Warm greetings on behalf of IMA Team-Erode

Last month concluded with the three 3 days UNICEF program for

Paramedics. We also had an excellent CME by our oncologist team on cancer

blue print and radiotherapy treatment aspects. Special thanks to our yoga and

reflexology team for there active role in our regular CME meetings. EIMA once

again congratulates the sports team for their victory at Inter Zonal sports meet .

This forthcoming month our State IMA Team & EIMA plans to felicitate

Dr. C.N.Raja, Dr. B.Sridhar and Dr. Kanagasabapathy for their success. This

program would be followed by our regular CME meeting .

We are also planning this month for a Trauma care awareness program

and a rally as part of our IMA TNSB Trauma care Project.

Medical Profession at Crossroads

Dr. K.M. Abul Hasan

Vivekananda Sami said to the Youth of India Arise, awake and donot stop until the

goal is reached with the dream of developing a Strong Nation. We witness similar kind of calls

from IMA HQS to its lakh & lakhs of members to safeguard the medical profession in our

country. It is sad and disheartening to see that we repeatedly fail in our efforts to have a safe

environment for practicing medicine.

Inspite of all adversaries happening our health parameters have improved and it is

the golden era of modern medicine. Govt&Media don't want to give credit to the medical

fraternitybut to the health policies – Nevertheless everyonewould agree that the medical

profession going more& more patient centric.

Only Doctor Friendly Acts and Rules and Judgements can be Patient friendly. A

Doctor should work in an environment which is friendly, relaxed, confidence building and

free of violence to deliver his best to the patient, which is ensured even in the theatre of war.

Now incidence of violence against Doctors and clinical establishments is the order of the day

and in-spite of IMA's demand for a Central Act against violence, it remains unaddressed by

the Government.

The small and medium level hospitals which cater to 60% of the population are

facing closure due to financial non-viability and Government's lopsided Health Policy.

Government should promote the segment of health care providers by a policy of AIDED

HOSPITALS. These small institutions run by doctors are situated in the neighborhood and

are present in villages and remote areas. They are friendly and easily accessible 24 x 7 at an

affordable cost. The violence and plethora of laws are pushing them to extinction. They are

national assets providing primary and secondary care. They help to keep the healthcare cost

down. They deserve a special status and promotion.

Govt could regulate the medical profession in our country, with concern for both

doctors and patients. Instead CEA, PCPNDT and CPA are crushing the medical fraternity

particularly small nursing homes. PCPNDT Act is a medical solution for a Socio economic

problem, which cannot solve the issue. The government is debating the need to abolish the

Medical Council of India and replace it with a 29-member National Medical Commission. If

NMC is enacted, it will allow non MBBS doctors to practise modern medicine, a step which

may be detrimental to the society. Schedule-IV of NMC which allows lateral entry of other

medical practitioners into the modern medical registry will be detrimental to the health of

the people.

Medical students are against being asked to appear for an exit exam after passing

MBBS. The Indian Medical Association is also not in favour of “NEXT”.

A ray of hope became visible when an inter-ministerial committee was constituted thby the Govt to examine the issuesraised by IMA on 13 November 2015, they are

1. Urgent Amendments in PCPNDT Act to stop criminal prosecution for even clerical errors

2. A central legislation to present assault on doctors

3. Doctor friendly CEA

4. Suitable amendments in the CPA for capping of compensation.

5. Bar Crosspathy

6. No to NEXT exam

Inter-ministerial committee had sent recommendations to the concerned ministries

on the above issue, But it in frustrating that nothing has happened, that is the reason IMA HQ

organized Dilli Chalo & Satyagrah as an outburst of anger & dejection.

But the majority of our colleagues moved little on these Problems, either they do not

realize it is our job to fix this or they just don't bother

Introduction

It is a common scene in every household. The lady of the house inspects her creations in the kitchen by tasting a sample of the food that has been prepared. Often, she takes a finger full of rice and examines to check if it is cooked completely. Why does she do that to those few grains of rice? What is the assurance that every other rice particle in that pot is also equally cooked? Here is where sampling comes to play. It is taken for granted that the sample of rice taken for the evaluation represents the entire pot of rice. This concept is similarly applied in medical and social sciences research.

Assuming for a moment that we want to do as study on hypertension among adults in Erode city, we will first think about taking count of the entire adult population in the city. This number will run into lakhs. Do we have the time, resources and manpower to examine the blood pressure of the entire population? It is absolutely difficult. We are inherently forced to resort to taking a sample of the population, assuming that the results of this sample will be similar to the results of the population.

What is sampling?

Sampling is a process by which some persons/ objects/ elements/ events are selected from the predetermined population for carrying out studies and drawing inferences about the population as a whole.

The concept of sampling rests on abiding with the following principles –

1. Representativeness

2. Randomness

3. Adequacy

Representativeness

When we choose a sample of the population, we do it with a main focus of extrapolating the results to the entire population. In other words, for example, if the results show that the prevalence of hypertension in a sample of population from Erode city is 12%, then the prevalence of hypertension for the entire Erode city will be 12%. This could vary plus or minus the value. However, how much variation is acceptable is what is determined by the sample size, which will be discussed further below. The ideal method of creating a representative sample is by having background knowledge of the disease and drafting a well planned inclusion and exclusion criteria. It is more important to exclude

people who may not represent the actual population. For example, in a study on myocardial infarction, we may prefer to exclude children and adolescents.

Randomness

Bias is an error which occurs inherently due to any defect in the selection of the sample. When we select a sample from the population, we should take adequate care to ensure that the sample matches with the population. If that is not the case, then the results may not depict the true picture of the problem. In other words, it is essential to ensure that every potential member of the population gets an equal chance of being the sample.

There are several techniques which help in ensuring a uniform selection process. These sampling techniques work on the principle of randomness. Random selection of the participants in a study eliminates bias. In other words, a person is selected in to the sample by chance and not by choice.

Adequacy

As mentioned earlier, the results of the sample should be extrapolated to the population. In the example of hypertension, when the prevalence in the sample is 12%, the researcher implies that if he takes another sample from the same population, the prevalence will also be 12%. This being the case, it is important for the researcher to examine the precision of the results.

The precision is indirectly reflected in a concept called 95% confidence limits. In other words, if a researcher repeats the experiment in 100 different samples of the same population, 95% of the times the prevalence will range from 'a' to 'b', where 'a' and 'b' are the lower and upper limits of the range. Therefore, more narrow the range is, between 'a' and 'b', more precise will be the results. Indirectly, to get a narrow range, we may have to take adequate number of the sample.

When we select a representative sample, it is important for us to know how many numbers are required, in order to ensure precise results. Depending on the study designs used, there are formulas for calculating this sample size.

Conclusion

Any medical research is done with a focus of creating evidences by putting forward the facts and figures. For this purpose, we need to have a rigorous methodology, which focuses on the sampling techniques and drawing inferences from an adequate sample of the population. This will help deriving conclusions for the entire population, thereby transforming evidences into practice guidelines.
