagesof a larger and more expensive steam ironing station with a separate water tank. The innovation in the“slider steam generator,” as the system is known, is a motor inside the water tank that operates a pump. The five-watt motor, which isn't much larger than a postage stamp, moves a small piston up and down toproduce up to 40 grams of steam per minute. Even as the water level goes down, the power steam doesn’ttaper off like it does in most conventional steam irons. A single push of a but-ton on the handle is enoughto re-lease a constant jet of steam from the holes on the sole of the iron. Even up-right steaming of delicatefabrics is possible without their coming into contact with the sole of the iron. The new iron can also beordered with an automatic cut-off feature. If it sits in the tray or on its base for more than eight minutes, the heat auto-matically shuts off. And if a mother leaves the iron on its sole on the ironing board to attendto a crying baby, for instance, it shuts off after 30 seconds.

A Motion Detector That Also Has “Eyes”

A new motion detector from Siemens can not only sense the body heat of suspicious individuals itcan also see them. The “Eyetec” system is equipped with an infrared detector that’s complementedby an optical sensor capable of recognizing motion patterns up to 15 meters away. Sophisticatedalgorithms are used to evaluate these patterns and determine their relative plausibility. If an alarmis triggered, the detector archives the images so that they can later be used to trace the cause ofan incident. At the International Fire and Security Exhibition and Conference (IFSEC) in Birmingham,Eyetec re-cently was honored with the Security Industry Award. Developed by Siemens BuildingTechnologies, Eyetec is the world’s only dual-principle motion detector that combines infrared andoptical sensor technology. Because of this combination, the system almost eliminates the risk offalse alarms. And while many conventional motion detectors only notice infrared radia-tion, suchas that caused by body heat, the Optical Detection System (ODS) installed in Eyetec employs a CMOS sensor and fuzzy logicto also determine an object’s size, speed and direction of travel. The detector notices if anyone attempts to block or coverthe sensor monitoring the room. Thanks to its anti-blocking function, the detector immediately triggers the alarm if its fieldof vision is blocked in any way.

New Traffic Management System can break up traffic jams.

A new, flexible traffic-computer sys-tem from Siemens is making it easier for communities to controltraffic and avoid congestion. The system, Sitraf-fic Scala, is making it possible for the first time toimprove traffic management in small towns. Until now, traffic guidance systems were either notdesigned for small road networks or were too expensive, but this new traffic computer system ismodular and can be individually configured to fit any road network. It monitors all traffic signals ina given road network and can provide traffic guidance at any time. It detects traffic jams withintelligent sensors and can help to break them up by activating traffic lights in a targeted manner. The key to this new advance is a detection device called the Traffic Eye Universal, a solar-powered device that is suspendedover a road-way and automatically detects the traffic situation via infrared sensors. Using a GPRS data transfer system, itimmediately sends traffic reports to the central computer, which means the control system is able to react faster. If theTraffic Eye detects that traffic is becoming congested, the central computer immediately adjusts the relevant traffic lights’switching times to the traffic situation and temporarily gives the green light to backed-up traffic.

Smoking is not only a sinister threat to ourhealth but it is also very detrimental tonational economy. Take for example theeffects of smoking in a well developednation like US. According to US Govt. datapublished in 2005 smoking cost the nationabout $92 billion in the form of lostproductivity in 1997-2001, up about $10billion from the annual productivity lossesfor the years 1995-1999. The new lostproductivity estimate when combined withsmoking-related health-care costs, whichwas reported at $75.5 billion in 1998,exceeds $167 billion per year in the UnitedStates. The report also finds that during 1997-

2001 an estimated 438,000 prematuredeaths occurred each year as a result ofsmoking and exposure to secondhandsmoke.

The constituents of tobacco smokeTobacco smoke is a complex mixture ofover 4 thousand chemical compounds –some in tiny quantities – that are theproduct of burning the ingredients of thetobacco and paper, many of which arepharmacologically active, toxic, mutagenicand cancer producing. The following majorcomponents of tobacco smoke have beenidentified as most likely to cause disease:l Nicotinel Tarl Carbon monoxide (CO)l Hydrogen cyanide and other toxic

agentsl Metals:

Thirty metals have been detected intobacco smoke, including nickel, arsenic, cadmium, chromium and lead. Arsenic andsome chromium are causally associatedwith cancer in humans, while nickel andcadmium are probably carcinogenic tohumans.

Risks of SmokingSmoking can cause at least 8 – 10 differenttypes of cancers including lung, mouth, food pipe, kidney, pancreas and bladder. It is important to note that it can causecancers of even those parts of body thatare not directly exposed to smoke, likepancreas and bladder.

Overall, smokers live 12 years less thannonsmokers and die an early death as aresult of their habit. As compared to thosewho have never smoked (or have quitdecades ago), the average smoker is:l Fourteen times as likely to die of lung

cancerl Fourteen times as likely to die of mouth

or throat cancerl Four times as likely to die of cancer of

the esophagusl Twice as likely to die of a heart attackl Twice as like to die of bladder cancer

Risks of Second-hand Smoke orPassive SmokingIt has been proven by studies that second-hand smoke is as bad as smoking andentails the same risks and damages forhealth. This is particularly bad news forinfants and children of smokers who areexposed to such grievous risks early on intheir lives when their body defenses arenot fully developed.

