Who can apply?
• Have a constitution or other relevant governance document
• Have a management committee/board of directors/trustees of at least three unrelated people
• Have a bank account under the name of the group with at least two unrelated signatories
• Are based in or supporting people living in the Leeds Metropolitan District Area
• Unincorporated groups (up to £10,000 income) can apply for up to £5,000
• Preference would continue to be given to small organisations based in Leeds with an income of less than £100,000 per year

Criteria
The Small Grants programme is looking to support projects:
• Which will improve the health and wellbeing of people in communities
• Building on local assets to address locally identified needs and inequalities, including those emerging from Covid-19
• That empower and amplify the least heard voices
• Put people and communities at the heart of approaches with co-produced local solutions
• Align with the Leeds System Health and Care priorities and link with their Local Care Partnership
• The minimum request is £500. The maximum is £10,000 for a two year project (up to £5,000 a year)

Decision Making Process
• LCF recruit an independent panel for each fund, liaising with the fund-holders and other relevant partners. We work to ensure that our decision panels are fair and diverse.
• The panel’s role is to bring an understanding of local and strategic issues. Final decisions are agreed by a majority vote.
• The majority of our funds do tend to be oversubscribed. Where possible we try to ensure that constructive feedback is recorded and given to all applicants.

Reporting and Evaluation
• Complete an end of grant report showing what you have achieved, what you have learned, what the health outcomes have been and how you know this.
• Complete a brief interim report by mid November 2021. The learning and evidence gathered will feed into the evaluation report of the wider HPoC programme due around this time.
• If you apply for 2 year funding - a short year 1 interim report (which will include budget expenditure details) will be needed to enable the second grant allocation/year 2 to be released.
• Think about how learning and health outcomes will be evidenced and demonstrated.
• Celebration event in October for successful grantees
