I am pleased to present the annual edition of the Diplomatic Club Medical Directory as a service to the diplomatic corp.

The aim of the bulletin is to assist diplomats and their families and staff who often come to Israel without knowledge of Hebrew, and perhaps without foreign health insurance that would cover them adequately in the event of emergencies or health problem.

We are greatly indebted to the local doctors and medical centers that have agreed to be listed in the Diplomatic Club Medical Bulletin, and for their agreement to accept the international health insurance scheme outlined.

Robotically-assisted surgery with the da Vinci Surgical System is changing the surgical world in much the same way as laparoscopy did twenty years ago. Nowadays, many procedures such as appendectomy, hernia repair, cholecystectomy and some lung surgeries are performed with this minimally invasive technology. The system enables surgeons to experience the advantages of open surgery – 3-dimensional views of organs with those of laparoscopic surgery - smaller surgical incisions, less blood loss, less trauma and faster recovery periods. In the operating room, the surgeon sits at the console away from the patient viewing a highly magnified 3-dimensional image of the body's interior. To operate, the surgeon uses master controls with his hands and feet to manipulate four interactive robotic arms, one of which holds a camera. According to Prof. Jack Baniel, Prof. Jacob Ramon, Dr. Andrey Nado, Dr. Evgeny Solomonov and Dr. Jacob Korach – five leading surgeons at Assuta who recently started using this innovative system – after using robotically-assisted surgery, it would be very difficult to go back to previous surgical methods. Dr. Andrei Nado, head of Urological Oncology at Rabin Medical Center and one of the most experienced renal laparoscopy surgeons in Israel said that the tissue and organ viewing capabilities of robotic surgery are extremely better than those of laparoscopic surgery. Moreover, the instruments are very thin and precise enabling the surgeon to approach regions in the body's interior that previously were difficult to reach. Prof. Jacob Ramon, director of the Division of Urosurgery at Sheba Medical Center, explained that the four interactive robotic arms allow the surgeon a greater range of movement. In addition, the robotic arm holding the camera is much more stable than that of the assistant surgeon who usually holds the camera during laparoscopy. The robot also has better technological solutions for stopping bleeding during and after prostate surgeries.

Prof. Jack Baniel, professor of Urology and Chief of Urology Section at Rabin Medical Center agrees that this is the modality of the future. From a medical point of view, robotically-assisted surgery implies less blood loss, faster recovery, easier movement and less pain for the patient. The duration of the procedures is shorter compared to open surgery or laparoscopy. According to Dr. Evgeny Solomonov, director of the Hepato-Pancreato-Biliary Surgery Unit at Rabin Medical Center, robotically-assisted surgery is very intuitive. The robot has great advantage in every parameter over regular surgery. The robotic arms enable surgical instrument movement of 360 degrees inside the body. He explained that robotically-assisted liver surgery decrease the rate of infections, the patient does not have to receive large quantities of liquids and there is reduced risk of edema and blood loss.Jacob Korach, deputy director of the Gynecologic Oncology Department at Sheba Medical Center, added that the clear advantages of the robotic technique are less blood loss, fast functional recovery and earlier release of the patient. He explained that before implementation of this this technology, most complex surgeries such as radical hysterectomy entailed a large abdominal incision and long recovery; today 80% of hysterectomies can be performed using robotically-assisted surgery. He adds that he would perform almost all surgeries with this technique if the system were available to him on a daily basis. Indeed, its high cost currently stands in the way of turning robotically-assisted surgery into a widely-available procedure. In the USA and in Italy, most prostate surgeries are performed using this technology. In Israel, however, robotic prostate surgery is currently only funded by Maccabi's supplementary healthcare services as well as by some of the private health insurance companies.

THE TIVONI CLINICIzhak Tivoni opened his practice over two
