Here’s what you will need:1. Applicant Details: Name, Email and Phone Number

2. System Owner Details: Name, Email and Phone Number

3. Dealer or Installer Information: Name, Address, Email, Phone Number, Website (if Available)

4. System Information: Purchase Date, Module Model Number, Number of Modules Installed, Serial Number (10 Numbers + 1 Letter), System Installation Address, Project Size in kW, Inverter Type or Make, Utility Company to which the system is con-nected, Estimated Yearly Energy Production, Residential or Commerce, Connected to the grid and Energy Monitoring System.

5. System Information: Purchase Date, Module Model Number, Number of Modules Installed, Serial Number (10 Numbers + 1 Letter), System Installation Address, Project Size in kW, Inverter Type or Make, Utility Company to which the system is con-nected, Estimated Yearly Energy Production, Residential or Commerce. Module Model Number:

The Module Model Number can be found on the product label located on the back of the module.

Module Serial Number:The Module Serial Number is 11 digits (10 Numbers + 1 Letter) and can be found on the top corner or frame of the module, usually on a white rectangular sticker. Contact your installer should you have any questions.
