HOMEOWNERS HOME SECURITY DISCOUNTS

A 10% PROPERTY PREMIUM DISCOUNT WILL BE APPLIED TO THE DWELLING AND RELATED COVERAGES IF ALL OF THE FOLLOWING REQUIREMENTS ARE MET:

1. Telephone line or radio transmitter central station connected security/fireprotection system with 24-hour monitoring.

2. Electronic smoke detectors connected to central station monitor.3. Electronic heat detectors connected to central station monitor.4. Motion detector(s) with interior/exterior alarm and connected to central station

monitor. (Optional due to animals.)5. Carbon monoxide detector.6. Door contacts with interior/exterior alarm and central station monitor.7. Fire extinguisher(s) (2 ½ pound minimum).8. Battery back-up for interior/exterior alarm and digital dialer.9. Certificate from the monitoring company identifying services provided.

10. Discount applied at renewal.

HOMEOWNERS AUTO/HOME DISCOUNTS

AUTO/HOME PACKAGE DISCOUNT APPLIES TO THE FOLLOWING POLICIES: • Homeowners (12% Dwelling and 10% PL premiums) • Renters Insurance with Personal Property contents value of $10,000 or more

(12% Property and 10% PL premiums) • Mobile HomeGuard with minimum value of $15,000 (12% Property and 10%

PL premiums)

GUIDELINES FOR AUTO/HOME PACKAGE DISCOUNT: 1. The named insured on the GMRC personal auto policy must be the named

insured or additional named insured on the FarMutual policy which covers the principle residence of the insured.

2. Current auto dec page must accompany application for discount. 3. Discount will be added or deleted only at renewal following the initial issuance

of the policy. 4. The Property 12% discount will not apply to any policy if a Loss Control

Report is issued or an endorsement that limits coverage is applicable the first year. Once the Loss Control Report is satisfactorily completed and the endorsement(s) that limits coverage is removed from the policy, the discount will be added at the next renewal.

AUTO/HOME DWELLING DISCOUNT APPLIES TO THE FOLLOWING POLICIES: • Homeowner Dwelling with replacement cost coverage (5% Dwelling premium)

GUIDELINES FOR AUTO/HOME DWELLING DISCOUNT: 1. The personal auto policy of the named insured must be written with a carrier

represented by the agency of record. 2. Current auto dec page must accompany application for discount. 3. Current auto dec page must be provided prior to each renewal. 4. Discount will be added or deleted only at renewal following the initial issuance

of the policy.

LONGEVITY DISCOUNTS

HOMEOWNERS LONGEVITY DISCOUNT APPLIES TO THE FOLLOWING FARMUTUAL POLICIES/PERCENTAGE OF DISCOUNT: • Standard Homeowners or Preferred Homeowners insured with Heartland

Mutual for three or more consecutive years (same policy #) - a longevity discount of 5% of total property premium

• Longevity Discount applied independently of Claims Free Longevity Discount

APPLICATION: • Applied at renewal following three consecutive, continuous policy years.

CLAIM FREE DISCOUNTS (HOMEOWNERS)

NEW BUSINESS CLAIM FREE DISCOUNT APPLIES TO THE FOLLOWING FARMUTUAL POLICIES/PERCENTAGE OF DISCOUNT: • Standard Homeowners ($60,000+ Dwelling) or Preferred Homeowners - 6%

of total property premium • No paid property claims in last three years (Determined by CLUE report)

CLAIM FREE LONGEVITY DISCOUNT APPLIES TO THE FOLLOWING FARMUTUAL POLICIES/PERCENTAGE OF DISCOUNT: • Standard Homeowners ($60,000+ Dwelling) or Preferred Homeowners

insured with Heartland Mutual for three or more consecutive years (same policy #) - a claim free longevity discount of 12% of total property premium.

• No paid property claims in last three policy years. (Note: See Applied/Removal)

APPLIED/REMOVAL: 1. Applied at renewal following three claim free policy years: if total property

claims paid (RCC Claims tab) divided by the total premium payment (RCC Billing tab) is equal to or less than 80%.

2. Removed at renewal following a paid property claim within the previous policy year, except when only one claim is paid during the policy period and the amount paid is less than 110% of the policy deductible.

