Minimising the Risk of Falls & Fall-related Injuries Guidelines for Acute, Sub-acute andResidential Care Settings is an initiative of the Victorian Quality Council (VQC). The development of the Guidelines Pack is one component of a strategic approach toreducing the risk of harm and improving health care quality and safety in Victoria, including:Establishing a Safety and Quality Framework, Providing Access to Better Data, Educatingon Safety and Quality and Responding to Known Problems and Risks.

Falls, related injuries and loss of confidence due to fear of falling are common causes ofmorbidity in Australia. In hospital and residential care settings, the risk of falling is evengreater than in the community setting, because of acute illness, increased levels of chronicdisease, and different environments and routines.

Research evidence indicates that interventions to minimise falls risk can reduce the risk of falling and fall-related injuries, even in olderpeople with high risk of falling. Staff involved in direct care in hospital and residential care settings have a key role in successfulimplementation of falls risk minimisation activities.

Purpose of these GuidelinesThe purpose of these Guidelines is to assist directcare staff and others responsible for ensuring qualityof care, to put in place an effective program forminimising the risk of falls and fall-related injuries.The information provided is based on the bestavailable evidence at the time of publication.

Who these Guidelines are intended forThese Guidelines have been developed for thosewho deliver, and are responsible for, patient/residentcare. This includes clinical, management, corporateand environmental serviceÕs staff.

Using these Guidelines in the differentsettingsAlthough a broadly similar approach may be takento minimising the risk of falls and fall-related injuriesin the different settings, circumstances may call fordifferent strategies in:

acute and sub-acute hospital settings, and

hospital and residential care settings.

These Guidelines have been structured as a globalresource for use across all three settings, wheredifferences exist, they are identified and describedseparately.

Use of the terms “patient”, “resident” and “client”For the purpose of this document the term ÒpatientÓrefers to both patients and clients in acute and sub-acute settings. ÒResidentÓ has been used torefer to people receiving care in residential caresettings.

Using these Guidelines in emergency departmentsForty-five percent of older people who attend a hospital emergency department after a fall are dischargedwithout admission. Emergency department staff have an important role in identifying the ongoing fall risks forthese patient/residents, as well as initiating appropriate referrals or interventions that may reduce the risk offuture falls and hospital presentations.

These Guidelines may provide a useful framework for staff in emergency departments.

Research findings and levels of evidenceThese Guidelines are based on research evidence and, where no formal research evidence exists, on expertopinion and the findings of expert working parties.

Guideline Statements have been identified for the four patient/resident-centred care steps in the processmodel.

The research methodology is described in the Research Supplement.

Levels of evidence of effectivenessThe evidence for the guideline statements presented in these guidelines was systematically assessed andclassified according to the National Health and Medical Research CouncilÕs (NHMRC) Guide to theDevelopment, Implementation and Evaluation of Clinical Practice Guidelines [1].

Levels of evidence of effectiveness describe the strength of the research evidence supporting eachrecommended strategy to reduce the risk of falls or fall-related injuries. From strongest to weakest, the levelsof evidence used for the Guideline Statements in this document are shown in the following table:

Definition of a “fall”For the purposes of these Guidelines, a fall isdefined as:

ÒA sudden, unintentional change in position causingan individual to land at a lower level, on an object,the floor, the ground or other surface [2].Ó

This includes:

slips being lowered

trips loss of balance

falling into other people legs giving way.

If a patient/resident is found on the floor, it should beassumed that they have fallen unless they arecognitively unimpaired and indicate that they putthemselves there on purpose.

It is recommended that a common definition isadopted for use across health services and within aspecific setting.

Falls risk factorsFalls risk factors are characteristics or behavioursthat make it more likely that a patient/resident willfall. Falls risk factors can broadly be considered as:

Personal (intrinsic) risk factors relate to healthproblems that increase the patient/residentÕs risk of falling.

Environmental (extrinsic) risk factors relate tohazards in the environment that increase the patient/residentÕs risk of falling (eg bed brakes not locked).These risk factors have been sub-divided into:

Individual environmental risk factors — relatedto hazards in the patient/residentÕs immediatearea

General environmental risk factors — related tohazards that are outside the patient/residentÕsimmediate area, but in places where thepatient/resident goes at times (eg corridors, therapy areas).

The interaction between the individual and theirenvironment can be considered a third type of riskfactor — also called behavioural risk factors. Thenature of the activities performed (how difficult ortiring they are) and the way patient/residents performthem (safely or not) will influence their risk of falling.

