    PREAMBLE

    WE, the people of God in the Immaculate Conception Parish,

    ECOG!I"I!G our role, under the authority of our Parish Priest, to assist in matters of pastoral care of 

    the Parish and to help foster pastoral action# A!D, $ein% %uided $y the &oly Spirit in spreadin% the Word

    of God to ena$le the development of stron% ecclesiastical communities founded on Christian values and

    faith in God, centered on the Eucharist, and in servin% as 'itnesses of (esus in &is mission of salvation

    and li$eration, DO E!DEA)O to promote the *in%dom of God in our Parish 'ith the %uidance of our 

     $eloved +ady of Peace and Good )oya%e, - &EE- ADOPI!G this Constitution and y/+a's.

    ARTICLE I – NAME, DOMICILE, AND PRIMARY !NCTION

    • "ection #$ he name of the or%ani0ation shall $e the Immaculate Conception Parish 1 Parish

    Pastoral Council 2ICP/PPC3, Antipolo City.

    • "ection %$ Its domicile shall $e at P. Oliveros Street, Antipolo City.

    • "ection &$ he ICP/PPC shall $e the consultative $ody of the Parish in matters of plannin%,

    coordination, and implementation of the Parish4s holistic pastoral care.

    ARTICLE II – OB'ECTI(E")!NCTION"

    • he main o$5ective of the ICP/PPC is to 'or6 'ith the Parish in $rin%in% Christ to society and to

    assist the Church in servin% as the Conscience of society to'ards social transformation into the

    *in%dom of God.

    • he ICP/PPC shall assist the Parish Administrator in the plannin%, coordinatin% and implementin%

    of Parish activities throu%h 'hich all the faithful may e7ercise their duty and ri%ht to colla$orate

    actively in $uildin% up the 8ystical ody of Christ. It shall $e composed of the Parish

    Administrator or his appointed representative and mem$ers of the Parish.

    he ICP/PPC functions are9

    • "ection #$ o serve as a vehicle throu%h 'hich all the faithful of the Parish may $e involved in and

    contri$ute to Church affairs of the Parish. As such the Parish community 'ould not only $e an

    evan%eli0ed community $ut also an evan%eli0in% community.

    • "ection %$  o serve as a coordinatin% and unifyin% $ody for all Parish

    8inistries:Or%ani0ations:8ovement:Communities and their activities. It shall oversee all activities

    and pro%rams in as much as they operate 'ithin the Parish to ensure they are consistent 'ith theCatholic ;aith and shall encoura%e pro%rams that promote unity and spiritual development of 

     parishioners for the common %ood of the Parish.

    • "ection &$ o assess, deli$erate and develop proposals:recommendations on apostolate activities

    and pro%rams, and issues relevant to Parish concerns.

    • "ection *$ o formulate an inte%rated pastoral plan.

    • "ection +$ o promote an active and participative community of Christ4s faithful sharin% in the

     priestly, prophetic and 6in%ly functions of the 8other Church.

    • "ection $ o develop and nurture families practicin% Christian values and livin% their faith in God

    to'ards $uildin% a community of disciples centered in (esus, follo'in% &im in &is mission of 

    salvation and li$eration.

    • "ection -$ o participate in the Church in servin% human6ind and $rin%in% all people of God.

    • "ection .$ o plan and or%ani0e the electoral process for 8PPC Officers and 8PPC 8inistry

    hi%her ecclesiastical authorities.

    ARTICLE III – "TR!CT!RE AND !NCTION" O T1E ICP2PPC

    ARTICLE I( – MINI"TRIE"6 7eneral unctions an4 Responsi8ilities

    • "ection #$ Rationale 9or the Ministries6 An underlyin% o$5ective of the 8inistries is to provide

     parishioners throu%h mem$ership and active participation in respective or%ani0ations:movements,

    the opportunity to share in Christ4s 8ission, to %ro' spiritually and to collectively share Christ

    'ith others. he 8inistries shall encoura%e and implement pro%rams that promote unity and

    spiritual development of parishioners for the common %ood of the Parish.

    a. In line 'ith the common mission of Christ4s faithful to participate in the priestly, prophetic, and

    6in%ly functions of the &oly 8other Church, the ICP/PPC shall have under it the different

    ministries WES-);+A, the various parochial communities, 8PPCs and ECs.

