It is important what you tell us is kept
private. We will ask you first if we need to
speak to other people about your care.
When to contact the team
Monday to Thursday, 9am to 5.30pm and
Friday 9am to 4.30pm

About Greenwich Community Learning
Disability Team or CLDT
Greenwich CLDT help people with
learning disabilities to keep well. They
also help people to learn to do things for
themselves as much as they can.
How to get help
You, your family or carer can ask for help.
You will need to tell us how we can help
you. You can talk to us or fill in a form.
The team can work with you to:
tell people what you want
eat, drink and swallow
make changes in your life
manage your behaviour
manage your feelings and
mental health
look after yourself and stay safe
stay well and healthy
manage your money
move around
do as much for yourself as possible
choose housing and where you live
find out about work and leisure
learn new skills
decide what support you need.
In the team there are:
speech and language therapists
social workers
nurses
physiotherapists
psychiatrists
occupational therapists
transition workers
psychologiststeam support.
We will arrange to meet you to askyou what help you need. We may visityou at home, your day centre, collegeor at work.
We will agree a plan. This is
sometimes called a support plan or
careplan. We offer help to people in
different ways and to plan for the
future. We also run some clinics and
groups.

If you are happy or not happy about
your care please talk to us. You can
speak to a member of staff or the
manager. You can also be helped to
contact PALS (Oxleas’ advice
helpline).You can call them on
