The approval issued for the subject equipment videf this office letters of even No. dated16/08/2001 is hereby extended for a further period of five years upto 15/08/2011. The conditionsspecified in this office letter of even no. dated 16/08/98 should be otherwise strictly followed.

The field performance report from actual user/your customers of the subject equipment mayplease be collected and furnished to this office for verification & record at regular intervals.

This approval is granted subject to observance of the fonowing conditions :-

1) The equipmentis covered under a validBIS licence.2) The design and construction of the equipment shall be strictly in accordance with

the description. condition and drawings as mentioned in the CMRI Test Reportsreferred to abovt..

3) The equipment shall be used only with approved type of accessories andassociated apparatus.

4) Each equipment shall be marked either by raised lettering cast integrally or byplate attachedto the main structure to indicate conspicuously :-

c) Name of the manufacturer.b) Name and number by which the equipmentis identified.c) Xumber & date of the test report of the CMRl applicableto the equipment.d) Number and date of this letter by which use of the apparatus is approved.

5) A certificate to the effect that the equipment has been manufactured strictly inaccordance with the drawing referred to in the Q.1Rl test report and is identicalwith the one tested and certified at CMRI, Dhanbad shall be fwnished with eachequipment.

6) The customer shall be supplied with a copy of this letter, an extract of theconditions and maintenance schedule, if any, recommended by CMRl in their testreports and copy of instructions booklet detailing operation & maintenance of theequipment so as to maintainits intrinsic safety characteristics.

Thi<;approval also covers the pennissible variation as approved under theCMRI test reports referred to above. This approval may be deemed to have beenrevoked with immediate effect at any time, if anyone of the conditions subject towhich approval has been granted is violated or not complied with. The approval mayalso be amended or withdrawn at any time, if considered necessary in the interest ofsafety.

The field petfonnance report from actual users/your customers of the subjectequipment may please be collected and fwnished to this office for verification &record at regular intervals.

This approval is otherwise valid for a period of five years trom the date of1SSue.
