2.Rig up/down upper sheave wheel at crownGravitational Potential Energy (Stepping, Handling, Lifting)

Lifting of personnel with weight and sheave wheel at high height. Risking falling down of personnel and equipment. Get caught in moving sheave wheel.

- Usage of certified lifting gear and safety harness

- Usage of safety pins securing personnel and equipments shackles.

- Certified air tugger operator

- Avoid sudden jerk or stop while lifting up personnel

- Third watchman to provide signal & instruction

- Radio communication between watchman and up-lifting personnel

- Exclude all personnel working underneath

- Operation to be conducted slowly

- Adequate cables been spooled on rig floor before personnel lifting

- Wearing PPE

3.RIH/ POOH cableGravitational Potential Energy (Stepping, Handling, Lifting)

Personnel are exposed to high movement of cable during operation. Pinch point injuries while latching and unlatching of spearhead and overshot. Cable or weight catches on top drive, thus breaking cable or swivel. Getting hit by moving drill pipe.

- Ensure no-go hardware is used and fishing assembly are check

- Great coordination between rig floor and winch operator

- Mark cable to assist winch operator controlling winch speed

- Wearing PPE

- Establish Intercom system and backup radio communication between driller and winch operator

4.Breaking weakpoint with top driveGravitational Potential Energy (Stepping, Handling, Lifting)

High tension on cable. Potential injuries to personnel

- Increase cable tension gradually

- Make announcement prior to operation

- Exclude all personnel around working area
