CCG Group Select Committee Enquiry● Recommendations on whether it was in the patient’s interest to

share information about them with other medical professionals

Escalation of concerns to CQC● Concerns over a specialist care provider led to us

recommending further investigation to Healthwatch England and the CQC

Adult Social Care Review● Report based on public consultation including targeted questionnaires and discussion events● 6 core recommendations

Renal Patient Transport● Working with Healthwatch Derbyshire to examine patient

experience of renal transport and a report is being taken to their CCG.

Working with Sheffield Teaching Hospitals on Discharge Information ● redesign of patient information with a view to making a reduction in further hospital admissions