Risks of Low-Tar CigarettesLow-tar cigarettes confer little to noadvantages over regular cigarettes. Whileunfiltered cigarettes with the highest tarlevels were linked to a somewhat greaterrisk of lung cancer, neither low- nor ultra-low- brands were associated withdecreased cancer incidence. In fact, some studies have found thatsmokers who switch to "light" cigarettes

are at higher risk of lung cancer than thosewho smoke regular cigarettes. This isbecause even if they do not smoke morepacks, smokers compensate by inhalingfiltered or lower-tar cigarettes:l More Often andl More Deeply, plusl Wait Longer before Exhaling, in an effortto obtain an adequate amount of smoketo satisfy their nicotine craving.A major drawback of light cigarettes is thereassurance they bring. This can makesmokers less inclined to quit who mightotherwise be more concerned about theconsequences of smoking on their health.

Risks of CigarsBeyond merely their larger size (they caneasily contain as much tobacco as an entirepack of cigarettes), cigars produce morecarcinogens than cigarettes. Due to thecuring process, the tobacco in cigarscontains elevated concentrations of andnitrates and nitrites. In addition, its tobaccoleaf wrapper causes a cigar to burn lesscompletely than a cigarette. This resultsin production of larger amounts of carbonmonoxide, tar, and other harmfulchemicals.

l In spite of increased carcinogen levelsin cigar smoke, lung cancer is not ascommon in cigar smokers as it is in cigarettesmokers, simply because cigar smokers areless likely to inhale deeply.l Notwithstanding, a cigar smoker stillhas a several-fold higher risk of lung cancercompared to a non-smoker. Likewise, cigarsmoking significantly increases the risk ofcancers of the mouth, esophagus, larynx, bladder, and pancreas.

Advice From TheMedical Centre

Overall, smokers live12 years less thannonsmokers and die anearly death as a result oftheir habit. As comparedto those who have neversmoked (or have quitdecades ago)

Research has estimatedthat more than 90% ofsuccessful quitters do sounassisted, and thatt h o s e w h o q u i tunassisted are twice aslikely to succeed as thosewho attend a quitprogram.

The Bathtub TestIt doesn't hurt to take a look at yourself from time to time and this should help you getstarted.

During a visit to a Mental Asylum a visitor asked the Director what the criteria was whichdefined whether or not a person should be institutionalized?

"Well," the Director said, "we fill up a bathtub with water to the top and then hand thepatient a teaspoon, a teacup, and a bucket and ask them to empty the tub."

"Oh, I understand," said the visitor. "A normal person would use the bucket instead of theteaspoon or the teacup because it is bigger."

"No," said the Director, "a normal person would have pulled the plug out of the tub.""Would you like a bed by the window?"

"DID YOU PASS-OR DO YOU WANT THE BED NEXT TO MINE?"

continuing smokers, and the benefits ofquitting extend to those who quit at olderages. For example, persons who quitsmoking before age 50 have one-half therisk of dying in the next 15 years comparedwith continuing smokers. Smokingcessation decreases the risk of lung cancer, other cancers, heart attack, stroke, andchronic lung disease. Women who stopsmoking before pregnancy or during thefirst 3 to 4 months of pregnancy reducetheir risk of having a low birth-weight babyto that of women who never smoked. Thehealth benefits of smoking cessation farexceed any risks from the average 2.3 kg(5 pound) weight gain or any adversepsychological effects that may followquitting.

Smoking Cessation

Quitting smoking is not easy, but it ispossible; every year thousands of peoplequit successfully. The vast majority of smokers quit withoutexternal assistance. Research has estimatedthat more than 90% of successful quittersdo so unassisted, and that those who quitunassisted are twice as likely to succeed asthose who attend a quit program. Stoppingsmoking completely appears to be a moresuccessful strategy than attempting to quitby reducing consumption gradually orswitching to lower tar cigarettes. Indicatorsfor success with quitting smoking are:l being highly motivated and expectingto succeed,l having a supportive environment, andl having the skills to cope with adaptationand change.

Some smokers try several times beforesuccessful quitting. This does not have tobe viewed negatively: they may learn from

relapse and be successful nexttime.

Tips for Smoking CessationThe following are suggestedsteps:l Develop strong personal

reasons for quitt ing.Consider, for example:

l Health risks of smoking foryourself as well as your familymembersInconvenience of buying, keeping and then finding proper placefor smoking

l Bad smell of cigarette in your mouthand your room

l Expense of buying that could go to someother more useful purpose

l Expense on sickness arising out ofsmoking like chest and heart diseases.

l Make a firm decision to quit.l Set a target date for quitting.l Inform family and friends of the target

date, and ask for their support.l Every night before going to bed,

reinforce your commitment to quitting.l Physically condition oneself for quitting:l Begin an program.l Drink Plenty of Fluids.l Get Plenty of .l Avoid Fatigue.

What are some ways to cut back oncigarettes?

l Some techniques for tapering off inanticipation of the quit date are:

l Switch to a distasteful brand.l Switch to a Low-Nicotine, Low-Tar brand

of cigarette. When using this technique,it is important NOT to:l Smoke more cigarettes.l Inhale more often.

l Inhale more deeply.l Place fingertips over the holes in thefilters.

l Smoke only half of each cigarette.l Each day, postpone having the first

cigarette.l Smoke only during odd or even hours

of the day.l Set a cigarette limit, at the beginning of

the day.l Alter eating habits to discourage

smoking, for example:l Instead of smoking acigarette, drink a glass ofjuice or eat a piece of fruit.l Smoke only when oneREALLY wants to. Stopwhen one finds him/herselflighting a cigarette out ofhabit only. To help b eaware of every cigarettesmoked: l Do notempty ashtrays.l Use the opposite hand to smoke.l Put cigarettes in a differentlocation/pocket.l Look in the mirror every time onelights a cigarette.

M a k e s m o k i n ginconvenient by:l Buying cigarettes by the pack,not by the cartonl Waiting until the pack is empty

before buying anotherl Not carrying cigarettes aroundl Making access to cigarettes

difficult by hiding them inwardrobes or putting them atplaces difficult to access.
