1. Logon to www.healthscopebenefits.com,.

2. Click on “Member”, Enter your Group Name “Bard”.

3. Click on “Member Self Service”.

4. Click on “Eligibility Status.

5. Enter your User ID and Password.

6. From the Quick Links menu click on “Electronic

Transaction Setup”.

7. Under “Modify E-Mail Address”,

Fill in the e-mail address that Electronic EOB notifications are to be sent.

8. Under “Add an EFT Bank Account”, Mark disabled.

DO NOT enter any banking information. Click “Next Page” when complete.

9. Review the summary information. If everything is

correct, submit change request. A prompt will ask you if you are sure, click yes. The e-mail address entered will

receive a notification when an EEOB is available to

review on the online claims and customer care portal.