3. The exception applies to only the first claim within three consecutive policy periods. The discount will be removed following the second paid claim of any amount within three consecutive policy periods.

SURCHARGE PROGRAM

Our experience has shown that an exceptionally large proportion of our total claim payments are made to or on behalf of a very small percentage of our policyholders.

A few policyholders experience one or more claims each year. The intent of the surcharge is to request that policyholders with above average claim frequency contribute additional premium, consequently stabilizing the base premium level for those who contribute to good loss experience.

The surcharge is an additional charge imposed on the existing rate structure of the policy.

AFTER CLOSELY REVIEWING EACH POLICYHOLDER EXPERIENCING EXCESSIVE CLAIMS, ONE OF THE FOLLOWING ACTIONS WILL BE TAKEN:

1. Monitor annually 2. Premium surcharge (30% of property premium with a minimum of $50.00)

Only paid claims will be considered valid claims in determining if a surcharge should apply. Reported but not paid claims will not be considered.

The surcharge will be imposed on the first renewal date following the determination that a surcharge will apply.

The policyholder will return to claims-free eligibility on the renewal date following a three-year claims free period.

Additional claims could result in non-renewal of the entire policy.

PREMIUM PAYMENT OPTIONS

The following premium payment options are available for direct-bill policies:

1. Quarterly premium payment plan; 2. Semi-annual premium payment plan; or 3. Annual premium payment plan.

An installment fee (currently $5.00 per installment) applies to the quarterly and semi-annual options; there is no service charge or installment fee applied to the annual payment option.

Payment may be made by cash, check, money order, MasterCard, Visa or ACH (Automated Clearing House). Credit card or ACH payments may be made by phone.

We understand that your insured may occasionally wish to make payment in your office. Any agency accepting premium on behalf of Heartland Mutual with the intent to mail the premium to us is asked to immediately notify us by phone in order to reduce the incidence of unnecessary late notices/fees. Premium received in your office may also be submitted online via agent web inquiry.

Mortgagee-bill policies are billed annually.

GENERAL UNDERWRITING

• Comprehensive Loss Underwriting Exchange (CLUE) reports will be obtained for all applicants.

• Insurance scores will be obtained for all applicants. • Exterior underwriting inspection(s) will be performed on all properties. • Minimum Policy Property Premium is $60.00.

UNACCEPTABLE RISKS

• Wood burner used as primary heat source • 60 AMP electrical service panel • Vicious dogs on premises (including but not limited to Pit Bulls, Doberman

Pinschers, Rottweilers, or any others excluded by GMRC) • Property canceled or nonrenewed by prior insurer for any reason • Vacant property of any kind • Property currently undergoing renovation and is not occupied • Property built up on stilts, poles or pillars (owner occupied or seasonal) • Property that would have any type Commercial exposure not used in the farming

operation such as Churches, Bed & Breakfasts, Thrift Stores and Hunting Lodges • Recreational Vehicles used or modified to be used as dwellings or seasonal dwellings

Provides repair or replacement cost coverage for damage caused by equipment breakdown in your Dwelling or Other Structures. Equipment Breakdown Coverage includes damage to covered property caused by 1) Mechanical or electrical breakdown, 2) Tearing apart, cracking, burning, or bulging of steam or hot water heating system or air conditioning system.

PREFERRED DWELLING UNDERWRITING GUIDELINES

1. Dwelling minimum coverage amount: $125,000

2. Dwelling minimum coinsurance requirement: 100% based on any industry recognized replacement cost estimating system.

3. Application must be completed in its entirety, giving special attention to

dwelling and roof ages, dimensions, fire protection class and type of construction. Detailed information must be supplied regarding prior carrier and loss history.

4. Loss experience:

a. One loss in previous 3 years not to exceed $1,000.

b. If the $1,000 claim amount is exceeded due to one catastrophe

(windstorm/hail) occurrence, the property will be accepted with a complete new dwelling roof and all other damage repaired. No existing damage to the property allowed.

5. Dwelling age not to exceed 35 years.

6. Must be owner occupied.

7. Duplex permitted with owner occupying one side.

8. Modular homes not permitted.

9. $250 minimum new policy deductible.

