We called Ubers for those whose cars were buried, mapped out alternate public transportation routes and swapped shifts based on ability to travel. We were one of the few Northwell ambulatory net- works to remain open on Sunday. It is an honor to be part of a team that has such a commitment to our communities, to our patients, and to each other, no matter the conditions. Bravo! KORN’S KORNERE. coli: The Science Behind the Headlines Robert Korn, MD, MBA, FACEP, Medical Director New York E. coli is a bacteria that lives in the in-testines of all humans, and in many ani-mals, including cows. Normally, we live with them in harmony: they help digest our food and we give them a nice, warm, moist environment to thrive.

Over the last 20,000 years, however, multiple strains of E. coli have evolved. Some of these strains have picked up "extra" genetic material called plasmids, which often carry genes that give a bac-teria extra survival advantage over its neighbors.

E. coli O157:H57 is one such strain; it produces a toxin known as Shiga tox-in, one of the most potent toxins in nature. It is similar in action to ricin, a poison that has gained fame in the world of espionage as a weapon of assassination. Other strains of E. coli have also developed toxin-producing abilities, often with even more dangerous potential than Shiga toxin. De-pending on the strain, there may be mild illness, severe bloody diarrhea, renal failure from something called HUS (hemolytic uremic syndrome), and even a high fatality rate.

Cow products are the main source of E. coli disease; it is not known why cows are colonized with this bacteria, nor why they don't themselves get sick. However, multiple outbreaks of toxin-producing E. coli have been linked to other foods as well; the list is frighteningly long: alfalfa, clover and radish sprouts, lettuce and spinach, unpasteurized juices, yogurt, dried sa-lami, mayonnaise, raw milk, game meats, hazelnuts, and raw cookie dough. Outbreaks usually occur related to a single strain, can have unique features, and be non- O157:H57. For instance, a German outbreak related to contam-inated seeds of fenugreek from Egypt, sold as sprouts by an organic farm in Germany, sickened 4,000 people, caused 50 deaths, and multiple cases of HUS, with a predilection for women.

Treatment is supportive; antibiotics have no place in the care of these pa-tients, which is true for MOST infectious diarrhea (giardiasis and c-diff are exceptions). The main thing to know about E. coli disease is that preven-tion is vital. Test all red meat WITH A THERMOMETER. 165 °F is absolutely required to kill this bug. If you want to swear off red meat completely, you will probably live longer anyway. Don't allow surfaces or other foods to be contaminated with raw meat, and handle all diarrhea (as when changing a diaper of a sick child or adult) with gloves and careful hand washing after-wards.

In the clinics, remember that these pathogenic E. coli can be identified on culture; it is worth sending one off for patients who do not appear to have a routine viral enteritis, which is of course the most common cause of in-fectious diarrhea.

We will end 2015 with having seen just over 50,000 patients. More impressive is the commitment to our clinical quality and custom-er service level. We have maintained an average NPS score of 87 for December.

2016 will be a big year for us. We have three new cen-ters coming online Q1 in our new Westchester Market, and we look forward to expanded growth within our current markets.

NW MARKET UPDATE NPS SCORE: 82 | RESPONSE RATE: 35%Greg Carroll, MD President, Northwest MarketIn 2015, we consummated our partnership with Leg-acy and, thanks to your efforts, we opened five new clinics in the Northwest Market. Your incredible com-mitment to our patients is reflected in an average NPS score of 82 for December.

In 2016, we will be opening at least 10 new centers.

I am inspired by your enthusiasm in supporting the rollout of our new queuing system. This will help our patients more easily check in and find the ideal cen-ter for their visit. There will be more exciting oppor-tunities to follow, such as our expansion into Wash-ington state.

After graduating from med-ical school, Andy decided on a different approach to healthcare by pursuing a ca-reer in healthcare manage-ment and administration. He served as the operations manager in a consulting firm managing international renewable energy projects worldwide. After return-ing to the healthcare environment, Andy served as the Practice Administrator for an ambulatory sur-gical center managing two facilities. Andy is now proud to be a part of GoHealth Urgent Care where he can positively impact the healthcare world.
