I have not been the best at updating this over the years. I work hard and don't do the best job of documenting it via CV. As you can see from my CV if there is a central theme it is hard work in FDA monitored studies. I was the largest implanter in the Artisan United States FDA monitored trial and was the investigator chosen to represent that technology at the FDA hearing. According to Abbott I continue to be their US Verisyse user with the largest experience.I was the largest AcuFocus implanter in the early phases and ended up the 2nd largest implanter in that trial overall and asked to represent this technology at the FDA trial. Phakic IOL's and inlays are areas where I have been a leader in the research arena for the United States. You both know I have also been a leader in femtosecond laser (cornea and lens) research from the beginning. I also was there at the beginning of intraoperative aberrometry. I guess I could go on but I think I will let the rest speak for itself to see if this qualifies for your prestigious club. I would be so honored to be a part of it. Hi Michael,I have known Vance Thompson for over 20 years.He is a real gentleman, a very hard worker, and an innovator.He has been involved both in keratorefractive and implants, and I am sure will be an active interested member of the club.He has been active mostly in the USA, but I believe he is known globally.He has been under published in the peer-reviewed literature, and this may be a concern of other members, limiting his global reach. Significant milestones in my research and technology development career:

1. Being a sub-investigator under my fellowship director Dan Durrie in 1990 and being integrally involved in the partially sighted eye study of PRK (the feasibility trial for the ultimate PRK study in the US) was a significant start to my research career. I worked hard to help this trial be a success. That lead to research opportunity #2.

2. Being asked to be an investigator in my home state of South Dakota in the PRK FDA monitored trials in the United States was a significant milestone in my career. It marked the beginning of my research career here in my home state. This lead to research opportunity #3.

3. I was both a principle investigator or PTK and also the Medical Monitor of the Summit Technology U. S. PTK clinical trial. I was part of the testifying team to help get this technology approved. I was honored to help shepherd it this as the Medical Monitor and it was exciting to lead the first FDA approved ophthalmic

use of an excimer laser in the United States. This lead to research opportunity #4.

4. When I was asked to come to New Orleans for an investigators meeting to learn about Excimer Laser MKM (the technique of taking a 300 micron cap off the cornea and lasering the back and then suturing it back on…in the Summit trials this was performed by Steve Brint, MD and Steve Slade, MD) this was a milestone for me. This was the feasibility trial for laser assisted in-situ keratomileusis (LASIK) and allowed me to be in the first group of 7 to perform hinged flap classic LASIK in the United States. We did not fully realize how much this procedure was going to revolutionize eye care in our world.

5. I was one of three investigators in the beginning of the development of the Hansatome along with Steve Slade and Richard Lindstrom. This project was a Chiron project of which Andy Corley was the lead on. This was a very exciting project for me to be a part of. For example, our first flaps were huge (corneoscleral), often 14 mm and we had to advise and help in the optimization of this exciting technology. It was the beginning of a career influenced heavily by Andy Corley.

6. When I was first asked to be a phakic IOL investigator in the Artisan/Verisyse US trial I declined. It was George Waring, III who contacted me and complimented my hard work in the PRK and LASIK FDA monitored trials and asked that I take a closer look at this procedure. I then traveled to Groningen the Netherlands with a mentor I have looked up to my whole career, Richard Lindstrom, MD; both of us were desiring to learn more from the respected Jan Worst, MD and be potential investigators. This was a very important trip for me. I ended up being the largest enroller of patients in Artisan Intraocular clinical trial in the United States and participated as a presenting researcher in the FDA hearing trials in Washington, DC to get this exciting technology FDA approved.

7. I have been on many Medical Advisory Boards over the years but one of them I was the most proud of was the Intralase MAB. When I took delivery of the 5th Intralase device in the world in the fall of 2001 there was no such thing as pocket software. We

had to develop techniques to deal with the meniscus (applanation) in a way to maximize OBL evacuation to the limbus. It was an honor to be an early user of this game changing technology and an honor to be involved in the beginning group of surgeons to optimize it.

8. I was an investigator in the first modern day corneal inlay study in the United States, the AcuFocus KAMRA device. During every phase of the AcuFocus study I was either the largest, or second largest, enroller. I worked hard to help with the optimization of the corneal inlay techniques and was asked to presenting researcher in the FDA hearings in Washington DC that lead to FDA approval.

9. Being asked to be a lead principle investigator in the Calhoun Light Adjustable Lens clinical trials in our country was one of the most exciting opportunities I have had.

10. Being asked to be a SMILE investigator in our country in combination with having been a PRK and LASIK investigator in their beginning trials put me in the unique position of having been involved in the United States beginning of all three of these important procedures in laser corneal refractive surgery in our country.

11. I was a Medical Monitor and lead investigator of the first FDA approved use of crosslinking in the United States…the Avedro technology. I also co-hold two patents with their technology.

12. I have been integrally involved in the development of the Zepto procedure to perform a capsulotomy with a soft collapsible tip and a circular nitinol cutting element. I lead the first study (results published in Ophthalmology) proving this to be the capsulotomy with the strongest resistance to edge tears compared to manual circular tear and femtosecond capsulotomies.
