Clinical InnovationsPolicy on Resolving Intractable Conflict Over Possibly Inappropriate or Harmful Life-Sustaining Treatment, Massachusetts General Hospital. In 1999, I adapted an earlier policy at Boston Children’s Hospital for use at general hospitals. The policy establishes a “due process” approach to resolving the most intractable conflicts over life-sustaining treatment. Palliative Sedation Guidelines, Massachusetts General Hospital. I wrote these guidelines in 1999 based on other guidelines in existence at the time. They provide medical anethical guidance to clinicians and make it possible for clinicians to assure dying patients that they do not need to suffer at the end of life. New Patient-Centered Code Status Order Format, Massachusetts General Hospital. In 2001, I developed and oversaw implementation of a new code status order format for the MGH computerized patient order entry system. The new format was designed to better convey the patient’s valuesgoals, and preferences to the code team and to make optimal use of life-sustaining treatments. Comprehensive HIV/AIDS Care in Vietnam. From 2002 – 2006, I directed a project to assist the Ministry of Health of Vietnam and the National Instituteof Infectious and Tropical Disease of Vietnam to introduce and scale-up comprehensive HIV/AIDS care and

I founded this project with no funding in 1999 to train Vietnamese physicians in HIV/AIDS and to provide technical assistance to the new US CDC Vietnam Office. From 2002, when I was awarded a $3 million cooperative agreement by the CDC Global AIDS Program, through 2006, I directed a greatly expanded project to train physicians and nurses throughout Vietnam in comprehensive HIV/AIDS care and treatment. Under my administrative leadership, VCHAP grew to a staff of nine in Vietnam and Boston and directly or indirectly trained most Vietnamese physicians working in HIV/AIDS care.

2005-present Director of International Programs, Harvard Medical School Center for Palliative Care This program provides training and technical assistance in palliative care to developing countries. Our main focus has been Vietnam where we have assisted the Ministry of Health with all aspects of integrating sustainable palliative care into its healthcare system including policy development, essential drug availability, training, and implementation of clinical services.

Narrative Report (limit to 500 words)

My academic and research interests include medical and ethical issues in palliative care and in caring for medically underserved populations, continuing medical education in Vietnam, and philosophy of medicine. Since my fellowship I have been studying and publishing on the special problems of minority patients facing life-threatening illness. I now am working on ethical and social justice implications of efforts to provide palliative care to the poor. I also am leading a collaborative study at Massachusetts General Hospital (MGH) of the effects on patient outcomes of hospital policies, developed by a committee that I chair, to promote optimum use of life-sustaining treatments. My ongoing work to develop, evaluate, and publish these policies was funded by a Faculty Scholar grant from the Soros Foundation’s Project on Death in America. As Director and later Senior Consultant in Palliative Care of the Vietnam-CDC-Harvard Medical School AIDS Partnership, I began ongoing studies to assess the effectiveness of our train-the-trainers program in Vietnam. My book The Disposition of the Subject: Reading Adorno’s Dialectic of Technology appeared in the Northwestern University Press series Studies in Phenomenology and Existential Philosophy. My teaching has been in the areas of palliative care, medical ethics, social medicine, and inpatient internal medicine. Within the Department of Global Health & Social Medicine, I directed the Social Medicine Commons Forum, a noontime series addressing critical issues in social medicine and medical ethics. I was Course Director of “Pain and Palliative Medicine: From Basic Science to Clinical and Ethical Concerns,” one of the courses in the HMS Advanced Biomedical Sciences curriculum. In 2002, I won a five-year, $3 million cooperative agreement from the CDC Global AIDS

Program to fund the Vietnam-CDC-Harvard Medical School AIDS Partnership (VCHAP). Under my direction through 2006, VCHAP directly trained over 700 Vietnamese physicians and 45 nursing leaders in HIV/AIDS care. Hundreds more have been trained since then by our first trainees and by the curriculum developed under my supervision. Also during my tenure as Director, VCHAP held Vietnam’s first national training courses in HIV nursing and pediatric HIV. I have mentored 15 US medical students doing HIV/AIDS-related service-learning projects in Vietnam over the past several years. In 2006, I stepped down as VCHAP Director in order to focus on promoting responsible palliative care in Vietnam and other resource-limited settings. Since then, I have helped Vietnam’s Ministry of Health to write national Guidelines on Palliative Care for Cancer and AIDS Patients, overseen development of a comprehensive curriculum in palliative medicine, and led Vietnam’s first training courses in palliative medicine for over 400 Vietnamese clinicians. I now am working with the Ministry of Health to launch a Fellowship and Certification Program in Palliative Medicine and an official national curriculum in palliative medicine for distribution to each of Vietnam’s 10 medical schools. At Harvard Medical School (HMS), I serve as Director of International Programs and Core Faculty member of the HMS Center for Palliative Care. In 2002, I was chosen as a Fellow of the Academy at Harvard Medical School. At Massachusetts General Hospital, I have been a faculty member of the Cross-Cultural Curriculum initiative that provides cultural sensitivity training to the house staff. My clinical contributions are inseparable from my teaching. At Massachusetts General Hospital, I served as Fellowship Program Director of the Palliative Care Service from it inception in 1998 to 2002. During this time, we trained our first five fellows, and all are now faculty members at US medical schools teaching and practicing palliative medicine. I continue to serve several months each year as attending physician on the Palliative Care Service. From 1998 to 2003, I served one or two months each year as attending physician on the Bigelow Medical Service where I supervised residents and medical students. I also served multiple times as attending physician on the Medical Consult Service where I supervised senior medical residents. When in Vietnam and other developing countries, I provide training and clinical mentoring in palliative care for local colleagues.
