The benefits of selecting a Steward Preferred Skilled Nursing Facility include:• Seamless care: We work hard to make sure you

receive highly coordinated care and transition planning from a Steward Hospital to a Skilled Nursing Facility and from a Skilled Nursing Facility to your home.

• Additional clinical support: A Steward nurse care manager will monitor your care during your stay.

• Timely information: We make sure there is ongoing communication between you, your providers and your family to help you make decisions through each stage of care.

• Important follow-up care: We will coordinate a follow up visit with your primary care physician or specialist after discharge from the Skilled Nursing Facility to reduce the likelihood of further complications and avoid any unnecessary hospital or emergency department visits.
