What is integration?
At its heart, health and social care integration isabout ensuring that those who use services getthe right care and support whatever their needs,at any point in their care journey…
and it’s also about helping to build stronger, healthier communities.
The Public Bodies (Joint Working) Scotland Act 2014Key elements of the Act…• Partnerships will be measured against 9 high
level national health and wellbeing outcomes.
• Partnership will have a Partnership Board and a Chief Officer.
• The Local Authority and Health Board will make payment to a joint budget overseen by a Integration Joint Board.
The Public Bodies (Joint Working) Scotland Act 2014Key elements of the Act• Partnerships are required to undertake
locality planning and segment their boundaries into at least 2 geographical areas.
• Partnerships have submitted an Integration Scheme to the Scottish Government by the deadline of 1 April 2015 and are now working on their Strategic Plans.