The four patient/resident centred care steps

The four steps are integral to an effective program to minimise the risk of falls and fall-related injuries inhospitals and residential care settings.

The four steps are:

The term ÔToolsÕ refers to a range of support documentation and resources used in implementingprograms to minimise the risk of falls and fall-related injuries. These include:

Screening may not be required in settings wheremost patient/residents are considered to be atrisk of falling (eg dementia specific wards inresidential care). In these settings, and for allpatient/residents in high falls risk populations,proceed directly to Step 2.

b) Conduct a falls risk screen at the following times:

on admission, or as soon as practicable afteradmission

when a patient/resident is transferred from oneward or department to another

if there is a change in a patient/residentÕs healthor functional status

if a patient/resident has a fall, and

as part of discharge planning.

c) Use a recognised tool.

d) Document the results and include in thepatient/residentÕs permanent file/medicalrecord/residential care file.

e) Carry out a falls risk assessment (Step 2) for:

patients/residents whose risk of falling exceedsthe threshold

patients/residents who are in high falls riskpopulations* (eg patient/residents withneurological dysfunction)

OR

refer the patient/resident for assessment as soonas practicable.

High falls risk populationsStudies in hospital and residential care settingshave identified high falls risk populations to includepatient/residents with the following:

a history of falls, falls with injury, or fall relatedfracture neurological conditions such as stroke andParkinsonÕs disease cognitive problems such as dementia or deliriumdepressionlower limb (leg) arthritisacute infections such as urinary tract infection,chest infectionshaematological/oncology conditions, orvisual impairment.

Definition Falls risk screening refers to a brief (often less than 5 items) check to identify patient/residents who are at riskof falling. These patient/residents need to have a more detailed assessment of their falls risk carried out.

* In some settings, the Screening component may be omitted, and the Model commences with Assessment (Step 2)

Rationale for Step 1The risk of falling and related injury increaseswith age [4] and increased unsteadiness [5].

Falls occur more commonly in the periodimmediately following a transition betweensettings [6].

Increased length of stay in a hospital settingincreases falls risk [7].

Falls risk is ongoing. The patient/resident shouldbe reviewed as part of an ongoing process [8].

Differences in implementing Step 1across the three settingsAcute Care

In an acute setting, the patientÕs health status islikely to change rapidly, and falls risk screeningshould be repeated on a regular basis (eg dailyor weekly) or when clinical judgement indicatesthat there is a change in the patientÕshealth/functional status.

In emergency departments, falls risk screeningcould be used to determine whether falls riskassessment should be completed by hospitalstaff, and/or whether referrals should be made tocommunity agencies.

Residential CareFalls risk screening may be less relevant in manyhigh care residential settings, because mostresidents are likely to exceed the falls riskthreshold. In this situation a falls risk assessmentshould be completed on all residents (Step 2).

Step 2: Conduct falls risk assessment

GUIDELINE STATEMENT FOR STEP 2

Conduct a falls risk assessment for each patient/resident who is identified asbeing at risk of falling.

Level of evidence: II [9, 10, 11].

Definition Falls risk assessment is a systematic and comprehensive process to identify an individual patient/residentÕsrisk factors for falling.

A patient/residentÕs level of risk of falling and fall-related injury is a combination of:

the severity of each risk factor (which can vary from nil to very high)

the importance of that risk factor (some risk factors have a stronger association with the risk of falls andfall-related injuries than others), and

the number of risk factors.

There is a direct relationship between a patient/residentÕs level of falls risk and their probability for falls.

Patient/resident centred tasks for Step 2a) Assess the following patient/residents for falls risk:

patient/residents whose level of falls riskexceeded the screening threshold (Step 1)

all patient/residents in high falls risk populations;and

all patient/residents in settings where mostpatient/residents are expected to have a highfalls risk (eg dementia specific residential care units).

b) Assess or reassess a patient/residentÕs falls riskat the following times:

as soon as practicable after admission

if there is evidence of change in thepatient/residentÕs health/functional status

when the patient/residentÕs environment ischanged (eg a patient/resident moves to anotherroom, ward or setting, or new equipment is used)

when the patient/residentÕs treatment is changed(eg different drug(s) prescribed)

when the patient/resident has a fall

when a patient/resident is discharged/transferredfrom one setting to another, either within thesame organisation or to a different organisation

as part of a routine review, and

at other times as required by your organisationÕspolicy.

c) Use a recognised tool to carry out theassessment.

d) Identify and describe all factors that contribute tothe patient/residentÕs risk of falling and fall-related injuries.

