Navigate to the subject 1. On the top right of the page, select Turn editing on

2. Navigate to the week / section to which you would like to add a Questionnaire activity and select Add an activity or resource.

Add a questionnaire activityA pop up menu will appear.

1. Select Questionnaire2. Then select Add at the bottom of the menu.

Add a name, description and adjust the timing1. Add the desired Name to the Questionnaire

activity and description (if required – the description will appear on the front page)

2. Add a Description if required. You can select the checkbox to display this on the front page of the LMS page.

3. You can adjust the Timing of the Questionnaire activity by modifying the dates if required.

Edit settings 1. You can adjust the Response options

which include:a. Type: allows you to select

whether respondents respond once, daily, weekly, monthly or many times

b. Respondent Type: allows for named or anonymous responses.

c. Students can view ALL responses: controls when the students can view the responses of other students.

d. Save/Resume answers: allows students to save their answers before moving to the next question.

e. Auto numbering: allows you to alter how the appearance of the questions.

f. Submission grade: allows you to select whether this activity will a gradable item (appears in gradebook).

2. Content options allows you to select from a range of public Questionnaires or Create a new questionnaire.

Adjust/Edit other settingsYou can also adjust the visibility of the activity in Common module settings, Restrict access and Tag as required.

Common Module Settings control whether content in visible to students and allows you to restrict content to student groups.

Restrict Access allows you to control access based on date, grade, users and activity completion.

If Activity Completion has been activated in your subject settings, it will appear in your activity or resource settings. You can elect for a student to mark the activity as completed or for graded items automatically indicate completion to students. Activity completion is indicated by a check box to the right of the activity on the home page.

Tags are not currently used.

Save and return to subject1. Once you have finished adjusting/editing the settings select

Adding questions: Questionnaire activity1. Once you have finished saving navigate back to the

Questionnaire activity then select the Add questions link.

2. Select from the dropdown menu the range of options.3. Then select Add selected question type.

Adding questions (continued)Once you have selected the appropriate question type you can adjust settings and the appearance of the question.The following example relates to the Essay box.

1. Add a Question Name.2. Adjust the size of the response in the Input box size

dropdown menu.3. Insert your essay question in the Question Text area.4. Select Save changes.

Moving questions1. You can move questions around by selecting the moving icon (a pair of up and down arrows).2. Select the position you would like to move the question to. This will happen automatically and doesn’t require saving.

Advanced settings1. The Advanced settings tab/link in the Administration tab

allows you to add a Submission option such as a Confirmation link or a Confirmation page.Note: Heading (in bold) and body text for the "Confirmation" page adisplayed after a user completes this questionnaire. (URL, if present, takes precedence over confirmation text.) If you leave this field empty, a default message will be displayed upon questionnaire completion (Thank you for completing this Questionnaire).

Viewing responses1. You can review responses by selecting the Questionnaire link in the LMS2. Then by selecting the View All Reponses link.Note: Unfortunately there is no notification system in this Questionnaire activity.

Download responsesFrom the Administration block select View All Reponses link.

From here you can select any of the following options:

1. View and reorder the appearance of the Questionnaire.

2. Delete ALL Reponses3. Download in text format.
