Guidelines to fill the Family Survey Format

Write the names of family members in the same order at the beginning

of each page as mentioned in the first page.

Do not confused and write ones information in others column

Take the information of all the members regarding one particular

content (horizontally)

Gather information of exactly the family members of the family other

than those who are relatives and friends at the time of survey

Fill the information using pen only

If the family is a joint and living in one house kindly furnish the

information of all the family members in one survey formats writing

the relation ship with the head of the family

If the size of the family is more than 7 members you take another

survey format but do not repeat the common information namely

details of dry/wet land/housing family assets /live stock

Write the details in understanding manner and avoid overwriting /

rewriting

Do not put the columns incompletely and put Dash (-) if not relevant

suitable information (Specify other) at the empty blanks