10. Must be in excellent condition and well maintained.

HOMEOWNERS BUILDER’S RISK POLICY UNDERWRITING GUIDELINES

1. The Preferred Homeowners policy provision will be used.

2. The dwelling must be insured to 100% of its completed value at the inception of the policy.

3. The HG-129 Theft of Construction Material Endorsement will be applied with

a minimum of $5,000 coverage for a period of one year. The rate per $1,000 coverage will be $10.00. Additional coverage may be purchased in excess of $5,000.

SURCHARGE PROGRAM

Our experience has shown that an exceptionally large proportion of our total claim payments are made to or on behalf of a very small percentage of our policyholders.

A few policyholders experience one or more claims each year. The intent of the surcharge is to request that policyholders with above average claim frequency contribute additional premium, consequently stabilizing the base premium level for those who contribute to good loss experience.

The surcharge is an additional charge imposed on the existing rate structure of the policy.

AFTER CLOSELY REVIEWING EACH POLICYHOLDER EXPERIENCING EXCESSIVE CLAIMS, ONE OF THE FOLLOWING ACTIONS WILL BE TAKEN:

1. Monitor annually 2. Premium surcharge (30% of property premium with a minimum of $50.00)

Only paid claims will be considered valid claims in determining if a surcharge should apply. Reported but not paid claims will not be considered.

The surcharge will be imposed on the first renewal date following the determination that a surcharge will apply.

The policyholder will return to claims-free eligibility on the renewal date following a three-year claims free period.

Additional claims could result in non-renewal of the entire policy.

PREMIUM PAYMENT OPTIONS

The following premium payment options are available for direct-bill policies:

1. Quarterly premium payment plan; 2. Semi-annual premium payment plan; or 3. Annual premium payment plan.

An installment fee (currently $5.00 per installment) applies to the quarterly and semi-annual options; there is no service charge or installment fee applied to the annual payment option.

Payment may be made by cash, check, money order, MasterCard, Visa or ACH (Automated Clearing House). Credit card or ACH payments may be made by phone.

We understand that your insured may occasionally wish to make payment in your office. Any agency accepting premium on behalf of Heartland Mutual with the intent to mail the premium to us is asked to immediately notify us by phone in order to reduce the incidence of unnecessary late notices/fees. Premium received in your office may also be submitted online via agent web inquiry.

Mortgagee-bill policies are billed annually.

GENERAL UNDERWRITING

• Comprehensive Loss Underwriting Exchange (CLUE) reports will be obtained for all applicants.

• Insurance scores will be obtained for all applicants. • Exterior underwriting inspection(s) will be performed on all properties.

UNACCEPTABLE RISKS

• Wood burner used as primary heat source • 60 AMP electrical service panel • Vicious dogs on premises (including but not limited to Pit Bulls, Doberman

Pinschers, Rottweilers, or any others excluded by GMRC) • Property canceled or nonrenewed by prior insurer for any reason • Vacant property of any kind • Property currently undergoing renovation and is not occupied • Property built up on stilts, poles or pillars (owner occupied or seasonal) • Property that would have any type Commercial exposure not used in the farming

operation such as Churches, Bed & Breakfasts, Thrift Stores and Hunting Lodges • Recreational Vehicles used or modified to be used as dwellings or seasonal dwellings

COMMANDER DWELLING UNDERWRITING GUIDELINES

1. Dwelling minimum coverage amount: $125,000

2. Dwelling minimum coinsurance requirement: 100% based on any industry recognized replacement cost estimating system.

3. Application must be completed in its entirety, giving special attention to

dwelling and roof ages, dimensions, fire protection class and type of construction. Detailed information must be supplied regarding prior carrier and loss history.

4. Loss experience:

a. One dwelling related loss in previous 3 years not to exceed $1,000 and

one non-dwelling related loss in previous 3 years not to exceed $1,000.

b. If the $1,000 claim amount is exceeded due to one catastrophe (windstorm/hail) occurrence, the property will be accepted with a complete new dwelling roof and all other damage repaired. No existing damage to the property allowed.

5. Dwelling age not to exceed 35 years.

6. Must be owner occupied.

7. Duplex permitted with owner occupying one side.

8. Modular homes not permitted.

9. $250 minimum new policy deductible.

10. Must be in excellent condition and well maintained.
