Your Project Goals & Objectives
• Stay Focused
• Identify the Factors leading to the integration of an EMR
• What Benefits are expected from this integration?
• Expectations of the system and project and the stakeholders?

• Constantine Papasavvas MA• Manager of Clinical Technology• M3 Meridian
Interfaces/Integration
•Improving patient care
through data exchange by
merging external data.
Next Steps
• Demographics
• Scrub the data for uniqueness
• Identify any issues
• Formats through HL7 – pushed data to the
EMR
• Old System Capabilities
• Import Options
Demographics
Source System
Insurance
Duplicate Resolution
Keys - tie things together
Simulation
Registration Update
What needs to be converted?
• Immunizations
• Growth Data
• History (Social, Family, Medical, Surgical)
• Problem Lists
• Allergies
• Medication
• Scanned Items
Capabilities of Old EMR System
• Paper print image
• CCD, CCDA
• Flat File
(like a Spreadsheet /Columns identified)
• HL7 Message Types
Import Options
• Manual Enter - screen to screen, system to system
• Scripting – Simulate User from a file
• OCR Scanning – print image manual import
• HL7 Message, OBR
– a desired method
– OBR – observation result
• CCD Import
– CEMR allows for CCD reconciliation
• 3rd Party application for presentation
• Direct updating of database table
• Other
Imported Data
Observations Captured
CCDA – xml version control
• How to import
• Importing the file XML File
• Convert into a format the version EMR
understands
• When attempting the simulation, grab the
worst case CCDA we could find
• Removed the attributes or the elements
that had bad stuff on them.
HL7 - Revisit
• The “Managing Interfaces with Centricity
EMR” documentation does not need to be
difficult.
• Third party software will make your life
much easier.
• Be willing to try different things.
An Interface Engine is Crucial
• An interface engine will allow you to accept HL7
messages that do not conform to EMR standards
and translate them to something the EMR can
import.
• EMR versions come packaged with Centricity
Clinical Gateway with Cloverleaf, there are also
various third party options available. Plan to
implement one ASAP.
CCDA Sample
• The “Managing Interfaces with Centricity
EMR” documentation does not need to be
difficult.
• Third party software will make your life
much easier.
• Be willing to try different things.
HL7 OBR Sample
• The “Managing Interfaces with Centricity
EMR” documentation does not need to be
difficult.
• Third party software will make your life
much easier.
• Be willing to try different thin
CCD Example
Results / Common Issues
Common Issues
• Effort hours for each option
• Correct Chart chosen
• Versioning of electronic version
• Presentation
• Validation of results
• Documents vs. discrete data elements
Items we encountered
• Patients not found
• Patient Duplicates
– Good demographic conversion should prevent
• Capabilities of source EMR
• Version of CCD
• Converting into electronic version
CCD ERRORS
Road Blocks
• Vendor supplying the data
• Getting the physicians to agree with
process
• Getting everyone on the same pages
• Scheduling the meetings
• Meeting Attending and follow through
Identify and Prioritize Integration
• Prioritize integrations that benefit the entire
group and not those that those that only
benefit one clinic or physician.
• Identify and engage key personnel involved
with each integration.
• Create a project plan for each project and
distribute to all parties involved.