These include:

the patient/residentÕs personal risk factors, and

risk factors in the patient/residentÕs individualenvironment.

e) Record the level of risk and the identified riskfactors (both personal and individualenvironmental) in the patient/residentÕs permanentfile/medical record/residential care file.

f) Proceed to Step 3 to determine actions toaddress the risk factors you have identified. 11

Rationale for Step 2Falls are usually caused by a number of riskfactors, [12] which vary between individuals, andchange over time [7].

Falls occur more commonly in the periodimmediately following a transition betweensettings [6, 8].

The risk of falling and related injury increaseswith age [4], and increased unsteadiness [5].

Increased length of stay in a hospital settingincreases falls risk [7].

Falls risk is ongoing. The patient/resident shouldbe reviewed as part of an ongoing process [8].

Risk factor status changes over time, so theremay be a gap between needs and services. This gap may be indicated by a change in thepatient/residentÕs health or a fall [13].

Between 10% and 50% of falls in hospitals andresidential care facilities involve an environmentalhazard [5, 7, 14].

Over half of the falls in hospitals and residentialcare facilities occur around the bedside [7, 10].

Differences in implementing Step 2across the three settingsAcute Care

In an acute setting, a patient/residentÕs healthstatus and treatment may change frequently.Because of this re-assessment may need to becarried out as often as staff shift changes, thetool used should be relatively short and quick tocomplete.

Although allied health staffing levels may be morelimited in acute hospital settings than sub-acutesettings, a multidisciplinary approach to falls andfall-related injury risk minimisation is still possibleand desirable.

Sub-acute and Residential Care A team may carry out components offalls risk assessments rather than onestaff member. For example, the PJC-FRAT is a multidisciplinary tool used inthe sub-acute setting, where:

a medical officer assesses fall history,medical condition and medications

a nurse assesses continence status

a physiotherapist assesses transfers andmobility status, and

an occupational therapist assesses functionalactivities (bathing and dressing).

If a multidisciplinary team is involved in thefalls risk assessment, responsibility forensuring timely completion of the assessmentshould be given to one staff member.

Direct care staff can assist by gatheringinformation for the assessment.

Personal risk factors identified in the literatureA number of falls risk factors relating to apatient/residentÕs health status andcharacteristics have been identified in theliterature. These are known as personal riskfactors (refer to the Research Supplement formore detail about personal risk factors).

There may be a number of possible causes for eachrisk factor, and an important element of the falls riskassessment process for a patient/resident is toidentify the cause/s of the risk factors identified(which may often require medical and other healthprofessional assessment).

Some causes may be modifiable (where the medicalcondition that is the cause of the risk factor is ableto be improved with treatment), while others may benon-modifiable (often associated with chronic ordegenerative health problems). Even for non-modifiable causes of falls risk factors, treatment canhelp to reduce the magnitude of the effect of the riskfactor (for example, for a patient/resident withParkinsonÕs disease, exercise can help to improvesteadiness during walking even though the exercisedoes not affect the underlying disease). Actions tominimise risk of fall-related injury should also beimplemented for patient/residents with non-modifiable causes of personal falls risk factors.

Definition An Action List for minimising the risk of falls and fall-related injuries is a list of strategies to address riskfactors that were identified by the falls risk assessment (Step 2).

Rationale for Step 3A targeted falls prevention program based on thefindings of a falls risk assessment has beenshown to reduce falls in residential care andhospital settings [9, 10, 11].

Modification of the individualÕs environment canreduce falls risk [15].

Falls are usually caused by more than one fallsrisk factor, so multi-factorial interventionstargeting all identified risk factors are likely to bemore effective than single interventions [16].

Falls risk factors may present differently inindividual patient/residents. These differencescan be determined through individual assessmentand management of each of the risk factors [13].

Differences in implementing Step 3across the three settingsAcute Care

Because the time spent by patients in this settingis usually short, the approach may emphasisepreventative actions such as enhancedsupervision of patientsÕ activities and environment.

Sub-acute and Residential Care For patient/residents receiving longer-term care,there may be greater emphasis on behaviourmodification and improving functions for daily living.

Table 3 outlines actions to minimise the impact of each of the personalrisk factors for falls, to assist in developing an Action List. The researchevidence clearly supports a multifaceted approach, where multipleactions are introduced to address the full range of personal risk factorsidentified.

Many of the actions listed in Table 3 have beenidentified in the research literature, and furtherdetails supporting these actions are provided in theResearch Supplement.

