The following private Health Care Organization & Diagnostic Centre have beenassessedas Service Provider by the Health & Family Welfare Department on the basis ofthe facilities available there for rendering service to the Government employees/Government pensioners and their beneficiaries under the West Bengal Health Scheme,2008. The Health Care Organisation & Diagnostic Centre have come into agreement withthe Government of West Bengal on the basis of Memorandum of Agreement for a periodof 2 (two) years or till they are modified or revoked whichever is earlier from the date ofthe notification.

Now, the Governor is pleased to recognize the following private Health CareOrganisation & Diagnostic Centre for the purpose of availing benefits of medicalattendance and treatment under the West Bengal Health Scheme 2008 and enlist them inAnnexure- A of Finance Department Notification No. 3473-F dated ri" May, 2009.

Empanelment period of the above Health Care Organisation expired on 14.06.2014.

However, its renewal process could not be completed till now due to procedural delay.

Accordingly, the Governor is pleased to extend the empanelment period of the

above Health Care Organisation up to 13.09.2014 from the date of expiry (i.e"

14.06.2014) on existing terms and conditions so that the beneficiaries under the West Bengal

Health Scheme may get services there uninterruptedly. In the meantime, the Health Care

Organisations shall have to complete the process of renewal of empanelment furnishing

up-to-date Clinical Establishment License, up-to-date Fire Safety License, Memorandum

of Agreement (MOA), Performance of Bank Guarantee and other requisite papers if not

already furnished.

This issues in cancellation of this Deptt. Notification No.3363-F(MED) Dated.
