Does this already exist in any form? Is this a key area to begin work?Health action plans – there is no LD specific leads to support Anthea Robinson however she is keen to be review updates whilst she is seeking a representative. Annual Health Checks

We need to liaise with children’s services in order to gauge the number of 14 plus year olds going through the transition stage, ensuring that these young people have health action plans and attend annual health checks.We need to bridge the gaps in services, many people with mild learning difficulties are slipping through the net, possibly POhWER could set up a group for people with mild learning difficulties?We are working closely with the surgeries that are reluctant to offer annual health checks and progress is being made.We need to record the successes and failures of implementation of the HAP’s / Annual Health Checks plus also collate the feedback from the regional

Keepsafe:Is a scheme which has been rolled out in many Boroughs throughout the UK. It helps people with a Learning Disability to go about day to day life with the security of having the contact details for someone who can offer assistance if they should come into any difficulties – such as being a victim of a crime, being lost, losing personal items etc.We are looking to implement a scheme like this in Luton, working closely with the police, customers, community safety, advocacy etc.; Val Sheridan is going to head this project over the upcoming months.The Adult Social Care Directory:This can be found on the Councils website under Health & Social Care. We would like to encourage people to view the site and forward any feedback and amendments please.Link to the web page:http://directory.luton.gov.uk/kb5/luton/directory/adult.page?adultchannel=0Learning Disability Week;Was extremely successful, so thank you to everyone who helped to arrange and attended the many different events.The feedback has all been positive and the media coverage was excellent.Hear my Voice;

Is a Mencap Campaign to encourage people with learning disabilities to take an interest in the politics that affect their lives and to use their vote in the 2015 General Election, this is now active within Luton.

Review of the Commissioning Strategy Delivery Plan:

The RAG ratings on many items was changed from either red to amber or amber to green, none were changed in a negative direction.Priority 1, item 3 was decided that we need to check with Ann Munroe to see what stage this is currently at.

Any Other Business: We would like to invite a transitions nurse from

public health to one of our meetings as it would be very informative.

We need to encourage more carers and service users to our meetings, in order to get more representation for people that actually use our services.

Beth from Healthwatch is to give a presentation, possibly at the next meeting? Also Anthea from the CCG and a representative from the