In addition, a number of actions, which should beconsidered part of standard care for allpatient/residents, can reduce the risk of falls andfall-related injuries. These include:

identifying causes of the risk factor (medical orallied health referral)

ensuring safety (adequate supervision,communicate mobility status to all staff, use ofappropriate walking aid)

ensuring safe and uncluttered environment

keeping call bell and other personal items withinreach

orientating patient/resident to area, and

identifying and addressing individualpatient/residentÕs needs.

This Table allows easy identification of actions to address individual environmental risk factors forfalls. A multifaceted approach, where actions are introduced to address each of the identified individualenvironmental falls risk factors, is likely to be most effective.

An Individual Environmental Audit should be carried out to identify prevailing risk factors for individualpatient/residents. Examples of Audit Tools are provided in the Tools Supplement.

Definition The appropriate response to a falls incident includes caring for the patient/resident and ensuring that theincident is reported and documented.

Use the standard definition of a fall developed by your organisation to determine whether an incident is to beregarded as a fall. An example definition is included in the Introduction section of this document.

Other definitions are provided in the Research Supplement.

Patient/resident centred tasks for Step 4a) At a minimum, an appropriate post falls response

Organisational tasks for Step 4a) Develop a protocol for care of patients/residents

after a fall has occurred. Ensure staff are awareof and understand the protocol.

b) Develop a stand-alone Falls IncidentReport template or add a falls sectionto your organisationÕs existing incidentreport. See Tools Supplement forsamples.

c) Provide staff education on the purpose ofincident reporting and training in the use of theincident report.

d) Audit the use of the incident report forcompliance with relevant policy.

e) Assign the analysis of falls incident data to aclinical staff member on a ward or to adesignated person in a facility, such as a ClinicalRisk Manager. An example Excel file template forentering data from falls incident forms,calculating rates of falls/1000 beddays, and examples of graphs forreporting falls data is included in theTools Supplement.

f) Ensure incident data is analysed regularly todefine the scope, common causes andcomplications arising from falls.

g) Provide clinically meaningful feedback on fallsand fall-related injury data to wards or units, on aregular and scheduled basis.

h) Ensure data is analysed at ward-level. Findingsfrom periodic reviews can help inform strategiesto minimise risk and reduce the rate of falls. Ahigh level of falls may indicate the need for rootcause analysis.

Note:

An incident report should include sufficient detailabout the fall, circumstances surrounding the fall andconsequences of the fall to provide a clearunderstanding of factors contributing to the falland/or injury if sustained. As a minimum, thefollowing information should be recorded in a FallsIncident Report or other standard documentation:

Patient/resident identification — minimum data set

Time of fall

Whether the fall was witnessed

Circumstances of the fall:location

activity being performed by the patient/resident atthe time of the incident, eg transfer details

relevant information about clothing, footwear,glasses and gait aids used at the time of the fall

environmental conditions, eg floor, lighting,clutter

Type of fall, eg slip, bumping into/falling on an object

Whether the patient/resident was injured and, if so,the nature and severity of the fall-related injury

The patient/residentÕs perception of the incident,including description of any preceding sensations orsymptoms

Action taken following the fall and or fall-relatedinjury

Signature of the person who observed and/orreported the fall

Designated process for:submitting the report

entering the report into the patient/residentÕsdocumentation.

If a death results from a fall in a Victorianhospital an investigation by the StateCoroner will be performed. An InvestigationStandard published by the Victorian StateCoronerÕs Office outlines the details required for thisinvestigation (see Tools Supplement).

These details outline the main features required bythe CoronerÕs Investigation Standard. In addition, the CoronerÕs Investigations Standard will review theorganisations policies on falls risk screening, fallsprevention and falls management.

An organisationÕs falls rate may increase in the shortterm when its management:

introduces falls and fall-related injury riskminimisation activities or programs

introduces a new definition of a fall, or increasesawareness of the definition, or of reportingrequirements, or

implements these Guidelines and process model.

Rationale for Step 4Some falls can cause injuries that may not beapparent at the time of the fall. All falls should bereported, even if injuries are not apparent, andpatient/residents should be observed after thefall. This is particularly important in cases wherethere is a possible head injury [19].

Information about factors that contribute to fallson a ward or unit can be used to inform targetedactivities to reduce future falls [20].

Using a standard definition of a ÔfallÕ [1, 21] enables:

shared understanding of the focus anddimensions of what is described (what isincluded and what is not), and

the grouping of falls incident data forcomparison and trend analysis.

Good quality falls data and meaningful analysiscan provide evidence to guide responses withinthe organisation and elsewhere [20]. Data hasimplications for:

developing management strategies thatbenefit the patient/resident and theorganisation, and

meeting legal, regulatory and insurancerequirements about recording adverseevents.
