Observation Forms This file gives you a different way to keep observation notes for your in home child care, center or preschool.

Supplies needed: • White card stock• Post it notes (3” x 3”)

Assorted covers• Laminating sheets• Clip board or pocket

Directions: • Type each child’s name in a rectangle. Print up

on white card stock.• Laminate sheets.• Peel off the back of each post it note and place

on a square. (Do this to each square). Print up asmany sheets as you need depending on howmany children you have you are observing.

You have 2 options when storing your observation sheets. To keep them handy and available for writing notes. 1). Place sheets onto a clipboard and have a pen attached. You can do you a couple sets of this template and place them in different areas of your playroom / school area.

Write down observation notes for each child on the post it notes under each child’s name. (Date each observation). Pull post it note off the pad, lift up the laminated observation sheet and store the observation note on the clipboard until you do the observation entries. 1 – 2 – 3 Learn Curriculum

2). Purchase a sturdy pocket folder. Print observation sheets onto white card stock and laminate. Staple each observation sheet to the inside of the pocket folder. (Top only of each observation sheet). Attach post it notes to each square.
