Nov. 30, 2018 Alaska Earthquake

I am the Emergency Management Manager at our local

hospital, we held a full scale Earthquake drill. Just a few

weeks later, we fully experienced the Nov. 30th quake. Our

facility did sustain a good amount of damage including loss

of utilities. Staff were fully prepared and they dropped,

covered and held on during the actual earthquake. We had

very few minor injuries. Staff directed patients and visitors,

some staff even covered vulnerable patients to insure their

safety during the earthquake.

I truly believe that it was the education of the ShakeOut that

we provided to staff that insured our overall safety and

allowed us to be better prepared to service our community

and those injured from the earthquake.

Because of the ShakeOut drill…

Most of the staff thought it was safe to stand in a doorway. Because of the ShakeOut drill they were educated on how to drop, cover and hold. Some of the team members had disabilities that kept them from being able to get under the desk. We were able to educate them on how to cover without being able to use a desk/table.

Earthquake Mitigation• Know your risk. Evaluate the chance of seismic

activity in your area. Ensure you have policies and procedures and hold drills and exercises related to earthquake response if it is an identified risk.

• Do a walk-through of your hospital, school, other place of business and even your home and identify heavy pieces of equipment, furniture or other objects that are vulnerable of falling, sliding or colliding. Many injuries in earthquakes are caused by these types of issues.

• If in a healthcare facility, very important to evaluate areas set up for infectious disease surge and management. These may include internal Alternate Care Sites (i.e. tents, converted shelled space) or any location that may have additional equipment (i.e. portable HEPA filters/air scrubbers, IV Pumps outside rooms).

• Plan ahead on how to evacuate and manage potential fires in these alternate care sites and how response may change due to infectious disease.

Earthquake Mitigation• Secure these items with restraints such as straps, anchor

bolts or other methods. Also consider installing latches to prevent cabinets from opening

• Store heavier items on lower shelves instead of higher shelves. This is especially important with hazardous and/or flammable materials. Keep chemicals (i.e. cleaning supplies like bleach and ammonia) separated out that if they mix due to falling could produce a hazard.

• Ensure utility risks are evaluated ahead of time and identify and correct vulnerabilities to water lines, gas and electrical lines. Ensure the testing of generators occurs following set requirements at a minimum (if applicable) to ensure continuing operations.

• Ensure there are multiple redundant methods (i.e. mass notification, call trees, radio, satellite phones, blast emails, social media) for communication with personnel and community.

• Provide not only emergency preparedness training for your place of business, but also personal emergency preparedness training as well, so the family members of your personnel can be ready and safe.

Earthquake Response• If in healthcare facility, direct patients and visitors to drop

to the floor and under a piece of sturdy furniture if possible and hold on. If furniture not available, everyone should get down on the floor and cover head with arms.

• If patients are using mobility assistance devices (i.e. wheelchairs, walkers) instruct them to lock the wheels. If in wheelchair have them cover their heads and if using walker, especially one with a seat, have them get to the floor as low as possible and cover head.

• If in healthcare facility in areas set up for infectious disease surge and management (i.e. tents, converted shelled space) be aware of the risk of the additional equipment (i.e. portable HEPA filters/air scrubbers, IV Pumps outside rooms) that may fall and cause injury.

• If patient is in bed, try to get them to roll to side and cover head with pillow for protection.

• Stay away from widows as may shatter.• If outside, move away from buildings and overhead wires

and trees and sit down with arms over head and neck for protection.

• Always anticipate additional aftershocks!

Earthquake Recovery• Immediately check yourself for injury and your location for

safety. Then, evaluate their area for injured people and damage to facility and equipment.

• If it is safe to do so, check for people who might be trapped in rooms, nursing stations, and other spaces. As rooms are checked, a diagonal mark (tape or marker) could be placed on the door and the door is closed – this means the room has been checked for victims.

• Ensure staff have equipment and resources needed (i.e. PPE, first aid, tourniquets, emergency lighting such as flashlights and/or glow sticks) to remain safe.

• Ensure life saving equipment (i.e. ventilators) are still functional and no tube displacement has occurred and in areas for infectious disease surge equipment ((i.e. portable HEPA filters/air scrubbers, IV Pumps outside rooms) are still functional.

• Everyone must be alert to possible broken glass or spilled fluids/chemicals and risks of fire. Avoid use of anything that may provide a spark.

• Activate your emergency plans, Incident Command Systems if applicable.

Earthquake Recovery• Evacuation may need to be a single unit or may need to be

the entire building. The determination of this is made by risk of safety and by those in charge (i.e. Incident Commander) and agency of authority (i.e. fire department, Office of Emergency Services).

• Important that methods are in place for the accounting of all patients, visitors and personnel during evacuation.

• Have to consider the types of vulnerable populations (i.e. pediatric, neonate, NICU, elderly, etc.) and the methods of evacuation to be used and the potential impact evacuation may have on them.

• Consider safe methods of evacuation of patients that require aspects of isolation (i.e. airborne, contact, droplet, enhanced droplet, etc.) and how these patients will be evacuated (i.e. use of an Isolation Pod or just with a mask) to protect others, locations on where to stage them and how staff will be protected (i.e. Powered Air Purifying Respirators, Enhanced Precautions, etc.) while evacuating and providing care.

Earthquake Recovery• Depending on the size, nature of the earthquake damage,

recovery could be rapid (i.e. cleaning up minor debris and spills, minor first aid) to long term (i.e. building repair and reconstruction).

• Safety is a primary issue, as are mental and physical well-being.

• Direct assistance can be provided to you in regards food, shelter, supplies and assist in clean-up efforts by your county Health Department and volunteer groups like the American Red Cross and Salvation Army

• If applicable due to the incident, contact you Insurance Agency as soon as possible to assist in the recovery and repair of insured items.

• In the most severe disasters, the federal government is also called in to help individuals and families with temporary housing, counseling (for post-disaster trauma), low-interest loans and grants, and other assistance.

• Be aware of new safety issues created by the disaster such as collapsed roads/bridges, gas leaks, broken glass, damaged or downed electrical poles.

Earthquake Recovery• Recognize signs of disaster related stress such as difficulty

sleeping, limited attention span, headaches/stomach problems, disorientation or confusion, depression, feelings of guilt, sadness, feelings of hopelessness.

• Stress can manifest in the general population, but very much so in our responders (i.e. medical personnel, EMS, Fire, Law Enforcement and more)

• It is normal to feel anxious about your own safety and that of your family and close friends.

• Acknowledging your feelings helps you recover.• Focusing on your strengths and abilities helps you heal.• Accepting help from community programs and resources is

healthy.• Children are of special concern in the aftermath of disasters

as they may not process what is occurring. Disasters can leave children feeling frightened, confused, and insecure.

• Whether a child has personally experienced trauma, has merely seen the event on television or has heard it discussed by adults, it is important for parents, grandparents, guardians and teachers to be ready to help if reactions to stress begin to occur.

COVID-19 Earthquake Planning Considerations

• Know your facility, school, or organization buildings and how you are serving your constituencies now related COVID-19

• Know your constituencies (staff, patients, students, etc.) and how you are addressing their COVID-19 needs and how an earthquake would further impact them

• Use the Great Shakeout Earthquake Drill to test your emergency evacuation or shelter in place plans during COVID-19

• Plan for the long-term effects of an earthquake impacting your building should it be damaged to serve your constituencies during the COVID-19 pandemic. Do you have an alternate location identified?