     $. he responsi$ilities and scope of functions of the 8inistries shall $e those related to their 

    respective areas of concern in 'hich their primary roles are9i. esearch

    ii. Consultationiii. Plannin%iv. ecommendation

    to the ICP/PPC and shall serve as advisory staff and central clearin% office of plans, pro%rams,

    activities, and policies of the ICP/PPC.

    c. Each 8inistry shall $e composed of a coordinator, assistant coordinator, secretary, treasurer, and

    auditor, the coordinator of 'hich shall $e the presidin% officer in their meetin%s and shall $e the

    representative to the PPC.

    d. Each 8inistry shall $e assisted $y or%ani0ations:movement cate%ori0ed as primary and au7iliary

    mem$ers of the respective 8inistry 'hich shall meet re%ularly to help thresh out pro$lems,

    deli$erate on su$5ects concernin% the parish and parishioners and ma6e proposals to the council for 

    approval of the Parish Administrator. Activities of or%ani0ations:movements are not limited to the

    activities of the 8inistry 'here they are assi%ned.

    e. he 8inistries may hold dialo%ues 'ith the people on matters affectin% the 'elfare of the

    • +et the Chairman and Spiritual Director of the 8inistry chec6 the prepared case study. If 

    they si%ned it, then let the Parish Administrator chec6 the case study for approval or 

     prepare a certification letter that the patient is a parishioner of the parish, and let the Parish

    Priest si%n the certification that 'ill $e send to the Diocesan Social Action Center alon%

    'ith the Case Study for ;inancial Assistance.

    RE!E"TIN7 O OICE "!PPLIE", MEDICAL "!PPLIE" AND OT1ER 

    MATERIAL")E!IPMENT

    • Prepare a re=uest directed to the Personnel/in/Char%e usin% a re=uisition form.

    • If the items that 'ill $e re=uested 'ill $e too much 2financially3, prepare a letter of re=uest

    addressed to the Parish Administrator and Personnel/in/Char%e.

    • +et the Spiritual Director and Chairman of the 8inistry on Social and &uman

    Development si%n the letter.

    MEDICAL AND DENTAL "ER(ICE"

    he schedules of the doctors are listed $elo'.

    MEDICAL

    • When assistin% the doctor, provide him a copy of our prescription pad. If there are patients

    that are $ein% chec6ed/up, record some of his:her information provided on the +o% oo6 of 

    8edical Chec6/up.

    •When the doctor prescri$e medicines, chec6 the prescription and %ive him:her themedicines if it is availa$le in the office. ecord the medicines that are $ein% %iven in the

    8edicines ecord oo6.

    OPT1A$ "ER(ICE"

    he schedule of the volunteer ophthalmolo%ists is every Sunday around 99 am

    • ecord the patients name, address, contact no. and if he:she is a mem$er of 

    Philhealth.

    • Provide a prescription pad for the ophthalmolo%ists.

    • If the medicines that are prescri$ed are availa$le in the office, provide the patient

    the medicines and record in the 8edicine ecord oo6.• If the patient has a cataract, provide him:her the contact of the ophthalmolo%ist so

    that he:she 'ill $e helped the doctor. sually, if the patient has a Philhealth, the

    cataract operation may proceed for P?,. instead of a P@,. cataract

    operation. If he:she is not a mem$er of Philhealth, the doctor may also help the

     patient $y see6in% help from PCSO.

    • Provide a snac6:$rea6fast for the ophthalmolo%ists.

    • DONT OR7ET TO T1AN? T1E OPT1ALMOLO7I"T" OR 1I")1ER 

    "ER(ICE"$

    DR$ MAL(AR he Schedule of Dr. 8alvar is every the >st Saturday of the month.

    o Prepare a 'or6place 5ust outside the office. A ta$le, the record of the patients 2inde7

    card3, a 'ei%hin% scale for $a$ies and adults and a $lood pressure apparatus.

    o Chec6 if the patient has already a record, if not, prepare a record usin% an inde7

    card indicatin% his:her name, address, a%e, %ender, the date of chec6/up, $lood

     pressure and 'ei%ht. If the patient is suspected to have a fever, chec6 his:her 

    temperature.

    o +et the patients stay at the 8ater Dei &all.

    o Sort the patients accordin% to cate%ories, if it is a$out medical, pedia or for readin%

    %lasses.

    o +et Dr. 8alvar decide 'hich cate%ory of the patients that he 'ill chec6/up first.

    o Assist Dr. 8alvar durin% the chec6/up. Provide him a prescription pad, $allpen and

    a stethoscope if he doesn4t $rin% one.

    o Chec6 the prescription of the patient and %ive them the medicines if it is availa$le

    in the office. ecord the medicines that are $ein% %iven in the 8edicines ecord

    oo6.o After the chec6/up had $een done, ma6e sure the 'or6place 28ater Dei &all, inside

    and outside of the office are in order3.

    o As6 Dr. 8alvar to have a lunch inside the refectory. DONT OR7ET TO

    DENTAL "ER(ICE"

    he schedule of Dental Services depends on the dentists $ut usually falls on hursday or ;riday.

    • his service is a first come, first serve $asis and usually caters > patients only.

    • Confirm if the doctor 'ill $e comin% to conduct the free dental service.
