A health risk assessment (HRA) is a health questionnaire, used to provide individuals with an evaluation of their health risk and quality of life.

HEALTH PROFILE
Health Risk Assessment (HRA) Module• The Meliora HRA is designed for adult
populations aged 18 to 65 years. The HRA
uses evidence-based guidelines and is
written at the eighth-grade level.
• A pediatric version for 6- to 17-year-olds is
also available.
• It is Health Information Products (HIP 1)
Certified by the National Committee for
Quality Assurance (NCQA) and complies with
the Genetic Information Nondiscrimination
Act (GINA).
Controlled Availability & IncentivesProgram settings determine when the HRA can
be completed and the incentive points that are
awarded for one or more of the following:
• Completion of the questionnaire
• Biometric results meeting client-defined
Health Risk Assessment ModuleOur profile is a health risk assessment for adult populations aged 18 to 65 years. The HRA uses evidence-

Long & Short FormThe Meliora HRA is available in long and short form.
Clients can deliver both long and short forms to the
same population and can switch between versions
from year to year. Aggregate reports combine data
from both versions.
• The long form assesses current health
(including lifestyle behaviors, biometrics,
health status, and compliance with
recommended preventive health
screenings), chronic conditions, and future
disease risk.
• The short form delivers all the functionality
required to assess current health, including
biometrics, but omits chronic and future
disease risk assessments
Online Assessment (PC or Tablet)The online assessment uses conditional branching
logic to personalize and streamline data collection.
Data is saved automatically, and the participant
may stop any time and return to complete
the questionnaire at a later date. The online
questionnaire can be switched between English
and Spanish at any time.
When the assessment is completed, the responses
are locked before the personalized report is
displayed, preventing data manipulation. Optional
review mode delays display of the report until
biometric data has been entered or loaded from
file.
The Meliora HRA can be completed at a health fair
or biometric screening event using your tablet.
Scannable Bubble FormsThe assessment is also available for paper-and-
pencil completion using bubble forms, in English
or Spanish, that support standard scanning
technologies. Scanned data from paper forms
is loaded into our database and merged with
data collected through the online questionnaire,
providing a single database for reporting.
Biometric Data Four options are available for entry of laboratory
results and vital signs:
• Self-report by the participant using ranges
or numerical values.
• Entry by health coaches using our lab tool
option.
• Entry by screener using our lab tool option.

• Batch load from file. The load process
supports completion of the HRA
questionnaire before biometric data
is loaded and, conversely, load of the
biometric data before the HRA assessment
is started.
Participant Reports When the assessment is locked, participants receive
a personalized report, available in English and
Spanish. The report details health risks and lifestyle
behaviors, together with recommended actions for
health improvement. Summary results from the
previous two HRAs are displayed for comparison
with current risks.
A printable provider summary report is also
available, in English only.
Administrator Reports Management reports track HRA participation rates
and biometric screening results. Aggregate reports
provide an analysis of health risks, readiness-
to-change, and compliance with recommended
screenings.
• Program reports profile the population for
a selected service level and/or employer
group or subgroup.
• Trend reports show progress over time for
the population (all participants in each time
period) and the cohort (participants in both
time periods only).
Reports can be run by service level, employer group
or subgroup, and date range.
Batch Printing & Export Batch-printing functions support fulfillment of
member and provider reports. Batch-export
functions allow access to all questionnaire and
assessment data for custom reporting and data
sharing with partners.
Third-Party HRAs Organizations currently using a health risk
assessment may be reluctant to switch to a new
HRA vendor because of the valuable data pool they
have accumulated over time. The third-party HRA
data-load process allows an organization to take
advantage of the advanced data-management
facilities provided by the platform, while retaining
the value of historical health risk assessment data.
Third-party HRA data loaded into the platform is
merged with current HRA data collected using our
profile to provide:

• Coach access to longitudinal HRA data for
individual participants.
• Administrator access to consolidated
aggregate HRA data for outcome reporting
• Participant access to previous and current
HRA results within the member report
summary table for progress tracking.
Personal Health Record ModuleOur personal record is an integrated health
record that provides a longitudinal repository of
participant health data from multiple sources.
Data Integration Selected data elements collected using other
our platform modules, such as the health risk
assessment, are copied and stored in the health
record. They can be supplemented by data entered
directly into the health record by the participant or
health coach.
Data from third-party sources, such as laboratory
results and prescription refills, can be loaded into
the health record using standard batch processes.
Self-reported and coach-entered data uses SNOMED
and NDC codes to provide data portability; loaded
data retains the coding used at source.
Components Components of the health record include:
• Personal information including demo-
graphics, lifestyle, family history, and ad-
vance directives.
• Comprehensive medication record,
including Rx and OTC therapies.
Prescription refills can be loaded from PBM
data, if available.
• Medical history comprising conditions,
treatments, allergies, immunizations,
screenings, and office visits.
• Biometric results batch loaded directly
from source data, if available, and used to
populate the profile health risk assessment.
• Contact information for provider, pharmacy,
hospital, insurance, and an emergency
• Printer-friendly health record reports in a
variety of formats, including PDF and XML,
to allow portability and data sharing.
